2 Introduction This document is intended to help practices create documents that provide evidence that the Standards & Guidelines of the NCQA Patient- Centered Medical Home (PCMH) Recognition process are being met. For many Standards and Elements, screen shots can be used to demonstrate how requirements are met within the practice. In some cases, screen shots alone can provide adequate documentation. In others, additional information may be needed to provide explanation before the documentation adequately shows the practice meets the requirements of the Element. 3 Creating Screen Shots Screen shots are freeze frames of the images on a computer monitor. To capture a screen shot of what appears on your computer monitor, press the Print Screen key which is typically located near the upper right-hand corner of the computer keyboard (Figure 1). 4 Creating Screen Shots On some keyboards, press the Shift and Print Screen keys simultaneously to capture a screen image (Figure 2) Figure Oklahoma Primary Care Association 4

5 Placing Screen Shot Print Screen places an image of what is visible on the monitor to your computer Clipboard To insert the image to a document, click Paste (Figure 3) This will place the last screen shot taken to the document Figure Oklahoma Primary Care Association 5

6 Creating Shapes To add a shape or textbox to a document or image, click Insert (Figure 4) Click Shapes to open a drop-down of shape options (Figure 4) Click the icon for the shape you want to add Figure 4 Figure Oklahoma Primary Care Association 6

7 Creating Shapes Click and drag the mouse to draw the shape into the document You can use the Drawing Tools to change the shape properties (Figure 6) Figure Oklahoma Primary Care Association 7

8 Adding Textboxes Add a textbox the same way as a shape by clicking Insert (Figure 7) Click Textbox to add the textbox (Figure 8) Click & drag the mouse in the document to add the textbox Figure 7 Figure Oklahoma Primary Care Association 8

9 Adding Textboxes Type your content into the textbox Use the Text Box Tools to format the way the textbox looks including color fill, outline, etc. (Figure 9) Figure Oklahoma Primary Care Association 9

10 Using Shapes to De-Identify Data Shapes can be used to de-identify data in screen shots used to document PCMH Elements Draw the shape over the information you would like to remove and change the Shape Fill to black (Figure 10) 2012 Oklahoma Primary Care Association 10

12 Using Textboxes to Add Information Textboxes can add information to screen shots to add explanation to documentation Draw a textbox onto the screen shot or into the document and type the information you wish to add (Figure 11) 2012 Oklahoma Primary Care Association 12

13 Type the information you want to add into the textbox. You can also insert arrows under Shapes tools to point out specific information. Figure Oklahoma Primary Care Association 13

14 Tips & Notes The instructions in this handbook of how to add textboxes and shapes use Microsoft Office 2007 programs. Programs in other versions of Microsoft Office may be slightly different. Once you have created your documentation (and added any shapes or textboxes) be sure to convert your file to a PDF or similar type document so shapes used to conceal information cannot be moved Remember to keep the documentation simple so that it is easily understood by reviewers Screen shots from Microsoft Office program used in this document are for non-commercial education use only and adhere to Microsoft s use policies.
