1 EIDM (Enterprise Identity Management System) Signing Up for QARM Requirements: Must have already created an EIDM Account 5 Minutes must have passed since EIDM Account Creation 3 Step Process Register for access to QARM Complete Experian Identity Proofing Set up Multi-Factor Authentication

2 Using EIDM to Register for Access to QARM 1. Open 2. Click Login to CMS Secure Portal Click Login to CMS Secure Portal

3 3. Click the I Accept button to accept terms and conditions Click I Accept

4 4. Enter your EIDM User ID and click Next Type in User ID (name) created with EIDM account. It is most likely your address. 5. Enter your Password and click Log In Type in the Password created when signing up for your EIDM account.

5 Note: If the password screen asks for an MFA Device Type and you do not remember setting one up you can: A. Click on Register MFA Device and follow instructions to complete setup (Review Setting Up Multi-Factor Instructions for details) OR B. Select One-Time Security Code from the MFA Device Type Dropdown (will be sent to the e- mail you registered with) OR C. Select Unable to Access Security Code link (will also send to your after you enter your User ID and answer your challenge questions) The Security Code is needed to proceed. All security codes sent to /phone expire after 30 minutes. Once you receive the MFA code, enter it in this field as the Security Code and click Log In

6 6. On the Welcome to CMS Enterprise Portal page, click the Request Access Now button Your user name will appear at the top right of the page if you are logged in Click here to request access to QARM

7 If you see QualityNet Authorization and Role Management (QARM) under My Access (on the right of the screen), you have already registered for QARM. You can log out of EIDM and proceed to to log into the QARM system. If you see (QARM) under My Access as in the example above, you do not need to continue with these instructions. Goodbye.

8 7. Search for QARM in the Access Catalog Type in the letter Q to filter the list and easily locate QARM from the list If you type in Q and no applications are found, look to the right and see if you have already registered for QARM. If so, you will it listed under My Access.

9 8. Once you have located QARM, click Request Access

10 9. Select QualityNet User from Select a Role drop-down QARM will already be selected in Application Description. DO NOT CHANGE. Select QualityNet User as your role. No other roles apply to you. 10. Click Next Important Note: Do NOT select CCSQ Security as your role. CCSQ Security is NOT the role of Security Official. Selecting any role other than QualityNet User will cause your account request to be rejected.

11 Complete Experian Identity Proofing After submitting the request for QARM access, user will be taken to Identity Verification Page. Experian Identity Proofing replaces the notary/security Official paper process used by QIMS (retired) to verify applicant s identity Experian Application poses questions based on personal and credit report information it has on file You have 10 minutes to complete the process. If you do not complete within the time frame, you must begin the process again Information provided is visible only to the applicant (or anyone peeking over applicant s shoulder do this in a private area!)

12 1. On Identity Verification Screen, click Next

13 2. Check the box next to I agree to terms and conditions and click Next You must agree to the Terms and Conditions to proceed

14 3. Complete Your Information. All fields marked with an asterisk must be completed

15 4. Click Next

16 5. On Verify Identity page, answer the questions and click Next

17 6. After verification of successful completion, click Next

18 What if I fail my Identity Proofing? Contact the QualityNet Help Desk to provide you with an EIDM Manual Identity Form (Paper process to verify your identity) Qnetsupport-esrd@hcqis.org or

19 Complete Multi-Factor Authentication Information After completing the Experian Identity Proofing, user will be taken to a page to set up Multi-Factor Authentication (MFA). Additional security code sent when logging in to EIDM or QARM Makes your login more secure beyond user name and password Associate the Security Code to your profile by registering your phone, computer or 4 Methods to receive MFA code: 1. Install validation software on phone/computer (Symantec VIP) 2. Receive via Text Message 3. Receive via 4. Receive a Phone call Can register multiple MFA Device Types (recommended) More details on the types can be found under Setting Up Multi-Factor Instructions. The next steps walk you through if you want to continue MFA setup at this time.

20 1. Click Next on Multi-Factor Authentication Screen

21 2. Select your preferred MFA Device Type, enter the requested information and click Next

22 3.Click Next on page confirming successful registration of your MFA You will also receive a confirmation .

23 4. Enter your facility information on the Request New Application Access page and click Next If you cover multiple facilities, enter information for your primary facility

24 Request New Application Access (again) 5. Add I need access to [insert system name(s)] in Reason for Request and click Next

25 Confirmation of request displays 6. Click OK Make a note of your tracking number for correspondence with the QualityNet Help Desk on any issues creating your account

26 Verifying your EIDM account setup is complete 1. Log into EIDM at 2. Enter your User ID, password, and multifactor authentication code 3. Select Request Access Now on the right side of the screen 4. Look for QARM under My Access

Data source for some QIP measures and facility information for external sources.
