1 How to Create and Update a DS-2019 Form Application: Campus Solutions Audience: Admissions Objective: In order to receive an official DS-2019 form from the Department of Homeland Security (DHS) SEVIS system, you must input DS-2019 information into the Campus Solutions SEVIS database. This tutorial will show you how to enter and update DS-2019 information. Before you can create a DS-2019 for a student, you must first enter basic contact, visa, and passport information for the student. See the "Enter Visa Information" and "Enter Passport and Citizenship Information" tutorials for more details. Before you Begin: To sign in to Campus Solutions, go to sis.illinoisstate.edu. Once there, click the Please click here to PeopleSoft logon page link. Then, you will be prompted to log in with your ULID and password. Once you log in, the options on your screen will depend on your security access. If you are unable to log in, contact the Technology Support Center (TSC) at (309)438-HELP(4357), supportcenter@illinoisstate.edu, or ITHelp.IllinoisState.edu. 1

2 1. To access the DS-2019 Form page, click Main Menu. Then, click Campus Community. Next, click SEVIS. Then, click DS-2019 Forms. Finally, click DS Form. 2. Click the Add a New Value tab. 2

3 3. Click the Look up button next to the Empl ID field. Then, use the search fields to search for and select the student. Next, click the Add button. 4. Click the Visitor's Category dropdown menu button. 3

4 5. Select the category that best describes the person or student for whom you are creating the DS-2019 form. For this example, select the Professor option. 6. Click the Look Up button next to the Permanent Country field. 4

5 7. Enter criteria in the search fields. For this example, enter "France" in the Description field. 8. Click the Look Up button. 5

6 9. Click the France link. 10. Click the Look Up button next to the Position Code field. 6

7 11. Search for the correct position code for this person. Enter "University" in the Description field. 12. Click the Look Up button. 7

8 13. Click the University Teaching Staff Including Researchers link. 14. Scroll down to enter additional information. 8

9 15. Enter the person's program start date in the Start Date field. For this example, enter "01/15/2016". 16. Enter the person's program end date in the End Date field. For this example, enter "01/15/2017". 9

10 17. Click the Look Up button next to the Subj/Field Code field. 18. Search for the appropriate field code for this person. Enter "Biology" in the Description field. 10

11 19. Click the Look Up button. 20. Click the Biology/Biological Sciences link. 11

12 21. Enter required remarks in the Subj/Field Remarks field. 22. Scroll down to enter financial support information. 12

13 23. Enter funding amounts in the available fields. For this example, enter "80,000" in the Personal Funds field. 24. Click the Funding Verified checkbox. 13

14 25. Click the Look Up button next to the Responsible Officer field. 26. Select your name from the list. For this example, click the Rita Redbird link. 14

15 27. Scroll down to enter dependent information. 28. You can only add dependents that are already in the Campus Solutions database. See the "Create a Dependent or Visiting Scholar Record" job aid for more information on how to add a dependent to Campus Solutions. Click the Look Up button next to the ID field in the Dependent Information grid. Then, use the pop up window to search for and select the desired person. 15

16 29. Click the Look Up button next to the Permanent Country field. 30. Use the search fields to find the permanent country of the dependent. Enter "France" in the Description field. 16

17 31. Click the Look Up button. 32. Click the France link. 17

18 33. Campus Solutions will send a request for a new DS-2019 form to the DHS SEVIS system once you run the SEVIS alerts process and the SEVIS export process. The official PDF of the DS-2019 form will be included in the response file that the DHS will send back to Illinois State University. 18

19 34. After you have created the DS-2019 form, you can return later and make changes to it. Navigate to the same component: Campus Community > SEVIS > DS-2019 Forms > DS-2019 Form. Then, search for and select your student. 35. Click the plus (+) button to add a new effective date. 19

20 36. Scroll down until you find the information that you want to edit. In this example, we will add another dependent. 37. Click the plus (+) button to add a new row. 20

21 38. Click the Look Up button next to the ID field. Then, use the pop up window to search for and select the dependent. 39. Click the Relationship dropdown menu. 21

22 40. Click the Relationship dropdown menu. 41. Select the Spouse option. 22

23 42. Add the dependent's permanent country. Click the Save button. 23
