3 GENERAL INFORMATION PRODUCT DESCRIPTION diasend Transmitter is indicated for use by health care professionals in health care facilities to transfer predefined data from home monitoring devices to a server database. The device can be used as a stand-alone product or in combination with the Glooko System. SUPPORTED DEVICES To get started, you will need: 1. A compatible blood glucose meter, insulin pump, or continuous glucose monitor (CGM); 2. The diasend Transmitter; 3. A Glooko account with Population Tracker access; 4. The device upload cable specific to the diabetes device. WARNINGS The Glooko device system for Glooko Application does not measure, interpret, or make decisions on the data it conveys nor is it intended to provide automated treatment decisions or be used as a substitute for professional judgment. 8 COMPATIBLE DEVICES INSULIN PUMPS ANIMAS OneTouch Ping (pump) Vibe Vibe(Plus) INSULET Special Upload Instructions (for Animas pumps above) 1.Suspend the pump.(main MENU, scroll to Suspnd/Resum and press OK; Suspend is highlighted). 2. Press OK. 3. Place the pump with its back towards the IR eye on the Transmitter. 4. When the transmission is finished: Resume pump (MAIN MENU, scroll to Suspnd/Resum and press OK; Resume is highlighted). Press OK. Omnipod System No cable is needed. The Animas pump works with the Infrared Window on the diasend Transmitter. Mini USB cable TANDEM t:flex t:slim t:slim X2 t:slim G4 CONTINUOUS GLUCOSE MONITORS (CGMS) DEXCOM G4 G4 with Share G5 Special Upload instructions (for all Dexcom CGMs above): If the upload doesn t start, disconnect the receiver, select Shutdown in the menu to turn the receiver off. Turn the receiver on again and connect it to the transmitter. 6

9 GET STARTED INSTRUCTIONS FOR USE diasend Transmitter You will need a Glooko account with Population Tracker access. The data uploaded using the diasend Transmitter will synchronize with your Population Tracker account, which can be accessed with your Glooko professional account login information. Internet (cellular network) is necessary for the uploaded data to synchronize to the Glooko server. NEED HELP? 10 UPLOAD DATA UPLOAD DATA These instructions provide steps to upload data using the diasend Transmitter. If you have difficulty, please contact Glooko Support. PLUG diasend Transmitter INTO A POWER OUTLET Plug the diasend Transmitter into a power outlet and allow the Transmitter to boot up. The diasend Transmitter is ready to use when: Green ok indicator has stopped flashing. Blue send indicator is flashing slowly. NOTE: Slow flashing ok light means the transmitter is starting up. Rapid flashing ok light means the transmitter is updating. NOTE: NOTE: If you have a USB hub connected to the diasend Transmitter, it must be powered by its own power supply. CONNECT THE DIABETES DEVICE TO THE diasend Transmitter Connect patient device to the diasend Transmitter. Each patient device connects in one of the following three manners: USB cable (USB port or USB hub) Bluetooth (requires specific Bluetooth adapter) Infrared (built-in IR receiver or for some devices a SmartPix cable). View Device Compatibility to learn more about the device transmission method and view special device-specific upload instructions, if applicable. UPLOAD DATA After connecting the diabetes device to the diasend Transmitter: Connect the patient device - the blue send indicator will flash rapidly while uploading the data. Wait until the blue send indicator stops flashing rapidly and the green ok indicator is lit. The upload is complete when the green ok indicator is lit. 8

11 VIEW DIABETES DEVICE DATA VIEW DIABETES DEVICE DATA After uploading your diabetes device data using the diasend Transmitter, you can view the data at by logging into your Population Tracker and navigating to the Assign Devices tab. For more information about using the Population Tracker, please refer to the: Population Tracker Instructions for Use Guide or the Population Tracker Sponsored Edition Instructions for Use Guide. 9

12 TROUBLESHOOTING: TRANSMITTER LIGHTS UNDERSTAND THE TRANSMITTER LIGHTS The diasend Transmitter lights provide insight into the status of your diasend Transmitter. OK LIGHT (GREEN) SLOW FLASHING The ok indicator flashes slowly when the Transmitter is connecting to the GSM network. RAPID FLASHING The ok indicator flashes rapidly over a sustained period when the Transmitter is updating. You can start using the Transmitter once the send indicator flashes slowly again. STEADY LIGHT ON A steady ok indicator signals a successful upload. SEND LIGHT (BLUE) SLOW FLASHING The send indicator flashes slowly when the Transmitter is ready for use. RAPID FLASHING The send indicator flashing rapidly when data is being uploaded. ERROR LIGHT (RED) STEADY LIGHT ON A steady error indicator indicates that the latest upload was not successful. This is most likely due to temporary issues related to the wireless GSM network. Please try to upload the patient device again. RAPID FLASHING If the error indicator is flashing red, the Transmitter cannot find a network. 10

13 TROUBLESHOOTING: TRANSMITTER LIGHTS TRANSMITTER LIGHTS DURING SUCCESSFUL UPLOAD The following sequence of lights illustrates a successful upload using the diasend Transmitter: 1. Connect the diasend Transmitter to a power outlet The ok indicator will flash slowly for a few minutes when starting up The send indicator will flash slowly. The Transmitter is now in stand-by mode and is ready for use Connect a diabetes device to the Transmitter The send indicator will flash rapidly as the data is being uploaded If the upload was successful, the ok indicator will be a steady green light and the send indicator will start flashing slowly again. The Transmitter is now ready for another upload. 6 11
