The forms have been customized to allow UCHealth staff to report adverse events, near misses, and good catches. Capturing and managing adverse events effectively allows hospital administrators to evaluate trends and develop action plans to prevent incidents from happening. 2 Accessing and Logging In Logging INTO RL Solutions Launch the application from following: Source click on My Apps select RL Solutions Source click on Clinicians select RL Solutions From within Epic click on your Learning Home Dashboard Scroll to the Weblinks section and click on RL Solutions At the RL Solutions login screen, be sure to address the following elements before clicking: 1. Domain Selector: select your campus domain from the dropdown menu 2. Login: use your UCHealth username and password to access the system 3. Anonymous: this will appear to the left of the Login button when you select Applications Account as the domain 4. Click the Login button. NOTE: When you log in for the first time, you will see a suggestions to get started pop-up box. How to Submit a File Search for a Form 1. Click the New File button, if it doesn t default to it, on the Navigation toolbar. 2. Locate a form in one of the following ways: A. To locate a specific form: Click inside the textbox labeled Find a form Enter a keyword or a form name Icons will filter to match search criteria OR B. Click on an icon to begin a new submission form. Press the F11 key to expand the screen or press F11 again to return to the original view. In the icon wall settings, turn the icon titles on to display the titles under the icon. Watch a demo searching for a form. CD last modified: 8/11/2017, Page 2 of 5

3 Fill in the Form Please be sure to keep responses factual and objective. As you encounter an issue, please be sure to also address these in real time. Complete the required fields on the form and note it saves information as you input it. REMINDER: Issues that require real-time follow-up, please do not only rely on RL solutions reporting. 1. Complete all the required fields, which are marked with green asterisks. 2. Search for patient by MRN/Last Name/First Name/Account number. 3. There are two ways to access sections on the form: A. Click section headings from the Table of Contents widget to navigate to different parts of the form. OR B. Use the scroll bar to view additional fields on the form. 4. Refer to the Status Bars under File Status to see how many fields have been completed and how many are remaining. TIP: When entering DOB, use month/day/year (04/01/2000) format. 5. Click the Add button to enter in multiple values in fields, such as the one for Parties Involved/Notified/Witnesses/Reporter. NOTE: It is important to include information about all parties involved (especially you the reporter) to facilitate tracking and follow-up efforts. CD last modified: 8/11/2017, Page 3 of 5

4 6. Refer to the Harm Score table to assist in scoring an event. Submit the File Be sure to submit the form once all required fields are completed. If you do not complete all of the mandatory fields, the system will direct you back to that field. 1. Once the required fields are complete, click the Submit button in the bottom right corner. 2. A new window will appear with the file number. 3. Click OK. If all mandatory fields are not complete, gray pop-ups will appear next to field(s), prompting you enter all information before you can submit the form. More Actions Button Click More Actions button for additional options: 1. Submit & Copy: Select this option if the event involved multiple people or the majority of the information in the new form will be the same. Once selected, most of the information will be copied over to the new file. You can then edit the information as needed. 2. Submit & Manage: This option is typically only available for file managers. As a manager, submit a file and manage it right away. This option takes you directly to the management form where you can add additional information. CD last modified: 8/11/2017, Page 4 of 5

5 Logging Out Logging OUT of RL Solutions After you have finished entering your incident, be sure to appropriately log off of RL Solutions, especially if you are using a shared workstation. 1. Click on your name in the upper right corner. 2. Select Logout from the dropdown menu. 3. A confirmation message appears NOTE: You have a brief moment to click Log in again, if needed.
