2 I. RTOG 1016 CASI Survey Application To start the CASI survey application, tap the RTOG 1016 icon on the desktop. A. FIRST STEPS BEFORE ADMINISTERING SURVEYS TO PARTICIPANTS 1. Establish a clinical research associate (CRA)/investigator user account. Participating institutions must register into the CASI system for access to computer software and hardware for administration of the following: 1) the mandatory demographic and tobacco survey for all participants; 2) the optional quality of life (QOL) Patient Reported Outcomes assessments; and 3) the optional head and neck cancer risk factor survey (BRASS). A CRA/investigator user account will provide you with administrator access to the ipad CASI system. All RTOG 1016 study site personnel who will administer surveys should have a CRA/investigator user account. Users must have an RTOG (or CTEP IAM for CTSU sites) user account. For sites participating through the CTSU, the site registration status of RTOG 1016 must be active and persons requiring access to the ipad CASI system must have an active site administrator, CRA, or DA role on the CTSU roster. After the RTOG 1016 study is activated at a site, site personnel should go to the RTOG@OSU homepage: Click the Register New User link at the top right of screen to create a user account for the ipad CASI system (see figure 1). Personnel should enter the same username and address that is used to register onto the RTOG website (for CTSU sites, use your CTEP IAM username and associated address), and then click the Initialize Account button. Your RTOG username will be your username for the ipad CASI system. Fig. 1. 3 After clicking the Initialize Account button, CRA/investigator users will receive an from RTOG OSU (RTOG1016@osumc.edu) directing the individual to click on a link to complete their account activation. The link will direct the user to the Activate Account webpage (figure 2). 1) The user s RTOG username (or CTEP IAM username for CTSU sites) and work phone number should be entered (fig. 2, a). 2) The user will be asked to create and re enter a password (fig. 2, b). This is the password that will be used to log into the ipad CASI system (in conjunction with the RTOG/CETEP IAM username). I t must be 8 15 characters with at least one number or special character (a prompt outlining these requirements will be displayed for passwords not meeting these criteria). 3) The user must select and provide an answer to a security question (fig. 2, c). The security question will be used to reset a user s password in the event it is forgotten. The Forgot Password link in the top right corner of the RTOG OSU webpage can be used to initiate this process. 4) The user MUST select Agree to the ipad Terms of Use (fig. 2, d) in order to receive ipad(s) for use in the RTOG 1016 study. CRA/investigator user account activation website: register new user a c b d 5) After activating your account, you will be redirected to the Login page where you will use the password you just created. You will be automatically redirected to the Institution/iPad Info page (link at top right of webpage) (see figure 3). 4 you will first see a list of these institutions, which can be individually selected. Indicate yes or no to statement Use of WIFI is available at your institution for data submission to OSU. Also indicate the number of separate study sites at your institution where surveys will be administered. Study sites with wi fi network capability will automatically receive an ipad. Study sites without access to a wi fi network will receive an ipad via just in time shipping. These sites will only collect baseline data via the ipad for patients. After collecting baseline data for a patient, the ipad will be returned to the CASI administrator so that the data can be uploaded to the central database. All follow up data for the study must be collected and submitted via case report forms, available on the RTOG website. You OR another designated coordinator at your site will be responsible for managing your ipad. Please fill out the shipping address for receipt of the ipad. When OSU ships the ipad, you will be sent an with a link back to this same page where you can view the Date/Time shipped and the FedEx or UPS tracking #. It is expected that the ipad should arrive at your site within 4 business days. IMPORTANT: In the event that the ipad is ever lost or stolen, contact the CASI administrator ( RTOG1016@osumc.edu) ASAP so that the device can be remotely locked and wiped of its memory. Fig. 3. Additional instructions will be included in the . We require CRAs to take a practice survey in order to provide familiarity with the CASI system. 