1 Gold Standard Award for Optimal Aging Nomination Instructions Thank you for your interest in submitting a nomination for the Gold Standard Award for Optimal Aging! Please review the following information carefully before submitting your proposal. Nomination Deadline: Saturday, April 15, 2017 at 11:59 pm Where do I submit my nomination? Please submit your proposal at: You will be taken to the Gold Standard Award for Optimal Aging s page. Please note: all nominations must be submitted via the online application. If this is an issue for you, please contact us by OptimalAging@louisville.edu or by phone (502) How do I submit my nomination? There are two ways to log into the online application: 1. Existing Users: if you already have a username/password: If you receive s from the Institute or have attended Institute events in the past, you already have a username and password. Your username can be found in your most recent from the Institute. Please fill in the information as prompted on the Login page. If you cannot remember your password or username, please click on the help links provided below the existing users login section, shown below.

New Users: if you DO NOT already have a username/password: If you do not have a username/password for the website, please enter your in the "New Users" section of the login page and click "Register." **For University of Louisville student/faculty applicants: please note, your username/password for the Institute's website are not connected to you ID/password for the University. What are the elements of the nomination application? Part I: Nominator Information Nominator Name Please provide appropriate prefix and suffix. Affiliation Provide the organization with which you are professionally affiliated (i.e. academic institution, service provider, etc). If you are retired, please indicate Retired in this field. Feel free to share a previous affiliation when relevant to your proposed presentation. Position

Physical Address Please provide the you would like to be affiliated with your nomination submission. We will conduct all communications via the you provide. You are given space to write on how your nominee or couple is living optimally in the following categories: o Physical: How they stay active and stay physically healthy. This can include health-conscious eating habits, exercise routines, etc. o Social: How the nominee stays in contact with friends, family, etc. o Spiritual: This is a relatively broad category. It involves church projects, groups, community involvement, gardening, whatever the nominee does feel spiritual. o Creative: This could include but is not limited to art projects, interior design, singing, cooking, etc. You must limit your description for each of these categories to 100 words. You may have less than 100 words, but no more. Next, you are given space to summarize the nominee in 250 words or less. Only the first 250 words will be used to describe the nominee so please limit yourself to 250 words. Part IV: Summary
