2 WARFARIN RESULTS ANYTIME ANYWHERE Now you have the freedom to receive your warfarin dose instructions and INR test results as soon as they are available. Anytime. Anywhere. Sullivan Nicolaides Pathology patients, or their carers, can now elect to receive warfarin dose instructions and INR test results via our Webster On-Line Results Service. No phone queues. No waiting for a letter. Just access your results on-line.
3 HOW DOES THE SERVICE WORK? STEP 1 Register Please log onto our website, to register your interest or contact us on (07) or STEP 2 Confirm Once you have registered your request to receive your dose instructions on-line we will contact you to register you for this service. STEP 3 Ready-to-go Proceed with your dose instructions as provided. The next time you have a blood test, instead of receiving a letter or phone call, you will be able to access your INR result, dosage, and next test date on-line. Below is an example of how your screen will appear.
4 FREQUENTLY ASKED QUESTIONS How do I enrol to access Warfarin dose instructions on-line? If you would like to register for this service please read the information in this brochure then log onto our website, or contact us on (07) Once you have registered your request to receive your dose instructions on-line we will contact you to register you for this service. After this we will send you an notification which will contain your user name. For security purposes your password will be sent via the post; which could take up to 3 working days. As part of the verification process you will be required to logon and enter your date of birth and will be asked to read and accept the terms and conditions of this service. On this first occasion of logging into this web page you will be asked to change your password (your new password can be up to 8 characters in length and must contain one 1 upper case letter and 1 number). You will now be able to view your dose instructions. Please allow up to one week for registration. What do I do to view my results on-line? Go to and click on the Patient Services drop down box, select Warfarin Care then click on Warfarin Care Webster on-line results logon. Enter your User name (that would have been ed to you upon registration) and Password (that you would have received via a letter). To maintain the integrity of your password, please do not tick the Remember My Password tick box. For more information please see the Patient Help Guide under the 'help' banner. If you do not access you results within 48-hours of them being available on-line, you will either be sent a letter or receive a phone call from Warfarin Care to give your INR result, dosage instructions and test date. If you repeatedly fail to access your on-line results in this timeframe we may contact you to clarify any problems. If appropriate we will change your designated contact method to either letter or phone call, depending on your results. Please note you will no longer receive a phone call or letter with your dose instruction when you are registered for on-line results notification unless Warfarin Care deems it necessary.
5 FREQUENTLY ASKED QUESTIONS Why is it important for me to confirm YES? Your reply is automatically registered in our system to indicate that the notification has been sent and received successfully. In addition, because of the importance to your health and safety, Sullivan Nicolaides Pathology needs to ensure that you have read and understood your dose instructions and are aware of your next test date. It is important for your health that you take the appropriate dose of warfarin every day, so if you do not understand your dose instructions please contact Warfarin Care immediately. What if I forget to confirm 'YES'? If you do not access your on-line results within 48-hours, you will either be sent a letter or receive a phone call from Warfarin Care to explain your dosage and next test date. Our system will also record a 'nonresponse'. If you repeatedly fail to respond to log-on to access your results on-line we will contact you to clarify any problems. If appropriate we will change your designated contact method to either letter or phone call, depending on your results. What happens if my result is abnormal or too high? If your result is concerning to our doctors and poses a risk to your health you may receive a phone call from Warfarin Care staff in addition to being able to access your results on-line. What if I don t understand the on-line results? If you do not understand your on-line display or unsure about your dose instructions please phone Warfarin Care on (07) as soon as possible. What do I do if my results are not available? Sullivan Nicolaides Pathology aims to provide your result and dose within 24-hours of your test. If your on-line results are not available within this period, please take your currently prescribed dose and phone Warfarin Care on (07) What if my nominated address changes? It is your responsibility to contact Warfarin Care on (07) as soon as you or your carer change the nominated address or any other contact details. If you have any questions about the On-line Results service please phone Warfarin Care.
6 ADDITIONAL INFORMATION Please phone Warfarin Care on (07) if: You do not understand your dose instructions You now have additional information about your health, medication or warfarin dosage that wasn t recorded when you had your most recent blood test At any time you or your carer feel uncomfortable receiving your dosage instructions on-line you can opt out of the service by contacting Warfarin Care.
