1 IView MODULE IView OVERVIEW For RNs/ LPNs/ PCTs
2 IView Layout Navigator View Window There are two main sections of the IView layout, the navigator and the view window.
3 Understanding the Navigator Bands Sections The Navigator is made up of Bands and Sections. The Bands are the name of the category to be charted on and the Sections are each subject to be charted on. If you think of the Navigator as a file cabinet, the Bands are the drawers and the Sections are the files.

5 Frequently Seen Icons
6 Time Range You can adjust your time range (by right clicking in the blue area where the time range displays) from the defaulted Last 24 Hours to Last 48 Hours, Last 72 Hours, Admission to Current, Today s Results or Other Be aware that the further you go back in time, the more data is requested and the slower the information will load.
7 Selecting Timescales Right click on the timescale column header. Select the frequency for which you want charting to occur. The default is Q1 hour.
8 Inserting a Date/Time Column Right click on the date and time and Select Insert Date/Time. Then enter the date and time for which you want to add a column for charting.
9 Activating Charting When charting an entire assessment or column, double click on the time at the top of the column to make the entire column available for charting. When documenting only one section, double click on that section header to open that section. When documenting on only one item, right click in that cell and click on Add Result. The check marks indicate that charting can occur. Remember that no results will flow from the monitor until the sections are activated.
10 Documenting Responses Single Select Multiple Select When you select an answer in a single select response question, it will automatically take you to the next question below (if you have opened the entire column or section, this will not work if you are charting cell by cell). When you select an answer in a multiple select response question, you will not be automatically taken to the next cell. Use the Tab key to move to the next question.
11 Conditional Logic A conditional logic question icon is circled above. When you answer a conditional logic question, it may open other pertinent questions depending on the answer chosen. These conditional questions may be required as is the case with the Fall Risk Protocol question. All required questions must be answered before charting can be signed.
12 Dynamic Groups A dynamic group is used to document results for an item that your patient may have more than one of, such as a peripheral IV, central line, chest tube, ostomy, catheter, etc. Open and add as many of the groups as you need to. Simply click on the grid with the blue arrow and it will open up the label window to complete with the identifying information for each individual group.
13 Dynamic Groups Continued When the label box opens, select the type, site and laterality for a PIV. Other dynamic groups may have different label selections. Once you have selected your answers, click OK to return to the flowsheet. If laterality is required, and left or right is not appropriate then select N/A or other as appropriate.
14 Dynamic Groups Continued After the label window closes, you can continue to chart on that item. Add as many dynamic groups as needed. For example, the patient has 2 PIVs, therefore, you should have 2 PIV dynamic groups. Each one labeled appropriately in order for all clinicians to be able to identify the information associated to each specific site. If you do not label each dynamic group, you will not be able to sign the charting. If you create more dynamic groups than needed, they will be removed when you sign your charting as long as you do not label them or chart on them.
15 Viewing Reference Text Reference text provides helpful information and links to policies for easy reference. To view this information, click on the blue text in the flowsheet. The reference text window will open. One window may contain several different reference text items which are accessible by choosing between the options of Care Plan information, Chart guide, Nurse preparation, Patient education, Policy and Procedure, if available. To close the window click OK.
16 Customizing Your View To customize your view to add or take away an item from your flowsheet (such as adding an Intra Aortic Balloon Pump), select the Customize View Icon. When you click on the circled icon, the customize view window will open to display all possible charting items for the specific flowsheet.
17 Customizing Your View Continued When the customize window opens, put a check mark in the box next to each item that you want to add to your flowsheet and then select OK. To remove items from the flowsheet because they are not appropriate for the patient. deselect the check mark from the box. You can not take away items that have documentation in them or if the box is grey. Click OK when all desired customization is complete.
18 Customizing Your View Continued When you return to your flowsheet, the items you selected will display and are available for documentation.
19 Associating the Monitor Associate Monitor icon Select the Associate Monitor icon. When the screen below opens, select the specific room number where your patient is located and then select Associate. To disassociate, select the room and select Disassociate. Remember to disassociate each patient upon transfer or discharge. If the patient is discharged and not disassociated, they will automatically be disassociated when the next patient is associated to that monitor.
