1 This job aid will walk you through the steps you need to take to create, open, save and send Safe Strides Individual Outcomes via your ipad utilizing the BLACK ADOBE READER ICON. If the reader you are downloading is BLACK please use the Black Adobe Reader Job Aid Preparing for ipad Utilization 1. Download the free Adobe Reader app from the App Store Creating a Template 1. Download the Individual Outcomes PDF from Gentiva University Resources Specialties or the Safe Strides web page to the Adobe Reader 2. You now have a blank template to use for your future patients in your Adobe Reader Creating Individual Outcomes For each patient you will need to make a new copy of the Outcome Template prior to data entry. 1. Open your Adobe Reader 2. 4 Renaming the Duplicated Document 1. Tap the check mark in the upper right on the Local Screen 2. Tap the copied file to rename 3. Tap the I-beam Icon 4. Enter new name (i.e. MR# with patient initials), then tap Rename Data Entry 1. 5 2. Complete patient demographics and Initial Scores (data will be automatically saved) 3. When finished tap the Home icon 4. Discharge Scores: Repeat steps 1-3 if you are the D/C clinician; if you are not the D/C clinician, the form to the clinician that is doing the D/C so they may complete it ( ing steps below). Fill in the To and tap send Completed outcomes should be ed to your Rehab Director for printing in color and 2-sided for physicians and to have a PDF placed in the patient s EMR.
