1 2018 Purple Army Band Camp Form Checklist This year Jackson Local Schools is moving to online medical forms. Once your form is filled out for band, you will not need to fill it out again for other activities! Deadline for this form, payment, and online submissions is May 31st. ****You must submit a printed copy of this page with your fees.**** Payment for band camp fees: $225 cash or check made out to: Jackson Band Boosters Student Name: Check/Cash amount: Check number: Amount requested from trip account: Instructions for accessing the online forms are located on the Jackson Band web page. Online Medical and Mount Union Authorization Forms Online Band Camp Rooming Request and Travel Authorization Form Printed Prescription Medicine Authorization Form (if necessary, must be signed by physician) Deadline for this form, payment, and online submissions is May 31st. ****You must submit a printed copy of this page with your fees.****
2 JACKSON LOCAL SCHOOL DISTRICT S ONLINE FORMS INSTRUCTIONS Click on the payschools.com forms link from For parents who have not already created a parent account for forms, you should create a parent account. To do this, click the Registration button and follow the prompts. You will need your student s ID (same as lunch number) and will need to enter first and last name. If you already have a parent account for forms, login to your existing parent login. If you don t remember your password, you can click Password Recovery. Once you have logged in you will see the following screen with your students listed. If you have just created an account or don t see your student listed, click the Add student button at the bottom left corner of the screen. You will then be asked to enter the Student ID, First and Last Name of the student. Choose Enter Forms to enter or update forms.
3 Once you have clicked Enter Forms, you should see your student(s) in a blue box. Click the name of your student to begin or edit a form. Click Fill Out Forms to enter a form for the first time. If you have previously entered a form and clicked sign and submit, you will see the form under the Signed Programs section. You can click on the pencil icon to review or update your information. You can click on the download arrow to download a copy of Click the arrow to download your completed form. a copy of the form Click the pencil to edit a form Fill out each section of the form completely. You will see a green check at the top of each section when you have entered all required information. When you have filled in all answers and each section has a green checkmark, click on Save and Mark Complete Once you save and mark complete you will see the following screen. You need to click Sign Completed Forms. Enter your initials and then click Submit Forms If your form is associated to a fee you will receive an with a remit payment link. 4 Instructions for Completing Online Forms The instructions on these pages will walk you through completing and submitting the online forms. You must have created an account (see first two pages) prior to following these steps. Step 1 Your first screen should look similar to below. Select enter forms once your student s name appears.
5 Step 2 This screen is used to select the student for whom you would like to complete forms. Click on the student s name, then select the checkbox next to Band Camp 2018). Finally, click Fill Out Form(s)
6 Step 3 This screen is where you will complete the forms. You will likely see all red warning symbols when you first start. Use the Expand/Collapse All button to see where you need to enter your information.
7 Step 4 Once all forms are completed, you will see green checkmarks next to each form. Click on Save and Mark Complete Step 5 This is the box where you will sign and submit your forms. Make certain to enter your address, check the box, and click next when it appears.
8 Step 6 This is the screen you will use to complete the signature process and submit your forms. Please make note of the forms you are signing. The text for the University of Mount Union Release is available at the back of this packet. All forms are required for band camp. If your student needs a prescription medical form, you will need to print it after signing.
9 Step 7 Once all forms are complete and signed, you should print a copy for your records. You may do this from the Download Student Forms button. If your student needs a Prescription Medication Administration Authorization Form, please print it, get it signed by a physician, and send it to school.
