2 Link to the Portal The web portal provides certificate owners with access to their certificate information once the certificate(s) have been issued. To access the portal click on the following URL: or accessing through home page, clicking on Solutions at the bottom of the page and then under Whole Life in Protection Solutions click Policy Log In. For questions around access to the portal or policy information call (844) (1-844-WRKPLACE) and Press 1 for customer service..

3 Registration Process In order to access the portal, you will need to register. 1 Step Action 1. Click Register link located in the top right of the page or under User ID..

4 Registration Process Continued Enter Personal Information following steps below. 1 Step Action 1. Certificate owners must first select Individual & Member under User Type in the dropdown box. Enter Social Security # 4 2 If you encounter an error, please contact customer service 3 2. Complete Required fields (*): First Name, Last Name, Date of Birth and Government Identifier (SSN). 3. Enter your Certificate Number. 4. Click Next. Portal will validate registration data against admin system before proceeding to next step.

5 Registration Process Continued Once validation is completed in previous step you will be required to complete all required fields (*) on this page. Step Action 1. Create User ID, Password and all security questions. 2. Click Next to move to the Preference Information Page. 2 1

6 Registration Process Continued Once Registration is complete and back at the portal Log In page, log in to the portal following the steps below. Step Action 1 1. No action is needed in Preferred Language or Preferred Date Format drop boxes. Click Submit. 2. Once completed with registration, click Log In to access portal. 2

7 Log In Page Once Log In steps are completed you may access the portal 1 Step Action 1. Enter User ID. 2. Click Next. 3. Enter Password. 4. 8 My Insurance Page This page will display all the certificates owned by product A dropdown is available for each certificate to access additional information or edit personal information. View Personal Information Page View Certificate Details Page View Billing/Payments Page Basic certificate information is displayed on this page including coverage amount, coverage effective date and premium.

9 Update Personal Information If you select Update Personal Information, policy owners will have ability to update their address, telephone number and address Asterisk (*) indicates required fields. Step Action 1. Enter new address, phone number or address. 2. Click Save. 1 st address line is required. 10 digits are required for phone numbers. 5 digit zip code is required. 2 1

10 Certificate Detail Certificate detail page provides certificate status, payment detail, cash value and loan information You can update personal information by clicking this link. View Personal Information Page To view coverage details click product. name To view Insured information click this link.

11 Benefit Detail Benefit detail page provides certificate status, payment detail, cash value and loan information To view insured information or update contact information click this link.

12 Billing/Payment Detail Billing and payment detail page provides current billing information and displays payment historytion or update contact information click this link To view payor information or update contact information click this link. Can also view last billing statement produced.
