A copy of the inspection report is enclosed for your records. Please refer to the Summary of Findings section of the attached inspection report and provide a written response for each violation that was noted. This response should be mailed to the attention of the Water Division Inspection Branch at the address at the bottom of this letter or e- mailed to Water-Inspection-Report@adeq.state.ar.us. This response should contain documentation describing the course of action taken to correct each item noted. The facility did not have a Class II licensed wastewater operator. This violates Part II, Item 1 of the Permit. 2. There was an accumulation of solids behind the outfall weir (Photo 1). This violates Part III, Section B, Item 1A of the Permit. 3. Flow measurements were not recorded with the time, individual performing measurements, location and method of measurements (Photo 2). This violates Part III, Section C., Item 8 of the Permit. GENERAL COMMENTS A letter will be sent to the responsible official. Box 185 Heth, AR, RE: Response to Inspection, Super 8 Motel WWTP AFIN: , Permit No.: AR Dear Mr. Patel: I have reviewed your response pertaining to my September 19, 2013 inspection of the above referenced wastewater treatment facility. Upon review, the information provided does not sufficiently address the violations referenced in my inspection report. The following item requires your immediate attention: 1. It appears that your facility still does not have a licensed wastewater operator. Part II, Condition 1 of your Permit requires that the operator shall be licensed as Class II. You will need to provide documentation that your facility has a Class II licensed wastewater operator no later than January 31, Thank you for your attention to this matter. Checked with Sandy and found out that no application has been received. Called Mr. Shah again, he changed his answer that he would be sending his application in.
