2 Progeny FHQ Configuration Menu 1. Folder Selection a. This is where you will choose the questionnaire folder you wish to configure. You can configure an unlimited number of folders for FHQ, but each folder can only be configured for one type of FHQ. 2. Configuration Tabs a. Main Config i. This section houses the base-level configuration for the FHQ assigned to the selected folder b. Skip Logic i. This is where you will configure the skip logic workflow for the Proband section of the FHQ c. Relatives i. This is where you will configure the skip logic workflow for the Relatives section of the FHQ (also referred to as the Family Table) 3. Family Table Fields a. Once you load a Relatives Datasheet format, you will add the fields you wish to be on the Family Table by using the Add button. i. Note: you do not need to add the First Name field this is configured within the Main Configuration Options and is automatically added to the Family Table. 4. WDC Link a. This field displays the configured URL for the FHQ assigned to the selected folder. 5. Main Configuration Options a. The primary configurable elements for the FHQ see below. 6. Configuration Option Details a. This will display the fields that can be configured for each of the Main Configuration Options. Page 1 of 6

3 Main Configuration Options Explained Enabled for WDC Questionnaire enabled for folder if checked Detail Button Label If the Show Add Detail Button Each Row option is checked, this option allows you to replace the default text (Add More Detail) with custom text Invite Message This is the initial patient invitation. For Progeny to use the custom-generated link to your FHQ that binds the patient s address to a specific pedigree, please insert [$FHQLINK] into the text where you want the link to appear o In the generic FHQ demo database Progeny uses for pre-sales demonstrations, this link is hidden behind a text hyperlink. Here is an example of the HTML used for that <a href="[$fhqlink]" target="_blank">» Click here to begin your Online Family History Questionnaire</a> Invite Subject This is the subject line for your invitation You will need to use plain text formatting for this option. Submit Message This is the final sent to the patient as a thank you. Unfinished Message This is used as a reminder if a patient begins the questionnaire but does not complete the entire questionnaire process Page 2 of 6

4 to Notify This is the internal address where FHQ notifications will be sent. Considering that many Progeny users might need access to these notifications, you should use a distribution list address for best results. Exit URL This is the URL (webpage) that the patient will be directed to upon exiting the FHQ application. External URL If the URL you want to use for the FHQ is not the URL assigned to the web server you are using, you can enter the desired URL in this option to have all questionnaire invites use it instead of the URL assigned to the server. This option is most commonly used when the Progeny users are accessing the Client from a server behind the firewall while the patients are accessing the FHQ from a server in front of the firewall. External URL Used Progeny Web will not use the External URL configured in the option above unless this box is checked. Family Table Headers If checked, this option will force the fields displayed on the Family Table to use custom-defined field headers instead of the default field name. This will allow users to configure a shorter or simpler field name to be shown to the patient while retaining their existing field naming scheme. Family Table Cell Highlight If checked, the active cell on the Family Table will be always be highlighted o We recommend that all users turn this option on. Family Table Instructions This option allows you to specify custom instructions for the Family Table screen. Page 3 of 6

5 Final Page Instructions This option allows you to specify custom instructions for the Final Page screen. First Name Field This is the first name of the individual being presented in the questionnaire. It is also used to identify relations within the Family Table section of the questionnaire. Help Fields This option allows you to define an address and contact number that the patient can use to seek assistance with the FHQ. A custom Help Message can also be created in the lower box. for the Help Message. Interview First If you want your FHQ to start with the Family Table and transition to the Proband questionnaire (reversed the default order of the FHQ), you can check this box. o We do not recommend using this option if you have the skip logic engine enabled. Login Logo Height This is the pixel height of the logo used throughout the questionnaire. This value must be an integer. Login Logo URL This is the URL location of the logo image used on your FHQ login screen (will also appear in the upper-left corner of the FHQ pages while the patient is logged in.) There are /gif/ or /png/ folders already in your Tomcat ROOT folder for easy storage of your logos. When configuring this option, use the ROOT folder as your starting point. Your image will be referenced similar to the following: o /png/logo2.png Login Title This is the title line for your FHQ login page. You will need to enter this using plain text. Page 4 of 6

6 Login Instructions This allows you to define a custom message for the login screen. It is most often used to welcome the patient, explain the overall purpose of the FHQ or to provide specific login instructions.. Next Button Image URL If you want to use another image (other than the default green arrow) on the next button within the FHQ, you can specify the URL of the file here. o This option should not be used with in conjunction with the Version 2 UI option below, as the Version 2 UI does not use these buttons. PNG files are recommended Patient Field This option identifies the address field in your database. An address field is required for use with the FHQ, even if you do not intend to use the invitation feature. Pedigree Name Fields This allows you to have Progeny automatically rename the pedigree upon completion of the FHQ using the values contained within these fields. You can select up to two (2) fields for this and can use computed fields. o This option is best employed when directing patients to the FHQ without sending an invite through the application. By default, these pedigrees will be named using the address entered. For example: johnsmith@ .com will have a pedigree called johnsmith@ .com s family We do not recommend using this option when planning to use the invite feature you will be required to create and name the pedigree before sending the invite. Pedigree Page View This option determines whether the final page of the FHQ will display the pedigree in full screen mode or split the screen with the pedigree on top and the relative data on the bottom. o This feature is disabled when the Version 2 UI option below is checked. The Version 2 UI has an entirely different final page. Page 5 of 6

7 Questionnaire Type This option should always be set to Family History Questionnaire unless you want to set up the Individual Follow-Up Questionnaire. The Individual Questionnaire strips out the Family Table section of the FHQ, allowing you to focus entirely on the patient. Relatives Datasheet This is the datasheet format for the relatives datasheet. Format is emphasized because you cannot select the pedigree containing the datasheet in this option with the datasheet open, you need to click the Save Fmt button at the top to save it as a datasheet format. This is what Progeny will read when loading the Family Table section of the FHQ. The selected datasheet format also determines which fields can be added to the Family Table Fields section of the FHQ configuration. Show Add Detail Button Each Row If checked, this will place an Add Detail button on each row of the Family Table. o We do not recommend using this option in conjunction with the skip logic engine as it will create unnecessary redundancy. Use Version 2 UI If checked, this will enable the 2.0 display format of the FHQ. o If your questionnaire was designed before the release of the 2.0 display format, you will likely need to revise your questionnaire layout to conform to the new display restrictions.