5 2. Set the CASI device passcode The RTOG 1016 CASI device passcode is a function intended to provide additional security to the CASI program when it is not connected to a wi fi network. When starting a survey in the absence of a wi fi connection, the passcode serves as a password so that staff will be the only individuals to access this function (when connected to wi fi, the system will ask instead for the user account password for the CRA user who logged into the application). Additionally, the CASI system will lock after 5 minutes of inactivity, and the device passcode must be entered to unlock the program. In order to set the CASI device passcode, you MUST have a CRA/investigator user account (see section I.A.1) and be connected to a wi fi network (see section II.5). 1) Start the CASI survey application and log into the system (see Section I.B.1 for more information about logging in). 2) On the Start Survey screen, tap the Setting icon. The Setting icon is the right most icon in the group of icons in the black bar at the bottom of the Start Survey screen. 3) On the Settings page (see figure 4), in the Device Passcode section, enter a passcode in the New passcode field (fig. 4, b). Confirm the passcode in the Confirm new passcode field (fig. 4, c), and tap the submit button (fig. 4, d). 4) The passcode should be a pin like number with at least one letter. It must contain at least 4 characters (for example, 999a, 7777b). 5) To change the passcode, you will need to enter the current passcode in the Current passcode field (fig. 4, a). 6) If you have forgotten the CASI device passcode, tap the Forgot passcode? button (fig. 4, e). You will be prompted to enter your CRA/investigator login password, and then the CASI device passcode will be displayed in a dialog box. IMPORTANT: The CASI device passcode should not be confused with the ipad passcode (see section II.6). These are two separate security functions. Login screen To launch the RTOG1016 CASI program, tap the program icon on the desktop (see section II.2). The first screen will be the RTOG 1016 Login screen (see figure 5). Enter your CRA/investigator user account login information (for information on setting up a CRA/investigator user account, see section I.A.1.): 1) The cursor should already be active in the username field, with the keyboard displayed. If not, tap in the username field (fig. 5, a) to activate the keyboard. 2) Enter username, and tap next on the keyboard to move the cursor to the password field (fig. 5, b). Alternatively, you can tap in the password field. 3) Enter password, and tap return on the keyboard to initiate login. Alternatively, you can also tap the Login button (fig. 5, c) on the login screen after entering your username and password to initiate the login. 4) After login, the Start Survey screen will be displayed. Start Survey screen and administrative functions Study Participant (SP) information is entered on the Start Survey screen in order to begin a survey (see section I.C for detailed information about administering surveys). The Start Survey screen is also used to access the administrative functions (see figure 6): Download Survey (fig. 6, a) (Section I.B.3); Submit Survey (fig. 6, b) (Section I.B.4); and Settings (fig. 6, c) (Sections I.A.2 and I.B.5). Administrative interface functions are available ONLY when the ipad is connected to a wi fi network. Tapping the Statistics (fig. 6, d) button will display pack years calculations for Baseline Demographics and Tobacco surveys that are stored locally on the ipad (see section I.D.2 for more info). Tapping the Log out (fig. 6, e) button will log out the user and return to the Login screen. Tapping the Quit button (fig. 6, r) returns you to the desktop, but does not close down the application (see section II.3 for information on closing programs). The number of queued and incomplete surveys stored locally on the ipad is summarized on the lower right hand corner of the screen (fig. 6, g). This summarizes information found in detail on the Submit Survey screen. Queued and incomplete surveys may need further attention (see section I.B.4 for further details). 8 Fig. 6. CASI Start Survey screen: administrative functions e f a b d c g 3. Download Survey screen The ipad will be shipped to your site with all appropriate surveys pre installed on the device. You should use this function only if you have been directed by a CASI administrator to download new or updated surveys. To download a survey, tap the appropriate survey to update (a check will be placed next to its name) and tap the Download button (see figure 7). Submit Survey screen When the ipad device is connected to a wi fi network (see section II.5), surveys are automatically uploaded to the central database when a SP finishes a survey and taps the Submit Survey button that appears at the end of each survey. Whenever a survey is successfully uploaded to the central database the CRA will receive a confirmation with the case ID, patient initials, visit number, and survey. Surveys are NOT automatically uploaded to the central database, but instead only stored locally on the ipad device itself, if: 1) The survey is stopped before the end by tapping the Save & Exit button in the survey (see section I.C.5 for more details). The incomplete survey is stored locally whether or not the ipad is connected to a wi fi network. The survey will be identified as IC: Queued. 