3 The Vitals360 is the FIRST professional grade mobile medical device with ALL the features listed below: 1. Measures Blood Pressure, ECG, Temperature, Blood Glucose, O2, Pulse; 2. 6 Diagnostic Overview Modes of Operation Single Test Mode WELCOME SINGLE TEST MODE POINT OF CARE MODE REMOTE PATIENT MONITORING This mimics standard medical devices. The Vital360 can be used without patient registration, as a convenient multidiagnostic device for spot checking by all healthcare providers. the device logs out after 5minutes of inactivity and goes back to the main menu screen. 6 Vitals360 User Manual
7 Point of Care Mode WELCOME SINGLE TEST MODE POINT OF CARE MODE REMOTE PATIENT MONITORING The highly efficient mobile point-of-care solution allows nurses to document patient data directly into the EHR (Electronic Health Record) from the Vitals360 mobile device in real-time. This mode can be used by a healthcare provider workforce caring for multiple patients. In this mode, the Vitals360 can replace a number of medical devices, making it ideal for care both inside or outside a facility. Other medical devices require manual entry of data, but the Vitals360 can be linked via a simple SDK (Software Development Kit) to any provider management tool or the information can be sent to the VoCare HIPAA-compliant cloud for review. Additionally, the Vitals360 has GPS location and time-stamping providing a superior audit trail for Medicare reimbursement. Vitals360 User Manual 7

9 Remote Patient Monitoring WELCOME SINGLE TEST MODE POINT OF CARE MODE REMOTE PATIENT MONITORING This mode is a daily monitoring tool for a single patient. The Vitals360 is simple enough for any patient to use on his or her own, remotely providing a picture of a patient s overall health. Patients no longer have to keep track of multiple devices, nor do they have to perform cumbersome manual logging of their vital information. This highly efficient Mobile Remote Patient Monitoring m(rpm) diagnostic device not only is easy to use, but the has the lowest cost of deployment. 11 4. Main Functions Blood Pressure Clip your device into the arm cuff. The blood pressure is easily influenced by the tester s body gestures, cuff wearing, physiologic conditionand the surrounding environment; therefore please DO make sure the middle of the cuff is placed at the same level of your heart. 12 Blood Pressure (cont) 1. Stay calm and rest at least 5 minutes before blood pressure measurement. Remove cloth from your arm or wear thin cloth to test. 4. Select the Blood Pressure icon in your device screen and then select Start to begin measurement. Keep calm while the device is processing. 2. Put the multifunctional health monitor to the cuff in a correct position, making sure that the bottom edge of the cuff is approximately 3/4 above the elbow, loose or tightenit as long as it s comfortable. 3. Select if whether you are testing your glucose before or afer a meal. 3. Insert the a test strip (sold separately) into the strip receiver (see page 5 to locate) 4. Wash your hands with soap and water before touching testing tools. 14 Vitals360 User Manual
15 5. Prepare the lancing device: Pull off the cap of the lancing device. Insert a lancet and push down until it s secured. Twist the protective disk of the lancet. Replace the cap by aligning the arrow with the release button. Vitals360 User Manual Select the depth of penetration by turning the adjustable tip. Pull the cocking control back until it clicks. You will see a color change in side the release button when it s ready. 15
16 Blood Glucose (cont) 6. Test your bood glucose Clean your finger. Hold the lancing device firmly against the finger and press the release button. 17 Blood Oxygen and Pulse The pulse oxymeter function measures SpO2 and pulse rate. SpO2 indicates the amount of oxygen being carried by the red blood cells of the blood. The rate of a person s SpO2 is a given percentage which is normally around 96%. 1. Select Blood Oxygen or Heart Rate 2. Open the O2 finger door Vitals360 User Manual 3. 19 Temperature 2. Maintain a dry and clean forehead; align the monitor probe on the middle of forehead and above eyebrows with the monitor in your hand about 3/4 from your forehead. 1. Select Temperature in the main menu. CAUTION: Do not use temperature probe directly on your forehead skin. Do not measure by a fan, air conditioner or under direct sunlight. Vitals360 User Manual 19

Once the process starts, the device will wait for 5 seconds while you hold the device in front of your forehead 4. Yes No CAUTION: To avoid external disturbance, please do the measurements in a quiet environment. DO NOT do the test while the Vitals360 is being charged. During test, both hands hould not touch each other. ECG measurements can get results of heart rate, HRV and breathing rate at the same time. 23 Weight The Vitals360 device can store weight data entered manually or record the result when connected to a wireless weight scale via Bluetooth Manual Entry 2. 27 5.2. Help The Help button (question mark) displays a instructional video with the user manual in visual format. INSTRUCTIONAL VIDEO 5.3. Reports The Reports button will display the data from the diagnostic readings (health records) for review or export. 31 6. Warranty In terms of the Vitals360, VoCare guarantees service as follow: 1. Product can be refunded, replaced or maintained free within 7 after purchase, under the usage of the manual of instructions. The device can be repaired for free within 1 year after purchase in the precondition of non-artificial damage. If any other reasonable requirements, VoCareprovides the repair service and technical services, but need to charge the cost accordingly. 2.
