2 Lead Data Monitor: This checklist serves as a guide to tasks that need to be completed each day. The order in which some tasks occur may vary by facility. The letter in the Task ID column corresponds with a more detailed narrative description in the STRIVE Resource Manual. The Assigned to column is used to delegate tasks and identify the individual Data Monitor to whom the task may be assigned. The check mark ( ) column tracks the completed tasks. Please return this completed form in the Accordion Folder package at the end of the week. M O N D A Y A. Meet nursing home contact person and pick up Lead Data Monitor accordion folder package B. Meet key staff; make introductions, determine which facility staff member will provide daily updated resident rosters and staff schedules. C. Take site tour. D. Organize workspace; set up equipment. E. Check laptops and PDAs. F. Import new master database to laptop. G. Check facility profile. H. Collect updated resident and staff rosters from facility s designated contact and enter into laptops; review payer source changes. Complete the laptop update tracking form. Download updated information to PDAs and place on chargers. Complete the PDA update tracking form. J. Train therapy staff on PDAs as they have time; complete PDA Sign Out/Sign In Log, start practice data collection. Train therapy manager on paper tool. N. Collect next day (or rest of week) staff schedules from the facility s designated contact and write them on the PDA Sign Out/Sign In Logs L.a. Meet with facility administration to deliver Culture Change, IT survey and business office information. M.a. Monitor therapy staff practice data collection. Resolve gaps and other time issues. R. Instruct therapy department to call and then fax paper tools to IFMC on Tuesday. H. Collect updated resident and staff rosters from facility s designated contact and enter into laptops. Complete the laptop update tracking form. U. Issue PDAs to day shift nursing staff; complete PDA Sign Out/Sign In Log; continue live data V. Issue PDAs to non-therapy ancillary staff; complete PDA Sign Out/Sign In Log, continue live data L.b. Collect PDAs from day shift nursing staff; therapy and non-therapy ancillary staff; upload data to laptops; resolve gaps and other time issues; complete PDA Sign Out/Sign In Log, staff completes evaluations, charge PDAs. P. Clean checked-in PDAs, re-ink numbers (due to fading), and place on chargers. Reminder: Complete the front page of this checklist. Verify that all forms are completely filled out before sending to IFMC. X. X.a.. Create final back up CD. Initial and date backup CD. Prepare accordion folder for shipping to IFMC. Call STRIVE for UPS pickup. Y. Clear all time data from PDAs. Use the PDA Update Tracking form to check off PDAs as they are cleared. Insert this form into the PDA case before shipping. Z.
