2 nline Consent Management & Monitoring System INTRDUCTIN nline Consent Management & Monitoring System is a web based application for Consent Management developed for the PPCB (Punjab Pollution Control Board), Ministry of Environment & Forest. The details of Industry User as given below. Features of CMMS Unified Global online application/ portal Role based application access Multi level Security Enabled System nline Industry Registration nline Submission of Application, Approval, Clarification and other activities for CTE, CT for all category of Industries/Institutions nline Industry Registration Industry registration is an online feature; which will enable Industry applicant to register himself online to use the application for applying for Consents (CTE/CT), Authorization and other features. Using this Industry will be able to generate the user temporary name and password, which he needs to use within the stipulated time. nce Industry uses the temporary user name and password for Consent Application; documents/details provided with this will be validated and user name will be confirmed and made available to him for future use. If Industry fails to use the temporary user name within the given time frame, then this user name will be deleted from the database and applicant will have to re-register himself to use the online application.

3 nline Consent Management & Monitoring System Steps to use this feature: Click on User opens the URL nline Consent Management& Monitoring System in browser.

4 nline Consent Management & Monitoring System pen Home Page by entering URL in Browser. Click on New Industry Registration to register Industry Screen Shot: Home Page Click New Industry/HCE Registration New Registration form will be displayed. For industry User select Industry button and for Health Care Establishment (HCE) select HCE. User fills the asked information and click Submit. nce submitted, System will validate the input data. If data is incorrect or mandatory filled not filled then system will prompt user to correct the information. If data filled by applicant is correct then System will generate user name and Temporary password and will be displayed on screen.

10 nline Consent Management & Monitoring System Login Process When you are login as Industry User with this User ID and Password, firstly it gives change password page as shown below. Enter the User ID & Temporary Password

11 nline Consent Management & Monitoring System Enter the old & new password ls and click on Screen Shot: update. Change Password Window Enter system generated password in old password. Enter the new password of minimum 8 characters size having at least 1 character, 1 digit and 1 special character (@, #, $ only). After changing password successfully, you can login in to CMMS by selecting radio button Industry and entering user id, password and Captcha code. Maximum 3 times you can enter wrong password, after that your account locks. nly way to unlock it is doing forget password. Steps for doing forget password i. Click on Forget Password link on Homepage. ii. Click on Industry User iii. Enter User ID and then click on submit. iv. Enter Hint Answer of Hint question as given in registration form and then click on submit. v. After entering correct password it gives password change page, Enter new password and confirm it same. vi. Then you created a new Password.

20 nline Consent Management & Monitoring System Upload the documents After uploading click on save If you want to delete uploaded documents For documents uploading select documents tab and click on upload button with non-editable document format as Pdf, each document will be less than 5 Mb. If u want to delete any uploaded document, then click on delete button select required document and delete.

21 nline Consent Management & Monitoring System If you want to add the other documents fill the document name& save it If not required then click on exit After uploading the documents then click completebutton and save a popup display please enter fee details click on K and select FEF Tab

23 nline Consent Management & Monitoring System Review the application form before the submission Then select payment mode for nline Payment Select payment type for PNB Net banking Select Net Banking and submit PNB Gateway will be open and login with PNB Customer ID and deposit the fee. After complete transaction a Challan will be generated, print the Challan Form and submit the application

24 nline Consent Management & Monitoring System Select payment type for NEFT/RTGS SelectNEFT/RTGS and submit If industry user wants to make NEFT/RTGS through net banking, Select the listed bank & made the payment through Beneficiary Name Punjab Pollution Control Board

26 nline Consent Management & Monitoring System Enter the Application No. & Submit Select transition type and enter UTR/Reference No., date of transition & Amount i.e and verify, if want to add more transaction click added UTRs