2) If the ipad is not connected to a wi fi network. Without wi fi connectivity, both completed and saved surveys will be stored locally on the ipad device. The surveys will be identified as queued. 3) The survey is autosaved because the CASI system was shut down or crashed in the midst of survey completion (see section I.C.4 for more details). Surveys will be queued if conditions 1 & 2 above apply (see section I.C.4 for more information on how to deal with autosaved surveys). It is important to upload these locally stored queued surveys to the central database as soon as possible. When you reach an area where ipad is connected to a wi fi network, tap on the Submit Survey icon on the Start Survey screen. The Submit Survey screen (figure 8) provides an overview of surveys administered on that particular ipad. Each survey will have the survey title in bold with information underneath describing the completion/submission status and date and time and SP associated information ( case the SP ID number and the visit and study protocol ( study ) ID numbers) (see section I.C.1 for more information on case, visit, and study IDs). C means the survey is completed, and IC means the survey is incomplete and will need to be finished by the participant (see section I.C.5). Submitted means the survey has been uploaded to the central database, and queued and autosaved mean the survey is only stored locally on the ipad device. In figure 8, for example, we can see that two completed surveys have been submitted to the central database, one completed survey is queued, one incomplete survey is queued, and one incomplete survey is autosaved. If there are queued surveys stored on the ipad, a red circle with the number of queued surveys on the device will appear next to the Submit Survey icon at the bottom of the screen (fig. 8, a). This icon is visible from all of the administrative function screens in the CASI system, and it s meant to serve as a visual reminder that there are surveys which need to be uploaded to the central database. Submit survey screen: overview of administered surveys b a Additionally, SP information can be edited for queued and autosaved surveys by tapping the blue arrow on the right side of the survey row (fig. 8, b). Tapping the blue arrow will open a dialog box where SP initials and ID# and study and visit numbers can be edited. It is expected that all SP and survey information will be entered carefully and correctly when a survey is initiated, and this option is only meant as a back up to correct any errors. This function is not available to surveys that have been submitted to the central database. To submit the queued surveys (see figure 9): 1) Tap the empty box next to the survey name. A check will appear in the box (fig. 9, a). Repeat this step for all queued surveys. If all the surveys in the list are to be selected, tapping the Select/DeSelect All button (fig. 9, c) is a quick shortcut to select all the surveys. 2) Tap the Submit button (fig. 9, b) to upload the surveys to the central database. Once the survey has successfully uploaded the word queued will change to submitted. 3) Once all surveys have been uploaded ( submitted ), they should be deleted from the local storage on the ipad. To delete surveys already uploaded to the central database, tap the box next to the survey name to place a check (or use Select/DeSelect All button if appropriate) and tap the Delete button (fig. 9, d). UNDER NO CIRCUMSTANCES SHOULD A SURVEY BE DELETED THAT HAS NOT YET BEEN UPLOADED TO THE CENTRAL DATABASE (i.e., those still with queued or autosaved status) this will result in lost data. 11 Fig. 9. Submit survey screen: uploading surveys a c d b 5. Settings screen You may need to scroll (tap, hold and slide up or down) to see the entire settings screen (see figure 10). The settings screen has several functions: 1) The Always Skip Login function (fig. 10, a) is recommended to be set to Off : a) When set to Off the app loads the Login page first when the app is started and wi fi is available. b) When set to On the app loads the Start Survey page first. 