2 Deadline to Submit Registration Submit early!! The deadline for the registration packets to be mailed back to College Board for the March 5, 2016 test is February 5, 2016 If envelope is not post-marked by that date, College Board will not complete registration for the SAT test.

3 What do you need? To complete this registration information, print in CAPITAL letters and use a blue or black pen NO PENCIL

4 For registration to be processed All items in red must be completed Do not staple anything to the form

5 Item 1: Name Enter your FULL legal name EXACTLY as it appears on your school information Enter your last name starting with the far left box; if you run out of room enter the 1 st 15 letters S M I T H Enter your first name starting with the far left box; if you run out of room enter the 1 st 12 letters J A N E Middle initial is optional B

6 Item 2: High School Code Because you are in middle school, you do not have a high school code. All must enter in boxes Bubble in the correct numbers underneath.

7 Item 3: SEX 3) Fill in correct oval o Female o Male

8 Items 4 & 5:Grade Level/Date of Birth 4) Current Grade Level Everyone should bubble: Not yet in 8 th grade 5) Use MMDDYYYY format to fill in Ex- Birthday is 02/15/2003 Enter Bubble in matching numbers underneath

9 Item 6: Photo Test-takers in the 8 th grade or below are exempt from this requirement

10 Items 7 & 9: Social Security and Student ID Number These fields are not required.

11 Items 8: Expected Graduation Date 8) Expected Graduation Date Everyone should write: 06-21

12 Item 10: Mailing Address SAT cannot process your registration without a full address Bubble US/US Territory Fill in address blocks starting with far left box Use standard abbreviations (such as St for street) to fit address into spaces provided

14 Items: 12 These items should be completed with a parent or guardian as they include payment information and information that should be decided with an adult.

15 Item 13: SAT Questionnaire This questionnaire (questions 1-34) deals with a student s high school experience. Since you are in middle school, this does not pertain to you and you do not need to answer these questions.

16 Item 14: Statement & Signature Copy the statement in cursive and sign the form. Forms that are not signed cannot be processed and will be returned.

17 Item 15a: Test Date Testing at the three middle school sites is only being offered on March 5, Parents- If your child cannot test on this date, please mark the date that he/she will be testing. If testing on an alternate date, you will be responsible for arranging testing with that high school.

18 Item 15a: Test Choices and Fees The cost of the SAT exam is March 2016 $ Enter $43.00 in the boxes.

20 Item 16: Test Center Codes (cont d) Parents If your child is testing on a different SAT test day due to a scheduling conflict, you will need to contact the high school where your child will be taking the test for their test code. Enter the test center code for that high school. Contact the high school guidance department for specific test center code.

21 Items 17 & 18 Score Reporting 17 a : enter a/b/c: You do not need to fill in or pay additional fees; you can skip this box.

22 Item 19: Non-US Regional Fee In item 20 Enter $ This is a non-us regional fee and is not applicable to students testing in US

23 Item 20: Totals Enter cost of SAT exam here: Leave these blank. No international fees or taxes. No other services or fees Enter total here: No fee waivers will be accepted for this 7 th grade exam. If paying by check, make check payable to the College Board.

24 Item 21: Credit Card Information If paying by credit card: You can enter the information from left to right in the boxes provided. Please include expiration date and billing zip code.
