1 TENDER NO TENDER FOR COMPUTER LEASING 1. Introduction REQUIREMENT SPECIFICATIONS 1.1 The National Kidney Foundation (NKF) would like to implement Computer Leasing all NKF end-user computing devices to avoid technology obsolescence. These devices include desktops, notebooks and tablets. 2. Quantity Requirement and Leasing Period 2.1 NKF is looking at a projected units of 510 with 36-months lease tenure for all end-user computing devices (EUCDs) leased under this agreement. This number is based on tech refresh of existing assets, excluding new deployments. 3. Qualification 3.1 The Vendor must provide one dedicated Delivery Manager who will oversee all the service deliverables throughout the contractual period with a strong background in operations and risk management together with a team of desktop deployment specialists (whenever necessary) to be successful. 4. Statement of Work 4.1 The Scope of this tender includes: The supply, delivery, installation, data migration, testing, and commissioning of enduser computing devices (EUCDs) End-to-end project management, including requirement-gap analysis, documentation, resource management, user acceptance testing, etc Asset management of all deployed assets and maintain updated and accurate inventory list of all deployed assets Monthly report and meeting on new deployment/ tech refresh/ inventory status. 4.2 The key deliverables of this tender are the smooth deployment and tech refresh of NKF EUCDs, minimising users disruptions in the process. Proper asset management is crucial to be successful. 4.3 This services covers all of NKF HQ and dialysis centres (total 36 by June 2018) islandwide. Refer to Annex B1-B2 for locations of NKF HQ and dialysis centres. 5. Business Requirement 5.1 NKF will provide monthly forecasted tech refresh schedule 3 months in advance to vendor. This excludes new deployment requests. NKF will submit order to vendor at least 1 calendar week prior to deployment. Vendor will prepare machine and deploy within the stipulated time frame (1 calendar week). 5.2 Vendor to provide a replacement/ buyover price schedule for total replacement in the event of accidental damage (beyond repair) or lost of equipment (refer to Annex A1-A7). 5.3 Vendor to provide a financial remuneration cost breakdown (based on the cost at the end of contractual period) for all the items (refer to Annex A1-A7) deployed under this service. This price schedule will be used for financial remuneration for all the missing/ broken items returned at the end of the contractual period. 1

2 5.4 Prices quoted in the proposal is for leasing of these 510 units. Vendor to cater for a pricing review upon fulfilment of these 510 units. The pricing will be subjected to mutual agreement between vendor and NKF in writing. NKF reserves the right to continue or terminate this agreement upon completion of the commited 510 units with a notice period of 60 days in writing. 6. Hardware Requirement 6.1 EUCDs is projected to be deployed over a span of 3 years. Projected number of units is for tech refresh of existing assets (excluding new deployments) Refer to Annex A1-A7 for hardware models and specifications of the EUCDs required. 6.2 Provisioning of Defective Media Retention (DMR) and Hard Disk Drive retention upon end of contractual period. With this requirement in place, data storage media should not leave NKF premises at all times. 7. Service Requirement 7.1 Provision of installation services, user settings and data migration (estimated not more than 50Gb with majority less than 10Gb). 7.2 Provision of de-installation and disposal services for deployed assets at the end of contractual coverage. This includes data storage (HDD/ SSD) removal service and handover to assigned NKF IT personnel with documented handover process. 7.3 Provision of asset management and asset tagging for all assets deployed. 7.4 NKF-built hard disk images for all EUCDs will be provided to the vendor. These images will be updated and provided to vendor on a half-yearly basis (if necessary). Provision of image load services for all EUCDs prior to deployment. 7.5 Provision of one dedicated Delivery Manager who will oversee all the service deliverables throughout the contractual period. 7.6 Vendor to provide contract renewal agreement 6 calendar months prior to end of contractual period. 7.7 Provision of monthly report and meeting on new deployment and tech refresh. 7.8 Provision of inventory status report to facilitate accounting and monthly payment. 8. Hardware Support Service Level 8.1 Provision of hardware support hours is 9 x 5.5 coverage. 8.2 Provision of onsite response is next business day. 8.3 Provision of support related to deployment issues is 1 calendar week from date of user acceptance. 2

Vendor Proposal 9.1 Each Tenderer should provide the price quote in the Price Schedule Part 1 to Part Under Price Schedule, Part 1, the Tenderers should provide the prices for all EUCDs listed in Annex A based on 36-month leasing with next business day onsite warranty for the entire lease period. 9.3 Under Price Schedule, Part 2, the Tenderers are required to provide: The financial remuneration cost breakdown (based on the cost at the end of contractual period) for all the items deployed under this service. This will be used for financial remuneration for all the missing/ broken items returned at the end of the contractual period Replacement/buyover price schedule for total replacement in the event of accidental damage (beyond repair) or lost of equipment. 3
