The entire set of 19 pages for the Child Abuse Report were reviewed. These pages thoroughly represent the non-compliance issues with the web application. The list of report sections (links) isn't identified and no instructions are provided on how to use them to edit the report. Page refresh is sometimes long, but the preloader isn't read - confusing. Some field names are somewhat "jargon" and it may be difficult for average person to understand (i.e. police officers may say "Unit Designator", but an average person might say "what type of domicile is it? Some field name need better descriptions, (i.e. Phone is titled TEL- Phone is more commonly used). No instructions for phone number-to not enter dashes. Headings should be provided for pages with multiple sections and multiple headings are provided for simple pages that don t require additional navigation. Suggest not to use URL's as links unless it is a very simple address. Top heading is always double spoke when page opens. Some form labels don t seem to agree with the title of the section (i.e Victim Info section form label reads Contact Info). Suggest to not use abbreviations, such as APT (which is read as a word, not Apartment). 20
