1 OHIO SOUTH STATE LEAGUE Season U15-U18 Team Carding Instructions NEW TEAMS TO OSSL Carding Closes: November 7, 2017 **IMPORTANT** Processing of cards can take up to three business days or more depending on the number of cards being processed at a certain time! Please plan ahead. Teams will be charged for expedited service. Teams playing in OSSL and the Buckeye League: Teams are to card through the Buckeye system first. Teams can use the Buckeye membership forms for OSSL. After receiving Buckeye stamped approval on the Buckeye roster and cards, send a copy of the carding materials, non-laminated cards, and Buckeye roster to OSSL for approval. Teams will still need to enter the player information in the Affinity system. U15 Teams If you played U15 in the Fall and carded through Buckeye or another league, you have to continue to use your original roster and cards even if you are no longer playing in that league in the Spring. Teams will still input player information into the OSSL system. OSSL will stamp your original roster from the Fall and any new cards created. TO BEGIN CARDING 1. Teams will not print coach and player cards. Teams are responsible to enter player information and upload player and coach color pictures. OSSL will print the cards. 2. Teams along with manager and coach information included in the application have been created in the system. Jen Schuppe at jschuppe@osysa.com with the names of any new/additional head coaches, assistant coaches, and team managers. If they are new to OSSL, please include their birth date, home address, address, phone number, and team role. 3. Please keep in mind: Each team must have at least two and a maximum of three coaches listed for carding. At least one coach on the team s OSSL roster must hold at the minimum a US Soccer Provisional D Coaching License or NSCAA National Diploma. o A copy of the coach s license meeting the requirement must be included with the carding materials unless OSSL has a copy on file. Please check the OSSL website for a list of coach s licenses OSSL already has on file. Pre-filled player membership forms will automatically print during the online registration process. Teams can use blank membership forms found on the OSSL website. NFHS concussion certification is valid for three years. Coaches with a certification prior to July 2015 will be required to submit an updated NFHS concussion certificate. Please check the OSSL website for a list of coach s concussion certification dates on file. 1

To begin carding, find your club s URL in the document OSSL 2017-18 Club URLs found on the OSSL website under the Carding Forms U12-U18 Age Divisions. 1. Click on your club s URL.

Select the Fall 2017-Spring 2018 season and then select your team from the drop down menu. Follow the instructions on the page to determine your team name/number in the list.

2 To begin carding, find your club s URL in the document OSSL Club URLs found on the OSSL website under the Carding Forms U12-U18 Age Divisions. 1. Click on your club s URL. You should see a screen similar to the one below with your club s name in the text. 2. Click the Registration tab at the top of the page to get started. 3. Select the Fall 2017-Spring 2018 season and then select your team from the drop down menu. Follow the instructions on the page to determine your team name/number in the list. If you do not see your team, please contact Jen Schuppe at jschuppe@osysa.com. 4. Enter your Affinity system username and password and click Login. If you cannot remember your username/password, click Forget UserName/Password. 2

3 5. On the first screen, click Continue>>. DO NOT CLICK Add New Player!!! You will now be adding players to the team. You will need player information, the player s parent information and as well as the player s jersey number. Please note if you will be adding only a few players at a time, you must go through Step 10 where you accept the ELA to save the information. 6. Ignore the message: You will first need to Add to Team as Coach/Admin. Click Add other Players to Team IGNORE 3

7. Under the Add to Team heading, enter your first player s information

Please continue click Save and continue adding other players.

You will verify player information and can upload photos at this point.

Please continue click Save and continue adding other players. The matched player will be added to your roster. 8. You will verify player information and can upload photos at this point. You will have another opportunity to upload photos. DO NOT upload birth certificates. Click Save. 4

NOTE: If you logout and log back in through your club URL and click the Registration button to add more players, you will NOT necessarily see the players you have already added. You will have to log in to your team account to see the players. Follow the directions below to log in to your team account. 12. You do not need to print any forms so click Log out and back to [My Account Login] page. 13. Click the User Login tab located in the upper left hand corner of the screen and enter your Username and Password and click Login. 6

You will now edit the player ID# and add player jersey numbers. You can add jersey numbers at another time. You can also upload color pictures to print on the player/coach card. 14.

Once in My Account, look under the Teams heading and click Team Info next to your team. Make sure the Season is Fall 2017-Spring This page shows the information for coaches and players that will be used to create your team roster. Click on the Team Roster Tab. Click on a player s name or ID number to edit the system generated ID#. 16. To edit the player ID#, click the pencil icon and then enter the number in the Update ID Num box and click Update. You will not see the updated number until the screen is refreshed. Please see below on how to generate each player s id number. 7

For teams carding through Buckeye: i. Please use the Buckeye number assigned to the player. 17. After clicking Update, upload a color picture for each coach and player. Player pictures will need to be updated each season. DO NOT upload birth certificates. To upload a picture, click on the picture icon and follow the instructions. To see changes, hit page refresh. 18. Repeat these steps for all players. 8

9 19. Now enter player jersey numbers. Click the Team Roster tab and then click on the Edit Player Team Info button. 20. Enter BOTH Home and Away jersey numbers (this will be the same number) for each player and click Save Changes. 9

10 You are now ready to print the Official Roster and Player Membership Forms. 21. To print the player membership forms, click on the Player Application Forms button and print the forms. Parents will have to fill out and sign the bottom portion of the form (three signatures). Blank Player Membership Forms can be found on the OSSL website under Carding Forms U12-U18 Age Divisions heading. 22. For coaches membership forms, go to the OSSL website under Carding Forms U12-U18 Age Divisions and click on the Coach Membership Form. Fill out the information and have the coach sign the form. 23. To print the team roster, click Print OSSL Roster. Review the roster for correct player and coach information.