2) See section I.A.2 for detail information about the Device Passcode function (fig. 10, b) 3) If you experience a bug with the CASI system, you can the CASI administrators directly through the Report a bug button (fig. 10, c). You will be asked to provide your address and a detailed description of the problem/bug you experienced with the CASI system. A CASI administrator will respond to your report. 4) The Ask your question button (fig. 10, d) is also another manner in which you may communicate questions or suggestions to CASI administrators. 12 Fig. 10. CASI settings menu C. USING THE RTOG 1016 CASI APPLICATION: ADMINISTERING SURVEYS 1. Starting a survey in the Start Survey screen (see figure 11) 1) Enter patient initials by tapping in the Subject Initials field (fig. 11, a). The keyboard function in this field is automatically set on all caps. 2) Enter the case ID by tapping in the Case ID field (fig. 11, b). THIS IS THE STUDY PARTICIPANT S STUDY ID NUMBER. Do not confuse this field with the Study ID field, which identifies the study as RTOG ) The Study ID field (fig. 11, c) identifies the study protocol matched to the survey. This entry should be 1 for the RTOG 1016 study (for CRA/investigator practice with the survey, set this field to 999). Do not change this field unless otherwise directed by a CASI administrator. Remember, this is not the study participant ID# the SP ID# is entered in the Case ID field. 4) Select the appropriate visit ID by tapping in the Visit ID field (fig. 11, d). A spinner function will appear, and tap and hold the spinner and slide up or down to select the appropriate visit. Tap OK when the proper study visit is highlighted on the spinner. 13 5) Select the appropriate survey ID by tapping in the Survey ID field (fig. 11, e). A spinner function will appear, and tap and hold the spinner and slide up or down to select the appropriate survey. Tap OK when the intended survey is highlighted on the spinner. 6) Tap the Begin button (fig. 11, f) to start the survey. If you are not connected to a wi fi network, you will be prompted to enter the CASI device passcode (see Section I.A.2). Fig. 11. CASI Start Survey screen: initiating participant survey a c e d b f 2. Survey types Practice survey The CRA/investigator should review the practice survey with the participant. Give the ipad to the participant to complete the practice survey and sit with the participant and offer to answer any questions the participant might have while they complete the practice survey. RTOG 1016 mandatory and optional surveys Every RTOG 1016 participant will complete the RTOG 1016 mandatory demographic and tobacco use survey. Participants will have indicated in their informed consent if they also agree to take the optional BRASS (risk assessment) and Quality of Life (QoL) surveys. Each survey will have to be launched by the CRA separately (although they can all be completed in one setting) the mandatory survey should be administered first, followed by the BRASS and QoL surveys (with appropriate consent for each). After each survey the CRA should complete the CRF survey, which documents details concerning the administration of the survey, such as completion status and whether assistance was provided. General survey screen Figure 12 illustrates the general set up of the survey screen, illustrating a question from the baseline demographics survey. Answers are selected by tapping the appropriate response(s), and check marks will be placed next to response(s). If a question allows only one response, only one answer will be checked at a time, and the response can be changed by tapping another answer. If a question allows multiple responses, then checks will appear next to each response tapped. Tapping a checked response a second time will remove the check. The CASI screen has a set of standard features: 1) The Save and Exit button (fig. 12, a) allows the SP to stop the survey at that point and save their responses the survey can be resumed later at the same point (see section I.C.5 for more information). The SP should indicate to the CRA/investigator that they wish to save and exit the survey if necessary. 2) The Help button (fig. 12, b) displays basic information about the CASI survey screen. 3) The Font/Screen Color button (fig. 12, c) will toggle between the standard display of yellow font with blue background (shown by research to be the best color contrast for all vision types, including color blindness) and a black and white display (as seen in figure 12). 15 4) The A and a buttons (fig. 12, d) will increase or decrease the size of the font, respectively. 5) The status bar (fig. 12, e) will indicate the progress through the length of the survey. Note that this bar is not a completely reliable representation of the number of questions remaining in a survey, since significant parts of a survey may be skipped depending on an SP s answers (for example, if they never smoked, the survey will skip through all of the smoking questions). 