3 1. Language selection After installing software, Polish is set as the default language. The language of the descriptions can be changed to English. To perform this action, select button Ustawienia (settings) from the toolbar, choose Język (language) from the list and then select Angielski (English). Note: the language will be changed after reboot. Close the program and open it again. 2

4 2. Launching the program, analysis selection Use the icon New project to start work with the program or select it from File New project. Analysis type selection pane will be displayed. 1 2 Select the type of analysis: 1 Process FMEA (PFMEA) 2 Construction FMEA (DFMEA) 3

5 After launching the chosen option, a window with the failure tree will open. 4

6 3. Project data After choosing the type of analysis, you can define Project data. To enter data, select the button Project from the toolbar and next select Project data from the list (The function is available after choosing the type of analysis P-FMEA or D-FMEA). A fillable dialog box will appear. In the tabs you can enter data for FMEA, CP, PF. At the bottom, in the first tab (FMEA) you can add a company s logo. To perform this action click the button Browse and then choose a file from any location on your computer. The logo will be placed in the FMEA form (the bottom left corner of the document). 5

7 4. Risk matrix The program allows you to develope a risk matrix according to a company s needs. (The function is available after choosing the type of analysis P-FMEA or D-FMEA). To adjust a matrix, select Settings from the toolbar and choose Risk Matrix Template from the list. Click on a box to edit it and adjust accordingly to the risk level (green, yellow, red). The list of colour selection will appear after clicking on a box. 6

8 5. Assessment criteria (S, O, D) In the program there are assessment criteria for Severity (S), Occurence (O) and Detectability (D). (The function is available after choosing the type of analysis P-FMEA or D-FMEA). Default data can be edited. To perform this action, select Settings from the toolbar and then choose criteria you wish to edit from the list.. In the dialog box enter desired text. All the boxes are editable except for scoring (1-10). Note: after using edit function in settings tab, tables will be saved In the program. If you want to change criteria description for a project, use the function Project from the toolbar. 7

9 6. Creating failure tree To maximize failure tree box, click the button (1). 1 Creating cells on failure tree To start creating a tree, drag the cell product (by clicking on it and holding) to the field Prosess phase (in PFMEA) or Product element (in DFMEA). 8

11 7. Deleting cells from failure tree Deleting cells is creating them process in reverse. Click and hold a cell you want to delete and drag it towards left. The cell must be placed on the white field of the previous column. Note: each cell is removable, except for the cell product. 10

12 8. Editing cells on a failure tree To edit a cell, double-click on it. A new dialog box will appear. 11

13 Enter text in the dialog box and confirm the change by clicking OK button at the bottom of the dialog box. 12

14 The text will appear In the edited cell. Each cell can be edited in the same way as described above. During editing Process Phase an additional row appears Machine, device, source. Write there a name of a machine, device, source which is used to perform a certain action. Data from a failure tree are automatically transferred to FMEA, CP, PF form during creating them on a tree. 13

15 Accordingly to a process phase, when editing a cell in the column Function, Associated characteristic box will appear in the dialog box. Describe product characteristic, which automatically will be transferred to CP and PF (section CP and PF). The Failure mode field can be edited in the same way as other columns. 14

16 Another field where data for CP and PF should be entered is Cause. A box Associated process characteristc will appear in the dialog box. Note: If product characteristic is defined in Function, it can t be defined in Failure mode, if process characteristic is defined in Function, it can t be defined in Cause and other way round. 15

17 9. Moving cells up and down the tree To move a cell above or below another one, click (and hold a cell) and drag it above/below a cell you want to relocate to another place on a tree. Remember to place a cell above/below auxiliary line (1), which will appear while moving a cell. 1 16

18 Note: cells can be moved around only within one branch. 17

19 10. Copying tree cells If there is a need, you can copy previously created cells together with their branches. Click a cell (1) you want to copy and drag it to a preceding cell (2) until it is highlighted in orange and release the button

20 As a result of copying another identical tree branch stuck on a new place is created. 19

21 Cells can be copied within different tree branches. If you decide that a cell from the column phase, function, failure mode, cause is needed in a different place on a tree, you can copy it 20

22 Copying is also possibile using the option Copy. Right-click on a cell, wchich you want to copy and select Copy form the list. Next, right-click on a cell to which you want to add a copied cell and select Paste from the list. 21

23 11. Folding and unfolding tree branches Right-click on a cell located on the level on which you wish to fold a branch and choose the option Fold. As a result of folding, a dotted line will appear, marking the place where cells are folded. You can fold all the groups altogether. 22

24 You can fold columns Process Phase, Function, Failure Mode as a group. Right-click on the white space in a column, which you want to fold, next choose Fold all the elements in this column from the list. You can perform the same action on a cell level in each column. 23

25 As a result of the action presented above, all branches right of a chosen column will be folded To unfold whole column, right-click on the white space on a column or on a cell and select Unfold all elements in this column from the list. 24

26 12. Opening tree branches in tabs (New View) Each branch can be displayed in a new tab. Right-click on a cell and select New view from the list, to display a branch in a new window. After selecting New view from the list, a new tab containing chosen branch will appear at the top left corner. 25

