Transcription
1 In order to submit a request for your military training to be evaluated for college credit or obtain other services online, you will need to have a CCH account on the College Credit for Heroes website. Click the Create new account link and a box will open. Ensure Register as a CCH Acount Holder is checked. Then check the box next to each of the two certification/consent statements, as shown in the illustration on the next page. Click the continue button when finished. 1
2 When you click continue another box will open asking for some general information such as your name, address, branch of military service etc. Fields marked with an * are required fields. 2
3 Ensure the Purpose for Visit is set to Requet Military Evaluation. When you are done entering your information click the Finish button. The following status box will appear. Congratulations! You have successfully created your CCH account. You will receive an at the address you entered during the registration process containing your CCH password. Once you have your provided CCH password go back to the College Credit for Heroes main page. 3
4 Click on Login, Enter your address and CCH provided password and click Log in to sign into the portal. Your Provided Password When you click Log in you will be taken to the home screen where you will see the following. 4
5 To request an official evaluation, click on the My Services link at the top of the page Once on the My Services page click the official evaluation link 5
6 A box will appear with some basic information like your name, date of birth, pay grade, last four, and address that will self-populate. Fields marked with an * are required fields for you to fill in. 6
7 Click the Save button when you are finished entering your information. You should now see a screen similar to the one on the following page, but your personal information will be displayed. NOTE: If you are NOT uploading scanned documents you can skip ahead to page 8 now. If you have any of the following: DD form 214, DA Form 1059, ERBs/ORBs or course completion certificates you may upload them onto the evaluation request so that it may be looked at. To begin uploading please see the following page. 7
8 Please Note, DO NOT alter or blackout any part of the document or it cannot be used and will delay or even prevent evaluation from being completed. To begin uploading document(s) click on Add Documentation. When the screen below appears enter a descriptive name of the document, select document type from the drop-down menu, enter the current date, and then use the Browse button to select the location on your computer where the file to be uploaded is located. Click Submit when finished. 8
9 You will be returned to the previous screen, where you can now see that your uploaded document(s) has been included in the Attached Documentation area. You may click on Add Documentation as many times as necessary to upload all additional documents. As an alternative to uploading documents through the CCH Portal, you may bring your document(s) into the CCH offices at Central Texas College campus in Killeen, Texas. You can also fax your document(s) to our dedicated fax, (254) or scanned document(s) to our office address: If you fax or your document(s) to us be sure to include your full name and last four of your social security number so we can match your documentation to your evaluation request. If you are done uploading document(s) for inclusion with your evaluation request, or if you don t need to upload any documents you must place a checkmark next to Submit for Processing and then click Save 9
10 NOTE: Not entering a checkmark in the Submit for Processing box only creates an evaluation request. In order to process the evaluation the box must be checked and then click on Save. You should now see the screen below Congratulations! You have successfully submitted your evaluation request to CCH. 10
11 Click on Login, Enter your address and CCH provided password and click Log in to sign into the portal. Your Provided Password When you click Log in you will be taken to the home screen where you will see the following. 11
12 To request an official transcript, click on the My Services link at the top of the page Once on the My Services page click the Online Transcript Request link 12

Please Note: DO NOT fill in anything in the Send to Self section. Instead when the information above that section is completed Click the Save button when you are finished entering your information. You should now see a screen similar to the one on the following page, but your personal information will be displayed 13
14 If you plan to have a CCH transcript sent to another institution please first click on Add Participating College. Once you click on the blue link that area will expand. Click in the box and check to see if your institution is on that list. If so highlight that institution the click on the Add College button. 14
15 Once you have done this the screen will refresh and the only change will be in the middle where you will see the following, depending on the institution you have selected. To have a transcript sent someplace other than a Participating College please see below: If you are not sending the transcript to one of the Participating Colleges but to another institution or anywhere else click on Add to Recipient List Here you will be able to determine how the transcript will be sent ( Send Method ). Select from the drop down whichever method you want. Will Pick Up applies only to those that plan to come in to our office to pick up the transcript. After you have determine how to receive your transcript the next step is to determine to whom (Recipient Type ), be it another Texas College (not on the participating college list), Non-Texas College, an Employer, or if you want an official transcript sent to yourself. Click on whichever one applies. If you want an Official Transcript sent to yourself ensure to us Other DO NOT use Self. Then proceed to fill in the fields from the section. If sending transcript to yourself ensure to put your full name in the Send To section. Ensure to use YOUR information as this guide is just an example. 15
16 Once completed with filling on the blocks click on Add Recipient. The screen will refresh and you will see the following: The final step to submitting your transcript request is to check the block to the left of the phrase Submit for Processing followed by clicking on the Save button. Once you have done that you will see the message to the right. This completes the transcript request process. Congratulations! You have successfully submitted your transcript request to CCH. 16