6) The Previous (fig. 12, f) and Next (fig. 12, g) buttons navigate through the survey. The SP must tap the Next button to move on to the next question. The Previous button can be used to review answers to previous questions, and change if necessary. Fig. 12. CASI survey screen 4. Stopping a survey before finishing Autosave The autosave function preserves a participant s responses in the event that the CASI program is inadvertently closed down or crashes in the midst of survey completion. Note: Simply pressing the home button during the survey does not close down the CASI program (see section II.3 for more details). If a participant seeks your assistance because the display switched to the desktop, it is possible they may have inadvertently tapped the home button. Tap the RTOG1016 CASI program icon. If the CASI program is still running, the screen will display the survey where it was left off. In this instance, the CRA can give the ipad to the SP to complete the survey. If the login screen appears, then the program did close down or crash. RTOG1016 ipad and CASI Survey User Manual, v4.3, 12/26/12 Page 15 of 25

16 In the event the CASI program is closed down or crashes, restart the CASI program by tapping on the RTOG1016 icon on the desktop. Log in, and open the Submit Survey screen. If a survey has been autosaved, it will be displayed on the Submit Survey screen. Autosaved will be displayed under the name of the survey in addition to other survey info, and will be highlighted orange. Unlike queued surveys on the Submit Survey screen, autosaved surveys CANNOT BE UPLOADED to the central database. An autosaved survey must be resumed and either completed or stopped by saving and exiting (see section I.C.5) and queued in order to be uploaded to the central database. To resume an autosaved survey, enter the appropriate survey, SP, and study information on the Start Survey screen and tap the begin button. A dialog box will appear asking if you want to restore the survey answers (see fig 13). Tap the restore button, and the survey will resume at the last question completed by the SP before the CASI program terminated. Fig 13. Dialog box to resume autosaved survey 5. Stopping a survey before finishing Save & Exit While taking a survey, a participant can save and exit any time they want. This may be necessary if a participant is called into an appointment or has other time constraints. The participant should indicate to you that they want to save and exit the survey. To save & exit, the CRA/investigator should tap the save & exit button on the top left of the CASI screen. Note: The survey will save only to the question/screen that is currently displayed on the ipad. It is important to check with the participant that they have only completed the survey to that point. 17 but then moved back several screens, questions after that screen will not be saved. As soon as possible, the survey should be uploaded to the central database. This can be done only when the ipad has wi fi internet connectivity (see section I.B.4 for uploading surveys and section II.5 for connecting to a wi fi network). The survey will be identified as incomplete ( IC ) in the CASI. Once the survey is uploaded to the central database, the participant can resume the survey on any ipad device at any time as long as it is connected to a wi fi network. To resume, enter the appropriate patient, survey and visit ID info on the start survey screen. The survey will resume at the first unanswered question in the survey. The participant can tap the previous button to review prior answers and information screens. 6. Finishing a survey When a survey is completed the CASI displays a screen with a large button that states Click to submit survey. If the ipad is connected to a wi fi network, the survey will automatically upload to the central database when the button is tapped. If the ipad is not connected to wi fi, a dialog box will appear stating that internet connectivity is not available at this moment. Do you want to save & exit? OK should be selected. The next screen states Thank you for your responses. Please give this device back to the person that handed it to you. Tap the exit button in the bottom right of the screen to return to the Start Survey screen. If the ipad was not connected to a wi fi network and the survey was saved locally on the device (queued), it should be uploaded as soon as possible to the central database (see Section II.B.4). D. ADDITIONAL CASI FUNCTIONALITY 1. Session timeout The CASI system is programmed so that after 5 minutes of inactivity on the program, your