27 Copied cell is moved together with the part of interrelated FMEA form. Creating new views (tabs) is similar to the process described above. You can create any number of tabs for each cell. Note: new views are only graphic presentation of the part of conducted analysis and all changes made in those partial views will influence basic analysis. 26

28 13. Zooming in and out failure tree in a window Push and hold Shift on your keyboard and use Scroll function in your mouse to change size of a displayed tree. 27

29 14. Working with FMEA form Click on the button (1) at the bottom left corner of the window to restore Form and Tree view 1 Use the same button to maximise the form window. 28

30 15. Fillig out the FMEA form The FMEA form is partially filled out with failure tree data. Double-click on a chosen cell to edit the form. A dialog box will appear after double-clicking a chosen cell. Write desired text in the window. Next, confirm the entry by clicking OK button. The text will appear in the chosen cell. 29

31 If there is a need to define more e.g. potential failure effects, click the button Add (1) in the dialog box. A new editable box will appear. (with or without content). Click on X button located at the right of the dialog box (2), to remove redundant editable box 30

32 While adding individual cells, prompts with previously used (within a chosen column) uterrances unfold in the dialog box. Double-click on a prompt to select it. Prompts can be edited. In the Project menu, choose Edit prompts option. Columns: Failure Cause, Preventive actions, Detection actions, Recommended actions are edited in a similar to described above way. 31

33 There is an additional option for columns Preventive actions, Detection actions, that allows to add defined actions to a failure (to all causes interrelated with the failure[1]). The Assign to failure mode allows to assign defined actions to all defined causes for a failure Note: all actions added with this mode will appear in blue. Deleting one of the actions will result in removing all the interrelated ones. 32

34 16. Assessment Severity (S); Occurence (O); Detection (D) Scores in the form are assigned through a dialog box containing an editable score list. To assign score, double-click on a cell in a chosen column: Severity (S); Occurence (O); Detection (D). 33

35 A dialog box will appear after double-clicking on the chosen cell. 34

36 There are two score tables in the Severity dialog box. Severity for the client (user) and Severity for the process (production/assembly). Switching between tables is possible by selecting a table at the top of the dialog box. After clicking on a chosen cell in a kolumn, a dialog box for scoring appears. (Assessment citeria S, O, D) Numbers for S, O, D can also be entered without the dialog box, using keypad. Selecting 0 from your keypad will result in entering 10 in the scale Note: scoring tables are editable, you can enter your own scoring descriptions for each table. After scoring S, O, D, the RPN is calculated automatically. (Assessment criteria S, O, D) 35

37 17. Adding special characteristics (Classification) Double click on a box to which you want to assign special characteristic (symbol or letter) (1). A dilog box, allowing symbol (saved in the program) (2) or letter (3) selection, will appear. If there is a need to delete an entry, select Unspecified (4) option Note: The program allows you to add your own symbols. They will be displayed in the dialog box. To add new characteristic symbols, upload graphic files to classification folder, in the location where the program is installed on your computer. 36

38 18. Saving actions in the Recommended actions column Enter actions in Recommended action cells directly in the form. Double-click on a cell. Next, enter an action. 37

39 If there is more than one action, use the button Add after entering the first action. Another editable box for the failure will appear. Actions can be assigned to Control or Prevention, using buttons located right of the dialog box. Prevention, Control. Click on to remove unnecessary box (empty or with entered action) for Recommended action. 38

Assigning people responsible for actions and dates of completion in Responsibility And Planned Completion Date Double-click on a cell in the Responsibility And Planned Completion Date column to assign a person to a previously defined action and to set a date of completion. An editable dialog box will appear after double-clicking on a selected cell. 39

41 If an employee list had not been defined previously, it can be edited from the dialog box. Click on Edit employee list. An editable box will be displayed. Enter desired information in boxes and next, click on the Add button. 40

42 After clicking on the Add button, the employee data will be entered in Employee list in the program. The data will be automatically displayed also in the Responsibility And Planned Completion Date box. Click on the OK button at the bottom of the dialog box to confirm. Repeat the action to add other employees to the list. There are Export and Import functions in the program. You can export/import employee lists. After creating an employee list you can save it in a file. A list can be imported each time you work with a new or different analysis. Use Export, Import buttons (in the Employee list dialog box) to perfom this action. Double click on an employee data bar or click, hold and drag it to <Drag to here> box, to assign an employee to an action 41

43 After assigning an action to a responsible person, it is necessary to define a date of completion of an action implementation. To do that, unfold the calendar using the button at the bottom right corner in the dialog box. After selecting a date, confirm it with the OK button. Data will be entered in the form. 42

44 20. Saving actions in Actions taken/completion date To make any changes in the Actions taken/completion date boxes, double-click on a chosen cell. An editable dialog box will appear. Enter actions taken, then select a completion date and confirm it by clicking OK button. 43

45 21. Calculating RPN before finishing action implementation If you want to calculate an assumed RPN level before implementing actions, you can do it in a similar way to the scoring described in Assessment Severity (S); Occurence (O); Detection (D). After clicking on columns, tables similar to those in the aforementioned section will appear. After estimating assumed RPN level, it will appear in red in brackets. After implementing actions (after completeing Actions taken and Completion date box), calculated RPN will appear without brackets. 44

