1 Page 1 of 7 Part A: Initial CMAT Registration All volunteers, even those who have previously registered with CMAT are required to create an online profile on the new CMAT database. This only needs to be done once. 1. Click on the Volunteer tab of the CMAT website and select Register Now!, or go to: 2. To create a profile, click the New Applicant Registration link in the upper-right corner of the page. (Green arrow below). Once you have created your profile, you can log back in at any time on this same page to keep your profile current. (Blue arrow below). Be sure to enter your Captcha phrase correctly. It is case-sensitive. This helps prevent spam. New Applicants click here Once registered login here 3. Complete the initial registration page with your information. Areas marked with an asterisk (*) are mandatory. Note: CMAT strongly suggests using a personal address, instead of a workplace address. Profession: Please choose from the list of professions provided, and specialty (if applicable).

2 Page 2 of 7 If you are not a health professional, or your profession does not appear on the list, please choose other. Terms, Declaration and Registration: This area must be completed in order to be eligible to volunteer with CMAT. Please read these statements carefully. 4. Once complete and submitted, you will receive an to acknowledge your registration. The CMAT Deployment Coordinator will then review your application. Once approved by the Deployment Coordinator, you will receive another acknowledgment . 3 Page 3 of 7 Part B: Completing your CMAT Profile 1. Now that you have registered on the CMAT database, and have been approved by the Deployment Coordinator, you will need to complete your online volunteer profile. Login using the address and password you provided in Part A, as well as the Captcha phrase. After logging in, you will come to the main screen. Click on My Profile. Click on My Profile 2. On the My Profile page, you will see the registration profile you created in Part A, as well as a few other options. Availability Menu emergdoc@nomail.com Personal Details

4 Page 4 of 7 3. To change your availability, select from the Availability drop-down menu (Red Arrow above). Please ensure this is kept current. Availability Options include: Available: able to depart at any time with minimal notice (24-48 hours), and with no other challenges (e.g. work schedule, childcare arrangements, vacation time etc.) Tentative: able to depart with 1-2 weeks notice, pending further arrangements (e.g. changing work schedule, arranging childcare etc.) Unavailable: unable to deploy at this time (e.g. currently on medical or maternity leave, inflexible work schedule etc.) 4. Your personal details can be amended at anytime (Blue Arrow above). It is your responsibility to ensure that contact information is kept up to date, especially address and telephone number. CMAT strongly recommends that you use a personal address for CMAT communications, not a workplace address.

5 Page 5 of 7 Part C: Uploading Documents 1. You will need to upload several scanned documents to the CMAT database, preferably in PDF or JPG format. These documents must be under 500kb in size. Click on My uploaded documents (Purple Arrow Below) The following six documents are mandatory for submission to CMAT and must be kept current, prior to being considered for a volunteer deployment: A. Photo page of your current passport, with a clear view of the photo (Permanent Residency Card) B. Scan of your professional designation card (or a screenshot of your provincial regulatory body s public register page if you do not receive a renewal card) C. Scan of your CPR certification D. Scan of your Police Clearance Letter E. Scan of CMAT Deployment Volunteer Liability Waiver (Signed and witnessed) (sent to you by Deployment Coordinator via , prior to deployment). F. Health Certificate and Scan of Immunization Card A Health Certificate is simply a note from your primary care provider stating that you are physically, mentally and emotionally fit and able to tolerate working in remote, unstable, and highly stressful environments. This note must be renewed prior to all deployments. The following additional documents are strongly recommended for submission to CMAT, and should be kept up-to-date: Resume/ CV (please put upload date as Valid From date) Additional professional certifications (e.g. ACLS, PALS, ITLS etc) Driver s license (if applicable) Click on My uploaded documents 2. Choose the type of document from the Type drop-down menu. Choose document type Waiver

6 Page 6 of 7 3. Enter a brief description of the document (e.g. ACLS card) 4. Enter the certificate number if applicable (e.g. Passport number, professional registration number) 5. Enter the Name of the Issuer (e.g. College of Nurses of Ontario, Heart and Stroke Foundation) 6. Enter the valid from date. For annually renewed certificates, enter January 1 st or the appropriate validity date. 7. Enter the valid to date (expiry). For annually renewed certificates, enter December 31 st or the appropriate expiry date. 8. Upload the supporting documentation PDF or JPG file type. File size must be less than 500kb. 9. Click Add certification. (Purple arrow below) 10. Certification will then appear under Certifications (blue arrow below), and will be marked pending until reviewed by deployment coordinator. Click view to view your uploaded document. Click delete to delete your document. (Red arrow below) You cannot undo a deleted document.

7 Part D: Uploading your Profile Picture Page 7 of 7 CMAT requires all members to submit a passport-style photograph. This does not need to be professionally taken, however the image must be a clear view and true likeness of your face, head and neck, on a neutral background. Please do not wear a hat in the photograph. (Head coverings for cultural or religious purposes are exempt). This photograph is used to produce your identification card, to identify members, and must be recent within the past 2 years. 1. Please save your photo.jpg format with your individual filename formatted with no spaces: LASTNAME-FIRSTNAME-CURRENTYEAR.jpg (e.g. DOE-JANE-2014.jpg). Ensure that the file size is less than 200kb. 2. Click on Profile Picture (Green Arrow Below) 3. Click on Choose file. (Red Arrow Below) 4. Once your file is selected, click on Process Image (Blue Arrow Below) 5. Once uploaded, your file will appear on the right. (Purple Arrow Below). You may delete your photo and upload a different one by clicking the delete button below the image And that s about it! The rest is up to us!
