1 Proposal for Services Security Camera Replacement Must be received by 10:00 a.m. on March 30, 2018 Services to be provided at Shiloh Towers th St. Zion, IL A Mandatory Pre-bid Meeting will be held on March at 11am Bidders are advised that attendance at the pre-bid meeting is strongly recommended. NO SPECIAL ARRANGEMENTS WILL BE MADE FOR THOSE NOT ATTENDING. Contractor proposal must be submitted using this LCHA provided Contractor Bid Sheet. Contractor may include additional documentation as they feel appropriate. LCHA is contracting for a system upgrade to existing CCTV system. Requested specification is listed below with sample gear types. The quote is to be this exact equipment or comparable equivalent. Also requested is a primary services contract for the maintenance of installed equipment as needed. All services and pricing must be listed. All existing equipment is to be carefully removed and returned to LCHA procurement staff or through separate proposal contractor may offer trade in value under this project. Any damaged areas during removal should be repaired to like new as necessary. The focus is to be based on the following major areas of enhancement to the existing infrastructure of the system using the following. The installation of upgraded interior and\or outdoor camera equipment with-in the housing development, including any needed cabling or wiring. The upgrade of recording equipment and security of the recording equipment in the office. The addition of cameras to the appropriate non-covered areas of the facility as listed. The ability of remote viewing capability and future expansion of the system. This project as proposed is to be a complete turn-key project fully contracted under the General Contractor

2 Outdoor Cameras Requested is also to add 6(six) outdoor cameras for door and parking area coverage. Exact locations will be discussed during the walkthrough. Exterior building mounts around the property may be utilized if applicable. The proposed service provider is to add new cameras with units equipped to handle viewing needs day and night. The requested replacement cameras are to be equal or greater than the following specification: 4(Four) Cameras: o 2.43 Megapixel HD 1080 Bullet Cameras. 2.8mm-12mm Fixed Lens, Color True Day / Night Matrix Infrared LED s, weatherproof, white metal housing, Heater. IP66 Outdoor Rated, Sony HD Chip Set 2(Two) Cameras: o 2.43 Megapixel HD 1080 Long Range Bullet Cameras. 2.8mm-12mm Varifocal Lens, Color True Day / Night Matrix Infrared LED s, weatherproof, white metal housing, Heater. IP66 Outdoor Rated, Sony HD Chip Set Interior Cameras Included in this proposal is the addition or replacement of cameras to the Lobby, Elevators, Community room, Maintenance shop and all common areas. The new cameras shall be equivalent 3MP HD High resolution with extended night vision, wide angle, have digital zoom and be in vandal resistant dome. As needed New wire will need to be run to each camera and interfaced to the Network Video Recorder. Power loss is to be protected through the UPS. Locations are listed for coverage areas, contractor proposal to detail quantity of equipment needed for full coverage of all areas. The cameras shall provide day/night functionality with wide dynamic range to give visibility of objects in both dark and light areas of a scene. Any licensing must be defined in the bid request. The equipment listed for this facility must allow for the expansion of the system at this location in the future, but also provides the capacity to integrate with future systems at other locations with-in Lake County. The ability to view the system off-site and the capacity to view multiple systems with the same client software is required. Alternate #1 Equipment must be tested for full functionality and training is to be provided to LCHA Management staff as designated. All areas of work must be cleaned. 5 Primary Services Contract Primary services contract for time and material for a service period May 1, 2018 April 2019 with an option of four (4) 1-year extensions. The service provided is optioned through May Quote to be based on hourly rate and include details for: 1. Hourly and material rates for all services anticipated to be used in the annual contract. 2. All holidays that will be billed at a higher rate and the hourly rate associated with the holiday. 3. 6 3. Term Negotiated calendar days to complete work after contract is signed as per contractor proposal. All work is to done during normal business hours of Monday- Friday 8:00am to 4pm. Pursuant to Section 15 of the Substance Abuse Prevention on Public Works Projects Act, 820 ILCS 265/1 et seq., a copy of the Contractor s written substance abuse prevention program must be submitted prior to the commencement of the Work If subcontractors are to be used they must be listed on the bid form. The general contractor or prime contractor must complete at least 12% of the project by himself or his employees. The job may not be subcontracted more than 88%. If subcontractors are used each subcontractor must submit proof to demonstrate the subcontract relationship (contract, incorporation papers, etc) and a Certificate of Insurance covering general liability and workers compensation for the subcontractor and employees. a.
