1 User Manual For Monitoring System of Health Infrastructure Development (Civil Works) Under Integrated MIS-GIS System (Version 1.0) (Date of Publication: 22nd Dec 2017) National Health Mission

2 Department of Health & Family Welfare Govt. of Assam About : It is an online web based application to track the progress of health infrastructure development projects going on throughout the State. The system provides the facility to monitor the progress of the civil works going in different Health Institutions of Assam. It keeps the record of various works, their approval and sanction amounts, contractor and agency details and status of the works. 5 Use username and password assigned by MIS Cell, NHM, State Head Quarter, Assam to login. After clicking the Civil Works icon, following screen will be appeared with a login form: Login using the username and password provided by MIS Cell, NHM, State HQ, Assam. After successful login, user will be landed in the following page: Page 5

9 1. Masters Some data is used throughout the projects. For example Scheme Name, Executing Agency etc. So it is necessary to add them once only in master pages so that we can save them in our master database. Master pages include the master data of the projects. It is one time entry form for each master data. Only authorized user can add new data, delete data and edit data. The section has following menus: Scheme Executing Agency Contractor Monitoring Officer Key Performance Indicator Architect Consultant Agency Sub oil Investigation Agency ROP Approval Page 9

10 Following are the master forms: 1.1 Scheme To go to Scheme page, go to Masters->Scheme. In the page, user can view schemes. If user has required authorization, he can add new Scheme, edit scheme and delete scheme. Page 10

11 1.2 Executing Agency: To go to the Executing Agency page, go to Masters->Executing Agency. In the page, user can view list of Executing Agencies. If user has authorization, he can add new Executing Agency, edit executing agency and delete executing agency. Page 11

12 1.3 Contractor To go to the Contractor page, go to the Masters->Contractor fro menu. In the page, user can view list of Contractor. If user has authorization, he can add new Contractor, edit contractor and delete contractor. Page 12

13 1.4 Monitoring Officer: Monitoring officers will monitor the progress of the projects. He will update the physical progress of the projects. For this module, data from HR-MIS System is used. User can add new monitoring officer, edit and delete monitorinbg officer in the page. Monitoring officer is an existing employee Page 13

14 in the HR database of National Helath Mission. There are different designation level present in the form. District and state level designations are present in the form. To go to the Monitoring Officer page, go to Masters->Monitoring Officer. 1.5 Key Performance Indicator(KPI): Key Performance Indicator(KPI) is the stages of the projects. It describes the stage of a project. Page 14

15 To go to the Key Performance Indicator(KPI) page, go to Masters->Key Performance Indicator. In the page user can view list of KPIs. Every KPI has its order. Only authorized user can add new KPI,edit and delete KPI data. 1.6 Architect Consultant Agency Page 15

16 To go to the Architect Consultant Agency page, go to Masters->Architect Consultant Agency page. In the page, we can view the list of Architect Consultant Agencies. Authorized user can add new Architect Consultant Agency, edit and delete new architect consultant agency. 1.7 Sub Soil Investigating Agency Page 16

17 To go to the Sub Soil Investigating Agency page, go to Masters->Sub Soil Investigating Agency page. In the page, we can view the list of Sub Soil Investigating Agencies. Authorized user can add new Sub Soil Investigating Agency, edit and delete Sub Soil Investigating Agencies. Page 17

18 1.8 ROP Approval To go to the ROP Approval entry form, go to Masters->ROP Approval from menu. It is form where we can make data about ROP approval. ROP approval data can be entered for different schemes. Page 18

19 2. Entry Forms: Entry forms are found under the menu entry Forms. 2.1 Project Entry: For project entry, go to menu Entry Forms->Project Entry and make entry of projects with their data. Page 19

22 In the above project entry form, only a few fields(scheme, executing agency, project id, project name, district name, BPHC name, hi name and project KPI status) are mandatory. After entering the mandatory fields, you can save the data by clicking Submit button. Project ID : enter the file name assigned for the project like NHM/C-CW/RHP/ ANM & MPW Qtr/ / 2398/14. Project Name: enter the name of the project like Construction of RHP/ANM Quarters at Sub-centres. Scheme: Select scheme to which the project is belongs to. Excecuting Agency: Select the executing agency who executes the project. Project KPI Status: Select the stage of the project. Example: Page 22

25 2.2 Edit Project: We can edit data for projects in the Edit Project. To go to the page, click Entry Forms Edit Projects. Page 25

26 Then the following page will be displayed with list of projects. The following page will be displayed with all projects: Page 26

27 In the page, we can filter projects by selecting scheme and district. Then click search. Then projects against the search conditions will be displayed. Then following project edit page will be displayed. Here we can edit value of the fields and need to click edit to update the data. Page 27

28 2.3 Physical Progress: For physical progress entry of the project, go to Entry Forms->Physical Progress. To go to physical progress entry form, we need to search projects. Page 28

29 After clicking search button, the list of the projects against the search criteria will be shown. To go to the Physical Progress Entry Form, we need to click on the Project Code. Page 29

30 In the form, enter the necessary data and submit the form. Page 30

31 2.3 Financial Progress Entry Form: For Financial Progress entry of the project, go to Entry Forms->Financial Progress. To go to financial progress entry form, we need to search projects. Page 31

32 After clicking search button, the list of the projects against the search criteria will be shown. To go to the Financial Progress Entry Form, we need to click on the Project Code. Page 32

33 In the form, enter the necessary data and submit the form. 3. Reports In the Reports module, there are three sections: 1) Summary: 2) Projects 3) ROP Approval 4) Contractor Wise Page 33

34 3.1 Summary: The Summary section contains reports in the following structure: Summary- District Wise Report Project Wise Report Project Detail Report Here, first we can view summary of projects scheme wise. To enter into to the page, go to Reports->Summary. Then following page will be displayed. Page 34

38 3.2 Projects: The section has following facilities: 1) List of projects 2) Link to go to the Project Details page. 3) Link to go to Physical Progress detail page. 4) Link to go to Financial Progress detail page. 5) Link to go to Edit Project page. Page 38

39 To go to the Projects page, go to Reports->Projects. Page 39

40 After clicking Projects, we will see a page containing list of projects. If the user is authorized, he will see a edit button in the right side of every projects. Page 40

41 3.3 ROP Approval The page shows the list of ROP approvals for the schemes. To go to the page, go to Reports->ROP Approval Detail. The page will show the list of ROP Approvals. Page 41

42 3.4 Contractor Wise The page will show the list of contractors with projects. Page 42

43 After clicking on the contractor name, the following page will be displayed. The page will show the list of projects under a contractor. Page 43