login session times out. The CASI login session may also timeout before 5 minutes if you are in an area that has intermittent wi fi connectivity. When the CASI system experiences a session timeout a dialog box with appear with a notification of the event. If this occurs while a participant is completing a survey, tap the OK button to close the box and allow the SP to continue with the survey. When they finish and tap the Click to submit button on the survey, another dialog box will appear to indicate that it will only be saved locally (queued) on the device. All surveys completed after a session timeout will be stored locally (queued) on the ipad device. These queued surveys should be uploaded to the central database as soon as possible (see section II.C.4). RTOG1016 ipad and CASI Survey User Manual, v4.3, 12/26/12 Page 17 of 25

Pack years calculation (Statistics) When the RTOG baseline demographics and tobacco survey is completed by a participant, you can tap on the Statistics icon at the bottom of the screen in any of the administrative function windows to see the SP s calculated cigarette pack years value (see fig 14). This function is enabled regardless of whether the ipad is connected to a wi fi network, and pack years will be displayed for all completed surveys which are stored locally (submitted and queued surveys listed on the Submit Survey screen). 19 II. ipad BASICS 1. Power To turn on the ipad device you can either press the home button (fig. 15, a) or the power button (fig. 15, b). Upon activation, an arrow will be displayed at the bottom of the screen next to the words slide to unlock. Tap, hold, and slide the arrow to access the ipad desktop. Fig. 15. The home button (a) and power button (b) To turn off the ipad, press the power button. The ipad also has a power saving feature where the device will automatically turn off after a certain period of inactivity. 2. Desktop overview and opening applications The ipad desktop screen (see figure 16) has device information and a set of application icons. If the ipad is connected to a wi fi network, a signal strength bar (fig. 16, a) will appear in the top left. The battery icon (fig. 16, b) is on the top right and indicates the remaining battery life. It is recommended that you charge the ipad at your earliest convenience when the device falls below 50% battery life. Applications installed on the ipad device are presented as square icons on the desktop. The ipad presents four standard applications by default at the bottom of the desktop (fig. 16, c): Safari (the ipad s internet browser), mail, photos, and ipod. Just above the four standard applications is a set of small icons (fig. 16, d): 1) Tapping magnifying glass on the left will initiate a search of the ipad. Pressing the home button while in the desktop screen also initiates a search. 2) The circle(s) indicate the number of desktop screens present on the ipad device. 20 than one screen, there will be multiple circles. The screen presently displayed will have its corresponding circle bolded. The RTOG1016 CASI application will be on the second screen. 3) To move between desktop screens, tap the screen, hold and slide in the appropriate direction. Remember that the application you are looking for may be on another screen, if there is more than one circle present. Fig. 16. ipad desktop To start an application, simply tap its icon once. 3. Closing applications When you are in an application, you can tap the home button to return to the desktop screen, from where you can access other applications. However, it is important to know that tapping the home button to return to the desktop screen DOES NOT CLOSE DOWN THE APPLICATION. When you are finished with an application, it is important to close it down to ensure the optimal performance of the ipad. To close down an application: 1) Press the home button twice. The desktop will fade and rise as a bar appears at the bottom of the screen. Icons of applications that are running will be presented in this bar at the bottom of the screen (fig. 17, a). After 2 seconds, all the icons will shake and a circle with a minus sign will appear at the top left corner of each icon (see figure 18). Fig. 18. 22 3) Tap the minus sign of the appropriate icon. The icon will disappear, indicating that it has been closed down. Again, to optimize the performance of the ipad, you should close down any application which is not being used at that moment. 4) To return to the desktop, tap in the desktop space above the bar of active program, or tap the home button. 