20 Associating the Monitor Continued When a patient is admitted and placed on the monitor, but not associated in PowerChart at the start of monitoring, you have the ability to back time the monitor association in order to capture all the patient data. Click the Associate Monitor Icon, select the correct monitor and click View Acquired Data. Manually change the time in the upper window to the time the patient arrived and/or was attached to the monitoring device. The system will allow the data from the identified time to be available to be pulled into the patient chart for documentation. Then, click OK to return to the associate monitor screen.
21 Philips Monitor Label Selection If any of your monitored vital signs do not display in the IView flowsheet make sure the label is correct in the monitor. Reference the Philips Monitor Label Selection guide at the end of this module. The label on the monitor, in IView, and in Acquired Data should all closely match.
22 Changing Acquired Data PowerChart acquires data from monitors up to Q1minute. To change the acquired data from the monitor simply right click on that cell and Select Different Acquired Result from the drop down menu to display other values from the monitor. If manual entry is desired, select clear from the drop down menu and type the desired value directly into the cell before signing our documentation.
23 Documenting From the Task List Tasks can either open a form or the IView Activity View. Tasks ending with the word IView always take you to the IView Activity View flowsheet to chart. Some tasks will still open a form, such as the Admission Database. The remainder of the tasks are done/not done tasks and do not open specific questions/forms for documentaton.
24 Documenting From the Task List As an example of an IView task, the Nursing Swallow Assessment task opened the IView Activity View flowsheet for documentation. As an example of a powerform task, the Nursing Admission Database task opened a powerform for documentation.
25 Co-Signing Documentation with the LPN When an LPN performs and documents an assessment, an RN must co-sign the documentation. The validation/co-signature question is documented in the Systems Assessment band at the top of the General Assessment section as seen above.
26 Co-Signing Documentation with the LPN When you open the cell to document, the binoculars appear, click on them to open the provider search window. To search for the provider name, enter the your last name, click search, select your name from the list generated and select OK. You should be returned to the General Assessment section with your name viewable in the flowsheet cell.
27 Copying Duplicate Assessment Details To duplicate the results of one column for entry into another time column, select the desired column by clicking on the time scale header. The entire column will turn blue.
28 Copying Duplicate Assessment Details Continued Select the Actions icon (it looks like a piece of paper with check marks beside it), and then choose Duplicate Results from the drop down menu.
29 Copying Duplicate Assessment Details Continued After selecting Duplicate Results, a Date/Time box will open. Enter the date and time you want to copy the results to and click OK.
30 Copying Duplicate Assessment Details Continued Green check mark is dithered Remember, for all the documentation from the entire column to chart in the new time column, you must scroll all the way down through the flowsheet and look at the purple text before you sign the documentation. Only information that was viewed will become signed charting. If there are any required fields left uncharted, the green check mark will not be available for signing. Once signed the documentation will turn black.
31 Adding Navigator Bands If you have not finished charting or need to modify charting on a patient that has transferred to a med/surg unit, you will have to add the IView navigator bands to the chart to allow access to the necessary documentation items. Also, in special circumstances, you can alter your navigator bands, such as adding the Labor and Delivery Quick View band on a pregnant patient in MICU, when needed. To add navigator bands, go to View on the tool bar and select Layout and then select Navigator Band.
32 Adding Navigator Bands Continued The Navigator Documents window will open. Select the band that you need from Available Document Types and then click the arrow in the middle of the box to move the band to Current Document Types. Repeat these steps for each band needed; then, select OK. The chart must be closed and reopened. When you reopen the chart the selected bands will be available for viewing and documentation.
33 Adding Navigator Bands Continued When navigator bands have been added manually as in the previous slides, you must return the navigation bands to the original layout after completing your documentation. To reset the Navigator bands, select View from the tool bar, then Layout and finally Reset Navigator Bands. After closing the patient s chart, the navigator bands will be reset to the appropriate bands for the patient location. For example, a med/surg floor patient bands only display Intake And Output and Graphs bands.
