2 Provider Portal Access If you haven t received notification to create your provider portal account, please providerportal@communitycarecw.org To most effectively support you please provide the following information in your 1. Your first and last name 2. Agency Name 3. address of individual needing access. 4. If there are multiple sites, please confirm which sites you need access to in order to complete your billing.

3 Provider Portal Account Set Up Once you have been added as a staff and website user for your specified provider you will receive an from E- Service User that appears like this:

4 Provider Portal Account Set Up The second link will allow you to establish a password for the first time. (There are no password requirements at this time)

5 Provider Portal Account Set Up Once you have entered your password and confirmed it you will be informed that the password was reset.

6 Provider Portal Account Set Up This link: will bring you to the Provider Portal Homepage which can be used for future use.

7 Provider Portal Account Set Up Enter your and newly created password to enter the portal

8 Provider Portal Homepage Your provider portal homepage will appear like this:

9 Hierarchy of Portal Set-Up Parent (CCCW) Access to all subproviders Sub-Provider (Stevens Point) Sub-Provider (Superior) Sub-Provider (Park Falls)

10 Provider Detail The parent provider is the account listed first and outlined in RED Provider details are specific to the account you are viewing Click on provider detail under the Parent account to view Parent details Click under the sub-provider account to view sub-provider details

11 Service Authorizations Click on Authorizations

12 Service Authorizations You will notice when you select Authorizations from your portal homepage you will receive an electronic signature message, prompting you to select ok (see the example below). Once you ve selected ok you will be taken to the authorizations page.

13 Service Authorization The Look Once you have selected Service Authorizations you will come to a page that highlights: Member ID First Name Last Name Middle Name Date of Birth Authorization ID Authorization Type Status Start Date End Date Service HCPC Code Modifier Modifier 2 Modifier 3 Modifier 4 Frequency Units per Frequency Rate per unit Auth Amended Created On

14 Service Authorization The Look You can search for a particular authorization by using the search box and then press enter. Search options include: All, Date of Birth, HCPC Code, First Name, Last Name, or Middle Name. If searching by name, use first or last (not both). You can sort all authorizations by clicking any of the subtitles in green. The Created On will be the primary view.

15 Service Authorization The Look Please note that the default view is set to Open Service Authorizations. This view can be changed to All Processed Authorizations, Closed Purchasing Authorizations, Closed Service Authorizations, and Open Purchase Authorizations.

16 Service Authorization The Look You have two options to view your authorizations 1. Export to Excel (See details on slide 17) 2. Authorization for Services (See details on slide 18)

17 Service Authorizations Export to Excel Click on a box (or multiple boxes) in front of the Member Id number. Then select Export to Excel. You can also export all authorizations to excel by simply selecting Export to Excel.

18 Service Authorization Authorization for Services Click on box (or multiple boxes) in front of the Member Id number. Then select Authorization for Services. You will receive a message that a report is being generated. You will be prompted to open or save, click on open, authorizations will open as a PDF. You can then go to file and print.

21 Adding Staff From the provider homepage select the provider account you wish to grant additional staff access to The parent provider is the account listed and outlined in RED Granting access under the parent will give the staff access to all sub-providers Granting access from one of the sub-providers will only give access to that provider Click on the STAFF MEMBERS button

23 Maintenance of Provider Staff After staff are created they will receive an to activate their account and create a password The provider primary contact will be responsible for maintaining staff access to the portal Staff must be deactivated in the portal if they have left the provider organization
