1 OCLQS Portal ODE License Application Description: This Job Aid describes the process of submitting an application for child care license as an ODE program in the OCLQS Portal. Click Log in as ODE User Step 1: Log In to the OCLQS Portal Note: An account must be created in SAFE in order to log in to OCLQS. Click Log in as ODE User Page 1 January 2017

2 Step 2: Enter Log In Information 1) Enter SAFE Username and SAFE Password 2) Click Log In Note: An account must be created in SAFE in order to log in to OCLQS. Click Forgot Username or Forgot Password to get your SAFE Username or SAFE Password from SAFE. 1 Enter SAFE Username and SAFE Password 2 Click Log In Click Create an Application Step 3: Create an Application Click Create an Application Page 2 January 2017

4 Step 6: Continue to Application 1) Click the Drop Down Arrow to select the appropriate Location 2) Click the circle to select the appropriate Address (physical) 3) Click checkboxes to select all applicable ages served 4) Click Continue to Application 1 Select Location 2 Select Address (physical) 3 Select ages served 4 Click Continue to Application Page 4 January 2017

7 1) Enter Facility information Step 9: Enter Additional Program Details a. Click on the applicable value(s) in the Available column of the Attendance Options field b. Click on the arrow to select the value into the Chosen column 2) Enter information in the remaining fields 3) Click Save and Continue Note: Only the values that appear in the Chosen column are selected. 17 Step 22: Attach Other Required Documents 1) Repeat Steps 18 to 21 for all required documents 2) After all required documents are attached, then click Save and Continue Note: When a document is added, the document name displays to the right of the document button.
