1 How to book an appointment to collect HPB Fitness Tracker For New Participants & Returnee Step 1: Enter NRIC Enter your NRIC number. Please note that you are able to book a new appointment only if: 1. You have not booked an appointment before on this website 2. You are eligible for a HPB-issued fitness tracker [Please see Eligibility Conditions under Terms and Conditions Step 2: Book an appointment You will be informed that you are a New Participant or Returnee. Click Book an appointment button. Read, acknowledge and accept the website s Terms and Conditions. Step 3: Enter your particulars Enter your NRIC number, address and full name (as per NRIC) to register for a new appointment. Collecting for your family members/friends (optional) You are able to collect the HPB fitness trackers for up to 2 family members/friends. Click + button to add new participant. Enter new participant s NRIC and full name (as per NRIC). Note: You are only able to collect HPB fitness trackers for participants with the same status. E.g. Returnees can only collect for Returnees. 1

2 Step 4: Select collection venue & appointment timeslot 1. Select preferred collection region 2. Select collection point under the chosen region (Click here for the appointed collection points) 3. Select preferred collection date from the calendar 3. Select preferred collection timeslot 4. Press Next to proceed Step 5: Verify your appointment Verify and confirm your appointment details: 1. Participant s details NRIC Full Name 2. Collection details Chosen Post Office (address shown) Appointment Date Appointment Time Click Submit to confirm your appointment. Step 6: Your appointment is confirmed! Click on the link (as circled in red) to access the appointment confirmation letter. Alternatively, it will also be sent to the address that you have provided. If you did not receive the , please look through your junk folder. Please print and bring along the appointment confirmation letter to your designated collection point when collecting. 2

3 How to book an appointment to collect Completer s Kit For Completers Step 1: Enter NRIC Enter your NRIC number. Please note that you are able to book a new appointment only if: 1. You have not booked an appointment before on this website 2. You are eligible for a limited edition Completers Kit i.e. you have completed all 6 tiers in previous seasons of NSC and have not collected a Completers Kit in NSC3/2017. [Please see Eligibility Conditions under Terms and Conditions Step 2: Book an appointment You will be informed that you are a Completer. Click Book an appointment button. Read, acknowledge and accept the website s Terms and Conditions. Step 3: Enter your particulars Enter your NRIC number, address and full name (as per NRIC) to register for a new appointment. Collecting for your family members/friends (optional) You are able to collect the HPB steps trackers for up to 2 family members/friends. Click + button to add new participant. Enter new participant s NRIC and full name (as per NRIC). Note: You are only able to collect the Completers Kit for participants with the same status. E.g. Completers can only collect for Completers. 3

4 Step 4: Select collection venue & appointment timeslot 1. Select preferred collection region 2. Select collection point under the chosen region (Click here for the appointed collection points) 3. Select preferred collection date from the calendar 3. Select preferred collection timeslot 4. Press Next to proceed Step 5: Select Completer s T-Shirt Size 1. Select your Completer s T-shirt size 2. If you are collecting on behalf of other Completers, select T-shirt size for Participant 2 and Participant 3 Step 6: Verify your appointment Verify and confirm your appointment details: 1. Participant s details NRIC Full Name 2. Collection details Chosen Post Office (address shown) Appointment Date Appointment Time Click Submit to confirm your appointment. 4

5 Step 7: Your appointment is confirmed! Click on the link (as circled in red) to access the appointment confirmation letter. Alternatively, it will also be sent to the address that you have provided. If you did not receive the , please look through your junk folder. Please print and bring along the appointment confirmation letter to your designated collection point when collecting. 5

6 How to authorize collection on behalf If you are collecting the fitness tracker or completers kit for someone, you are required to: a) bring along the printed copy of the appointment confirmation letter b) fill up the authorization section (found at the bottom section of the confirmation letter) c) ensure that the actual participant signs on the authorization section You will not be able to collect the fitness trackers or completers kit without the confirmation letter and / or the completed authorization form Sample appointment confirmation letter 6

7 How to change your appointment details For All Participants Step 1: Change details of appointment After entering NRIC on the main screen, hover over Change details of appointment tab and select either: 1. Change appointment location, date & time 2. Add/Remove participants 3. Cancel appointment for all participants Read, acknowledge and accept the website s Terms and Conditions. Step 2: Enter your details Enter the address that was used for appointment booking. Confirm all participants particulars and click Next. Option 1: Change appointment location, date & time 1. Your existing appointment details will be shown at the top 2. Proceed to select a new preferred location, date & time Note: By changing collection venue, date and/or time, it applies to all the trackers booked under the same appointment booking. 3. Press Next to confirmation page 4. Once confirm, a new appointment confirmation letter will be sent to you. 7

8 How to change your appointment details For All Participants Option 2: Add/Remove participants Click + button to reveal new participant field. Enter new participant s NRIC and full name (as per NRIC). Note: You are only able to collect HPB Fitness Trackers for participants with the same status. E.g. Returnees can only collect for Returnees. Note: If you are collecting the steps tracker on behalf, you are required to bring along a printed copy of the appointment letter with the authorisation section completed by the actual participant. Click Remove button to remove participant. Option 3: Cancel appointment for all participants To cancel the appointment for all participants, enter your details and click Proceed to Cancel to cancel appointment. Note: By canceling the appointment, it applies to all the participants booked under the same appointment booking 8
