1 EIDM and QARM Registration Required to access CROWNWeb, the QIP system and EQRS

2 Overview All users must create a user account through EIDM and sign up to use QARM to access CROWNWeb, the QIP system and EQRS EIDM (Enterprise Identity Management System): Create User ID and Password QARM (QualityNet Authorization & Role Management): Apply for system access

3 EIDM/QARM Registration This guide will walk you through: Part 1: Creating your EIDM account Setting up User Name and Password Setting up Challenge Questions (to unlock account, etc.) Part 2: Apply for access to QARM (Application used to access CROWNWeb, QIP and EQRS) Locating and adding QARM as an application Verifying your Identity using Experian credit report Identifying a second authentication method (MFA) for login

4 Part 1: Create an EIDM Account 1. Go to Part 1: Create EIDM Account Slide 4

6 3. Choose QARM: QualityNet Authorization & Role Management For access to CROWNWeb, QIP, and other applications in the EQRS Secure Portal, select QARM: QualityNet Authorization and Role Management. After selecting the application, check the box next to I agree to the terms and conditions and click Next: Part 1: Create EIDM Account Slide 6

7 4. Complete required information on Register Your Information screen NOTE: Address provided will be used for communications regarding your account. You must use a distinct . (Do NOT use a shared such as clinic12345@clinic.org) Part 1: Create EIDM Account Slide 7

8 5. Create User ID, Password & Challenge Questions Make sure you know the answers/format of challenge questions for future reference: Did you use a dash - or a forward slash? Did you use a space or no space? Where did you use upper or lower case in your answer? NOTE: If a user ID is already in use, you will get a message stating that and will need to create a different one. Never use a shared as your User ID. Part 1: Create EIDM Account Slide 8

9 Password Requirements Password must be minimum of 8 and maximum of 20 characters Must contain 1 letter and 1 number Must contain 1 upper case and 1 lower case letter Must contain special character (cannot use? < > ( ) / \ & Cannot contain your user ID Must be different from previous passwords used Must be changed every 60 days Part 1: Create EIDM Account Slide 9

10 6. Review your Registration Summary information and click Submit User Part 1: Create EIDM Account Slide 10

11 7. Respond to the Confirmation Screen When the Confirmation screen appears click on the x in the upper right to close it. Your user account has been created and you can now log in and request access to QARM. END PART 1 CREATE YOUR EIDM REGISTRATION Part 1: Create EIDM Account Slide 11

12 Part 2: Request Access to QARM Once you complete the EIDM registration process, you will need to request access to QARM by taking the following steps: Part 2: Create QARM Account Slide 12

13 1. Log into the CMS Secure Portal Enter your EIDM UserID and Password Check Agree to our Terms & Conditions Click Login Part 2: Create QARM Account Slide 13

14 2. Click Request/Add Apps You are logged in if your name is in the top right-hand corner Part 2: Create QARM Account Slide 14

15 3. Search for QualityNet Authorization and Role Management (QARM) from the catalog and click Request Access TIP: Type q in the Access Catalog to narrow the search If you don t see QARM listed, check your existing access. You may have already applied Part 2: Create QARM Account Slide 15

16 4. From the Request New Application Access screen, select QualityNet User as your role and click Next TIP: If your page looks blank, scroll up Part 2: Create QARM Account Slide 16

17 5. On the Identity Verification screen, read the information and click Next Part 2: Create QARM Account Slide 17

18 6. Check the box I agree to terms and conditions and click Next Part 2: Create QARM Account Slide 18

19 7. On Your Information screen, complete all required fields and click Next Part 2: Create QARM Account Slide 19

20 8. On Verify Identity screen, review and answer the questions and click Next Part 2: Create QARM Account Slide 20

21 9. On screen showing successful completion of Remote Identity Proofing click Next Part 2: Create QARM Account Slide 21

22 10. On the Multi-Factor Authentication (MFA) Information page, click Next Part 2: Create QARM Account Slide 22

23 11. Choose your MFA method and click Next There can be a delay if you select as your method. Part 2: Create QARM Account Slide 23

24 12. After confirmation of successful MFA registration click Next Part 2: Create QARM Account Slide 24

25 13. Enter your facility information and click Next If you cover multiple facilities, enter your primary facility.. Part 2: Create QARM Account Slide 25
