1 1 Click on the Online Registration button which opens an information box. At the bottom of the box it will list the 2 names of your children. Click on the name of the first child to begin the online registration process. If you have more than one student enrolled in CHSD155 schools, you will see all of your children s names listed at the bottom of the box. IMPORTANT: Please complete the registration steps for each student you want to enroll. The system will start you at the first step of the online registration process. In each step please enter/update the information requested. As you complete 3 each step please click the checkbox next to I have completed this step. If you try to click the checkbox to mark the step as completed but a check mark will not appear, there are items to be completed that are required for that step that have not yet been done. Once all required items in a step are completed the check mark will appear when clicked. 4 To move forward through the process please click the Next Step button. 5 If at any time you want to stop and complete the process later, click the Close and Finish Later button. The next time you log into Skyward Family Access and re start the online registration process the system will start you at your previous step.

Health Information Please enter information in all of the fields. If there are no medical concerns for your student, type "NONE" in each text box.

4 For Steps 2 5 be sure the box is checked and you click the save button in the top right hand corner. STEP 2: Emergency Treatment Release STEP 3: Student Rules and Expectations STEP 4: Student Information Release STEP 5: Children of Military Personnel (Optional) The optional step to complete the Military Children Form can be skipped if it does not apply to your family. The information collected will help identify Illinois military families and help our district obtain U.S. Department of Defense assistance for children struggling with their parent s/guardian s military deployment.

5 STEP 6: Registration Fees School fees and other payments can be made in one step using a credit/debit card for all of your children. Cash or check payments can also be made in person during the on site registration days. The total amount due will be displayed in the bottom right hand corner. If you wish to add items such as a yearbook or activity ticket simply click the button labeled Add Charges. You can only add additional items one at a time. To add another item repeat the process until you are ready for check out and then click Pay with RevTrak. If you have never made a payment through Skyward Family Access/RevTrak before, you will need to create an account. To create an account, a valid address is required because payment receipts and password assistance will be sent to this address. You only have to create an account once. Use your newly created account information to make any additional payments you want in Skyward Family Access/RevTrak. You will also be able to use this account to add money to your students food service account.

6 STEP 7: Complete Online Registration When all of the steps have been completed the final step is to complete the online registration process. Each step will show a message of completed, not completed, or skipped (for any optional steps that were not completed). If any steps show as not completed, the Submit Online Registration button will not be available. Once all steps are marked as completed or skipped (for any uncompleted optional steps) click the Submit Online Registration button to complete the online registration process for your student.
