3 Introduction The aim of Clinical Profiles (formerly Surgical and Medical Profiles) is to stimulate reflective clinical practice and facilitate improvements in the care of patients, by making good use of existing Scotland-wide datasets. Clinical Profiles comprise a portfolio of collaborative projects led by Healthcare Improvement Scotland (HIS) and the Information Services Division (ISD) and are being developed in close collaboration with a number of clinical specialty groups in Scotland, the Royal College of Physicians of Edinburgh and the Royal College of Physicians and Surgeons of Glasgow are also linked in to the project. The profiles comprise of a suite of clinical indicators about the care provided by an NHS Board and its hospitals. The indicators are developed from operational definitions which are a developed jointly with clinical specialty groups for a particular disease or procedure and are categorised into outcomes of length of stay, mortality and readmissions. Dashboards are the new presentation methods for the profile data aimed to enable more in depth and continuous work directly with clinicians and specialty groups to seek to develop indicators that are credible and fit-for-purpose. REQUIREMENTS Software Requirements In order to view the dashboards created in Xcelsius, Adobe Flash Player needs to be installed and the appropriate permissions may need to be granted. To install Flash Player please go to or request from your IT department. Due to a known problem with compatibility, the dashboards may not work with flash player version 10.1 and you are advised to upgrade your version of flash if the dashboards do not appear on screen. The latest version of flash player is recommended. It is recommended that you do not run excel or any other large programs while using Xcelsius, as it requires a fairly large amount of processing power. Recommended for GPU hardware acceleration dependent features. Flash Player will use software mode for systems that do not meet the system requirements. 2. Flash Player supports browser versions listed when run on operating systems supported by the browser vendor. 3. Beginning with Flash Player 10.2, Flash Player is integrated as part of Google Chrome. 4. ATI Rage 128 GPU does not support full-screen mode with hardware scaling. 5. Only Advanced Linux Sound Architecture (ALSA) is supported. Systems using OSS/ESD will not play audio. Only GTK2-based browser versions are supported. 6. Content protection using Adobe Flash Access is not available on this platform. For more details/updates please go to Software Permissions To ensure the Xcelsius flash files created work as designed, you may have to give Adobe Flash Player permission to access the file location. Instructions on how to do this are given below. Go to the Flash player setting Manager accessed by the following link Flash Player Global security settings. Select global security settings from the left hand menu if you are not already in the security panel, select edit locations, add locations, add the location of the file(s) 4

5 then click confirm. You may have to log off your computer and log back on again to ensure the permissions are updated. Click to select Global Security Settings panel Click on Edit locations and then select Add location Copy and paste web address stated above into the trust this location box and click confirm Alternatively access Flash Player Manager via Control Panel on your computer, by going to Start Settings Control Panel. 5

6 Double click on Flash Player icon on the list. When Flash Player Settings Manager opens select Advanced tab And scroll down to Trusted location settings button. 6

7 Add the location of the file(s) or paste link to the website with the dashboard, then click confirm. You may have to log off your computer and log back on again to ensure the permissions are updated. If while trying to open the link your internet browser security notification pops up, right click on it, select Allow blocked content and confirm. 7

8 Screen resolution In order to experience the optimum viewing of the tool, the recommended screen resolution is 1020x768 for 15 screen or 1280x1024 for 17 screen. If the text appears too small or blurry, pressing F11 to switch between Normal and Full Screen mode should enable easier reading. If the whole of the tool appears too small or too big on screen, temporarily adjust the screen resolution may correct this. To temporarily change screen resolution go to Start Settings Control Panel Display Settings tab and move the slider to appropriate setting. Don t forget to note the original screen size so you can change it back if you wish. ACCESS The Profiles dashboards are accessed via dedicated area on ISD Quality Indicators website. The dashboard will take few minutes to fully load, depending on the speed and specification of your computer. After loading you will be asked to select your Health Board and enter your unique password and click OK. The page will automatically switch to the Introduction tab. Select your health board from the list Type your unique password and click OK General notes These data are released for management purposes only and have not been adjusted to protect against potential disclosure risks and may contain information which enables (perhaps with the aid of further knowledge of the topic) an individual patient to be identified. Please ensure circulation is restricted and that patient confidentiality is not compromised. For further guidance see ISD s Statistical Disclosure Control Protocol. 8

9 DASHBOARD Overview Exact structure of each dashboard depends on characteristics of the specific profile; however there are some main generic features common for all the tools. Generally data is grouped in separate tabs for each performance indicator used. Tab opens when selected. It may take a moment while tab contents loads when opened for the first time, after that user should be able to quickly switch between tabs. Indicator tabs You can print the dashboard as displayed on the screen by selecting Print button situated in the left bottom corner of each tab. It will link to a default printer installed on your machine. You can change the layout, colour and orientation of the printout via your printer s properties menu. Introduction tab Introduction tab consists of: - Description of the data presented within the dashboard, including overview of the data sources used - List of operational definitions / codes used for that particular profile - Detailed description of each of the performance indicators used. Performance indicator tabs Typical performance indicators include readmissions, mortality and length of stay. Each tab consists at least of: A. Funnel plot chart Funnel plots allow comparisons to be made between different NHS Boards or hospitals and the average for Scotland. Data are plotted in relation to the Scottish average and control limits set at three standard deviations from the average. There are 5 key lines in the chart: - Average for Scotland, depicted in dark blue - Two sets of limits plotted on either side of the average 9

10 o Warning limits are plotted at 2 standard deviations from the average and these limits are depicted in orange. o Control limits are plotted at 3 standard deviations from the average, and these limits are depicted in red. - Data points o Those within the control limits are said to exhibit common cause variation or to be in control, these are depicted in dark green. o Data points out with the control limits (sometimes called outliers ) are said to exhibit special cause variation and this is where further investigation is required. These are marked with orange. Mouse over data point will show the hospital name for that data point in the right top corner just above the funnel chart. Name behind the data point pointed at with a mouse. Control limits Data point in control Data point highlighted when selected from the table; colour indicates it is an outlier Outlier B. Table chart Table chart includes one year data for Scotland and hospital/ health board. It includes numerator, denominator, standard and crude rate. There is also an up or down arrow indicating if the hospital / health board is an outlier. Data in the table can be sorted by clicking on the heading. Selecting hospital from the table highlights corresponding data point on the funnel plot chart. 10

11 Click to sort data by selected column C. Trend chart Trend chart displays trend for selected hospital plotted against Scotland on a quarterly basis over a period of time. Hover with the mouse cursor over any of the data points on the hospital trend to display numerator, denominator and a rate for that data point. Trend charts are available only for hospitals from selected Health Boards with recorded cases in the period. Hover over data point to display numerator, denominator and rate Troubleshooting Troubleshooting Problem Unable to log in to view dashboard Adobe flash error message when accessing the tool (problem with add on) Solution Check you have correct password to access. Update flash to latest available version. Due to known errors with dashboard loading flash player version 10.1 is not recommended. Tool does not load properly Make sure you do not have flash player 10.1 as the dashboards do not work with this 11