46 22. Displaying row numbers The program allows you to number rows. Choose View from the toolbar and select Display row numbers. An extra column Row will appear in the form. 45

47 23. Highlighting causes The function allows user to define cause highlight. Choose View from the toolbar and next, select Highlight causes from the list. You can highlight failure causes using this function. You can enter value for S, O, D, RPN above which failures will be highlighted. E.g. for RPN > 600 all Failure causes above this value will be highlighted. 46

48 Similarly to RPN, you can set values for S,O,D E.g. if you set OCC on level 8, all Failure causes above this value will be highlighted. You can also use the highlight option Only special characteristics (only failure causes with assigned special classifications will be displayed) or the Hide remaining causes option. Select a chosen option. 47

49 Highlight color can be edited. To change the highlight color, click the Color box. A new dialog box for color edition will appear. 48

50 The is a variety of colors to choose. During using this function, the information Filtering is active will be displayed at the top of the form. To turn off filtering, select View from the toolbar and from the dialog box, select Disable causes highlight. 49

51 24. Visibility of phases You can filter phases which you want to be visible during your work with the program (in a form and on a tree). Choose Project from the toolbar and click on Visibility of phases. A phase selection dialog box will appear. 50

52 Unselect phases you don t want to display and leave selected those you wish to display. Confirm by clicking OK. The program allows you to save your settings (visibility of phases). Click Save... to save your configuration. A dialog box, in which you ought to enter a configuration name will appear. 51

53 Settings will be saved in the Saved configurations box. You can remove saved settings using the Remove button. Select a configuration (so it is highlighted in blue) and use the Load button to load a chosen configuration. 52

54 25. Pareto Analysis The pogram allows user to generate Pareto analysis for RPN value. Choose Statistics from the toolbar and then select Pareto Analysis. A new dialog box with Pareto Analysis will be displayed after choosing this option. 53

55 You can diplay a planned RPN in the dialog box. Select Show planned RPN at the bottom of the dialog box. Planned RPN will appear in yellow. You can also sort RPN acording to planned one. Select Sort according to planned RPN. 54

56 26. Generation of tasks list After assigning tasks to responsible employees, you can generale lists of tasks for each employee (previously defined in the form as responsible for a task). Choose Statistics and then Tasks, to generate lists A dialog box with an employee (defined as responsible for tasks) list will be displayed. To choose list for an employee, double click on a chosen name. A list of tasks for a chosen employee will be displayed. You can print it. Print 55

57 27. CP Control Plan To move to CP edition, choose the button located on the toolbar, at the top left corner Data previously entered in FMEA, will be automatically moved to the CP form. (8. Editing cells on a failure tree). Note: Data will be trasferred to CP after defining product or process characterictics. 56

59 Characteristics defined on the failure level, will be moved from FMEA form to the Control Method column. If you define product characteristic on the function level in FMEA form, all next saved controls will be assigned to a product. Firstly, unselect assign to failure. 58

61 If you want to assign control to a process characterisic, do not assign it to a failure. 60

62 Double-click on the space in the Classification column to assign classification to any row. This action allows you to assign a symbol (1) or letter (2) regardless FMEA form. If you want to keep previously assigned classification, select From FMEA (3) in the dialog box Other boxes in the form are editable. Double-click on a box to edit it. 61

63 The program allows you to add pictures to the form. Double-click on Product (1) or Process (2) to add pictures. 1 2 A dialog box allowing adding pictures will appear. Browse file selection load confirm OK. To remove a picture, double-click Product or Process (depeneding which Picture you wish to remove) and choose Remove button in a new dialog box. The program allows you to print CP or export it to an Excel file. Choose File from the toolbar at the top of the window and next select Print CP (or Export CP to XLS). 62

64 28. PF Process Flow To move to PF edition, choose the button at the top left corner Data previously entered in FMEA, will be automatically moved to the CP form. (8. Editing cells on a failure tree). Process and product characteristics will be exported to suitable form columns. 63

65 To establish proceeding and succeeding process phases, double-click on a field you want to edit. A dialog box allowing you to assign proceeding/succeeding phases will appear. The program allows you to add symbols marking a process phase. Double-click on a box in the Symbol column. Choose symbol in a dialog box and confirm with OK button. Note: The program allows you to add your own symbols. They will appear in the dialog box. To add new symbols, upload graphic files to pfsymbols folder, in the location where the program is installed on your computer. 64

66 Phase description is editable directly from the form. Double-click on a box in the Description column to edit it. The program allows you to print PF or export it to an Excel file. Choose File from the toolbar at the top of the window and next select Print PF (or Export PF to XLS). 65

67 29. Toolbar description Opens a new project after launching the program. Employee list. You can add employees to a list with this function. The list is used to assign employees to tasks in the form. (Section 19) Opens previously saved project. Sets windows vertically. Saves changes in a project ( saving during or after making changes in a project). Sets windows horizontally Performs previously undone action. Switches to form view. Switches to tree view.
