Enter your name, address, and create a username and password. We have a strict privacy policy for your protec5on. To successfully complete The Course to become a Registered Prac55oner with the NYSDOH, you must complete the THREE Required Syllabus Topics AND at least ONE specific Disease State Syllabus Topic (included in your purchase).

2 From the homepage, click the Study Now tab to access the Medical Educa5on Library Once you are in the Medical Educa5on Library area, select New York State Prac55oner Educa5on-Medical Use of Marijuana Course The syllabus topics for this course will then appear in the list at the boyom of the page. The arrow in a green circle to the lez of the list of specialty areas indicates that the New York State Prac55oner Educa5on-Medical Use of Marijuana Course library is being displayed. 3 Click on the Study Now link next to Syllabus Topic I to begin The Course.

4 Review the ayesta5on page, check the box, and then click Start Course. Con5nue scrolling to the next page.

5 Now you should see, Syllabus Topic I, Part 1. Read each of the Ques5ons and Answers in the drop-down menus and then proceed to the Next Part by clicking the blue Next Part buyon in the upper right corner (each syllabus topic has several parts). Con5nue scrolling to the next page.

6 If you d like to see all of the ques5ons and answers of a Part on one page, click View En5re Ques5on & Answer. This View En5re Ques5on and Answer area also allows you to print the en5re part.

7 If you have clicked the View En5re Ques5on & Answer link and want to go onto the Next Part of the syllabus, click close window and click the blue Next Part buyon to navigate to the next part of the syllabus topic.

8 AZer reviewing all the parts of a syllabus topic, click the To earn CME link above the ques5ons, or the blue CME buyon at the boyom of the page, to navigate to the CME Crossword or Quiz.

9 Complete the Crossword or Quiz (your choice). In order to earn CME credit, you must score at least 70%.

10 Con5nue with Syllabus Topic II, Syllabus Topic III, and at least ONE Disease State Syllabus Topic, and follow the same outline as above. You do not receive addi5onal CME credit for the disease state topics. However, you can complete as many of the disease state topics as you wish for your educa5on. Once you have passed all three Syllabus Topic and at least ONE Disease State Syllabus Topic tests, you will have earned 4.5 hours of AMA PRA Category 1 CME Credits and now be eligible to be a Registered Prac55oner with the NYSDOH. To view your CME ac5vity and to print a CME cer5ficate, go to the My Account tab and click the My CME op5on. Con5nue scrolling to the next page.

11 From the My CME area, you may print your CME Cer5ficate by clicking Print A Cer5ficate. Con5nue scrolling to the next page.

12 Then enter the desired dates and click Calculate Credits. Con5nue scrolling to the next page.

13 Finally, click Request Cer5ficate. Con5nue scrolling to the next page.

14 Your CME cer5ficate will be ready for prin5ng and saving as a pdf file.

16 Prac55oners who have completed the course and who wish to register with NYSDOH to cer5fy pa5ents for medical marijuana must follow the below steps: 1. Log into the Health Commerce System 2. Select All Applica5ons from the My Content dropdown menu 3. Click the M tab, find the Medical Marijuana Data Management System (MMDMS) applica5on in the list, and click the green and white + icon in the Add/Remove column to add the MMDMS applica5on. 4. AZer selec5ng the MMDMS applica5on, prac55oners will be prompted to enter informa5on to complete the registra5on process.