4. Keyboard and number pad function In instances when you need to type words, letters, numbers, or symbols on the ipad, a keyboard and/or number/symbol pad function will display at the bottom of the screen. TIP: A convenient feature of the ipad is that the screen rotates to match whichever manner the device is being held. You can hold the ipad so that the display is in either a portrait frame (narrow left to right) or landscape (wide left to right). The keyboard and number/symbol pad displays are largest in the landscape view, which may make typing on the device easier. The keyboard letters are displayed as they are found on a computer keyboard (see figure 19), including standard buttons such as shift (fig. 19, a) and backspace (fig. 19, b). If the shift arrow is outlined or filled in blue it indicates CAPS set for one key entry (generally will re set to small caps arrow outlined in black after one key entry). Tap the shift button twice to set to CAPS LOCK, indicated by the button turning blue. The globe button (fig. 19, c) will change the language between English and Spanish. The symbol/number button (fig. 19, d) will switch the keyboard to the number pad (which also includes symbols; see next paragraph). 23 The number/symbol pad (see figure 20) is similar to the keyboard function. The symbol button (fig. 20, a) displays more commonly use symbols. The letter button (fig. 20, b) will switch the number pad to the keyboard. To close the number pad, tap the close out button (fig. 20, c) at the bottom right. Fig. 18. Number/symbol pad display 5. Wi fi and internet/network access If you are connected to a wireless network ( wi fi ), a connectivity icon will appear on the top left of the screen, next to the word ipad (icon (a) in figure 16). The icon will indicate the strength of the connection. To connect to a network (see figure 21): 1) Tap the Settings application icon. 2) Select Wi Fi (fig. 21, a) in the column of options on the left (alternatively, in the General settings, select Network, then select Wi Fi). 3) On the Wi Fi bar at the top, the control on the right (fig. 21, b) should be set to on. If it is set to off, slide the control to on. 4) A list of available wi fi networks will be displayed (fig. 21, c). Choose the appropriate network. A check mark will appear next to the network to which the ipad is connected. NOTE: Please contact a representative from the Information Technology department at your institution to inquire about wi fi access in your clinic and office space. 24 wireless network that you should use. Often more secure wireless networks are hidden and you will need to get the SSID, Username, Password, and other configuration information from you IT department. A connection to wi fi is necessary for transmission of data from the ipad. Some institutions may have secured wi fi and will require you to obtain a user name and password for access to the wireless network. Registration into your local wireless system may take some time, so please look into this at your earliest convenience. To disconnect from a network/turn off wi fi: 1) Tap the Settings application icon. 2) Select Wi Fi (fig. 21, a) in the column of options on the left (alternatively, in the General settings, select Network, then select Wi Fi). 3) On the Wi Fi bar at the top, slide the control on the right (fig. 21, b) from on to off. TIP: When you are in sub menus within a settings feature, you can tap on the arrow on the top left of the sub menu screen to return to the previous menu screen (not present in figure 21). Fig. 21. 25 6. ipad Passcode The ipad passcode protects the ipad from being used by unauthorized individuals. The passcode must be entered when the ipad is turned on (and activity resumed after >5 minutes of inactivity) in order to access the desktop. When you receive your ipad, you will be prompted upon the first use of the device to set a passcode. The passcode will need to have a minimum of 8 characters with at least 1 complex character. All RTOG 1016 study site personnel should know the passcode, and it should never be shared with participants or other individuals. If a participant experiences the enter passcode prompt, they should seek the assistance of a CRA/investigator. Note that the ipad passcode is NOT the same things as the CASI device passcode (see section II.A.2 for more information on the CASI device passcode). IMPORTANT: If you forget the ipad passcode will be required to return the device to the CASI administrator so it can be reset. After 6 failed entries to enter the ipad passcode, the device will be disabled for 1 minute. A subsequent failed entry will disable the device for 5 more minutes. If you enter the wrong passcode 16 times in succession, the ipad will be automatically reset. THIS WILL PERMANENTLY DELETE ALL DATA STORED ON THE DEVICE (including queued and autosaved surveys), and the device will need to be returned to the CASI administrator in order to be reinitialized.
