1 Release Notes 1/18/ Change PCP order (Inpatient and Outpatient-Expected Date 1/23) There is now an order you can use to update the PCP. The order will task either your clinic staff or the admitting staff to update the PCP on record for the patient. 2. Change PCP PM Conversation (Inpatient and Outpatient-Available Now) If you would prefer to update the PCP yourself, there is now a PM conversation with only that function available to you. To update the PCP go to PM Conversation on the Toolbar. On a smaller screen, you may need to click on this to see the PM Conversation option:. Then select PCP Update:
2 Find your patient in the search box: Choose a location (any will work) Enter the name of the PCP (you can only select names from within the database of physicians, this includes VCHCA physicians and community physicians)
3 3. Relationships Updated (Inpatient and Outpatient-Available Now) The relationships that you are asked to select when opening the chart were updated. Most physician and non-physician providers will see these options: Selecting the correct relationship is helpful for many reasons. See the primer on relationships below for more information about the types of relationships and what they mean. Primer on Relationships There are 2 different types of relationships, lifetime relationships (that are effective for any visit or encounter and do not expire) and encounter specific relationships that are limited to one patient visit or inpatient stay and expire after a certain amount of time. There is one lifetime relationship available to physicians and non-physician providers: PCP. This field is assigned through registration. Only one person can be assigned as the PCP. For coordination of care, it is very important that this field is populated and accurate (see the section above on changing or assigning the PCP). There are other relationships that are also assigned through registration: admitting physician, attending physician and referring physician. These relationships are limited to the specific encounter/visit where the relationship was created. Supervising physician may be assigned when a resident, PA, or NP places orders in the chart. It is also encounter specific. If you do not have a relationship assigned one of these ways, you will be asked to select your relationship with the patient when you open the chart to declare the reason for accessing the patient s Personal Health Information. Select the option that is most relevant to your relationship with that patient during that visit.
4 One of the reasons to select the correct relationship is for using results FYI to get results forwarded to your message center inbox when they are resulted after the patient is discharged. Results FYI Subscriptions - Message Center 1. Open Message Center. 2. Click the Inbox menu and then click Manage Preferences. 3. Click the FYI Subscriptions tab in the Setup window. 4. Click Add to create a new subscription. Hint: You can choose to assign yourself the Inpatient Attending and Admitting relationship which will forward you results posted after discharge on all patients where you are assigned as the admitting physician or attending physician from registration (through the admit/transfer order). To add this subscription, click on Inpatient Attending and Admitting and then click Assign:
5 5. To create a new, custom subscription, type the desired subscription name and description in the Name and Description fields. Selecting options in the Filters pane allows you to determine when the results will display in the Results FYI folder. You may select multiple options within the Lifetime Relationships and/or Encounter Relationships panes. Lifetime subscriptions post results across all encounters (the only lifetime relationship we have active is Primary Care Physician). Encounter-based subscriptions post results related to the encounter with the selected relationship(s). 6. Select the appropriate option(s) in the Lifetime Relationships and/or Encounter Relationships panes. NOTE: If an option is selected in error, deselect the option by clicking the green check mark in the check box. The Relationship option in the Type pane is the default selection.

Choose what type of postings you wish to include: The Late Postings field defaults to Exclude meaning all selected results during the active encounter from the time of relationship activation to the time of relationship deactivation will appear in the Results FYI section. The Include options allow you to receive results throughout the encounter AND after the patient is discharge. The Exclusive option for forward only results posted after the encounter is discharged. 8. Click Add in the Event Types pane to select the results you wish to display in the Results FYI folder. 9. Double-click the ALLOCFSETS folder in the Result Hierarchy Browser window.
7 10. Click the plus sign (+) in front of the ALLRESLTSECT and ALLSRVSECTS folders. Continue clicking the plus sign in front of the nested folders to select very specific results to include or exclude. 11. Select the appropriate folder. 12. Click OK to close the Result Hierarchy Browser window. NOTE: Multiple event types may be added to the Event Types pane; however, only one event type may be selected at a time. Repeat steps 8-12 to add additional result types. 13. Click Include to include the selected event type results in the Results FYI folder. Hint: To include lab results: click on the + sign next to ALLRESLTSECT, then select Laboratory. To include radiology results, click on + sign next to ALL DOCUMENT SECTIONS folder and select Radiology.
8 14. Select the appropriate option(s) within the Encounter Types pane to filter the results notifications by encounter type. 15. Click Include. NOTE: The Exclude option is the default selection in both the Event Types and Encounter Types panes. The Exclude option should remain selected if a provider wants to exclude specific results and/or encounter types from the Results FYI folder. 16. Click Apply. The new subscription displays in the Assigned Subscriptions pane under the Personal heading.
9 17. Click OK in the Set Up window and then click OK in the Commit Progress window to save the settings and close the Set Up window. Results will display in the Message Center Results FYI folder and subfolders based on your relationship to the patient and the subscription filter selections. NOTE: Only values resulted after a subscription has been added will display in the Results FYI folder. Values resulted prior to the addition of a subscription will not display
