3 Page No INTRODUCTION 1.1 Scope This report documents the test procedures followed and the results obtained during Vibration Resonance Search and Resonance Dwell Testing performed on a FH-Spring Return Actuator for Bettis Corporation. The FH-Spring Return Actuator was received at Wyle Laboratories on May 24, 2007, and subjected to an inspection upon receipt. The receiving inspection revealed the FH-Spring Return Actuator to be in good condition. Testing was performed at Wyle Laboratories Huntsville, Alabama, Test Facility from May 24 through June 14, References Bettis Corporation Purchase Order No Wyle Laboratories Quotation No. 542/ C1/DB, dated April 11, 2007 Wyle Laboratories Quality Assurance Program Manual, Revision 2 ANSI/NCSL Z540-1, Calibration Laboratories and Measuring and Test Equipment, General Requirements ISO , Quality Assurance Requirements for Measuring Equipment MIL-STD-45662A, Calibration System Requirements 1.3 Test Specimen Description The test specimen is one FH-Spring Return Actuator measuring approximately 91 inches by 26 inches by 26 inches in diameter. The weight of the Actuator is approximately 1,507 pounds. The Actuator is identified by Model No. FH5024-SRlCW and Serial No A Summary The Actuator was subjected to Vibration Resonance Search and Resonance Dwell Testing. These tests were performed in accordance with Wyle Quote No. 542/ C1/DB, dated April 11, Upon completion of all required testing, the Actuator was visually inspected for any signs of damage or degradation. No anomalies were noted. The Actuator was returned to Bettis Corporation for post-test inspection and evaluation. The test results contained herein apply only to the test specimen identified in this report.

4 Page No TEST PROCEDURES AND RESULTS 2.1 Vibration Resonance Search Test One FH-Spring Return Actuator was subjected to a Vibration Resonance Search Test in accordance with Wyle Quote No. 542/ C1/DB, dated April 11, Testing was performed in each of three orthogonal axes. All tests were completed in the first axis before continuing to the second axis. Likewise, all tests were completed in the second axis before continuing to the third axis. The Actuator was mounted on the vibration table for testing as shown in the photographs presented in Attachment A. The FH-Spring Return Actuator was subjected to a singleaxis sine sweep from 5 to 100 Hz, at a sweep rate of one octave per minute, in order to determine resonance prominences. During Resonance Search Testing in the Vertical Axis, response prominences were found that resulted in potentially damaging response of the Actuator and test equipment at initial acceleration levels. These input acceleration levels with such response prominences were modified. Wyle personnel obtain approval from Bettis Corporation Representatives to apply 35 psig of air pressure to stroke the piston inside the actuator to limit movement of the spring that could cause damaging responses during testing. The input acceleration levels omitted during the Resonance Search Test in the Vertical Axis fell between 5 g s and 0.5 g s. The Vibration Profile shown in Table I, below, was created by Wyle technical personnel and approved by Bettis Corporation Representatives to reflect these changes. At test completion, Wyle Laboratories personnel performed a visual inspection to verify satisfactory post-test condition of the Actuator. No damage or deformation was noted. No anomalies were reported. The Actuator successfully completed all requirements for the Resonance Search Test and testing progressed to the Resonance Dwell Test. 2.2 Vibration Resonance Dwell Test The Actuator was then subjected to a single-axis sine dwell at each response prominence determined during the Vibration Resonance Search Test. The total duration of the sine dwells in each axis was one hour. The Actuator was subjected to an additional 30-minute dwell in the lateral axis for a total of 60 minutes at 77 Hz. The test levels are listed in Table I, below. All test levels determined to be unsafe during the Resonance Search Test were omitted. Frequencies that showed the highest peaks were selected for this test. Table I. No damage or deformation was noted. No anomalies were reported. The Actuator successfully completed all requirements for the Resonance Dwell Test. Photographs of the Actuator and test setup are presented in Attachment A. The Resonance Search and Dwell Test Data are presented in Attachment B. The Instrumentation Equipment Sheet for the test setup is presented in Attachment C. 3.0 QUALITY ASSURANCE PROGRAM All work performed on this test program was completed in accordance with Wyle Laboratories Quality Assurance Program. The Wyle Laboratories,, Quality Management System is registered in compliance with the ISO-9001:2000 International Quality Standard. Registration has been completed by Quality Management Institute (QMI), a Division of Canadian Standards Association (CSA). Wyle Laboratories is accredited (Certificate No.: ) by the American Association for Laboratory Accreditation (A2LA) and the results shown in this test report have been determined in accordance with Wyle's scope of accreditation unless otherwise stated in this report. 4.0 TEST EQUIPMENT AND INSTRUMENTATION All instrumentation, measuring, and test equipment used in the performance of this test program were calibrated in accordance with Wyle Laboratories' Quality Assurance Program, which complies with the requirements of ANSI/NCSL Z540-1, ISO , and Military Specification MIL-STD-45662A. Standards used in performing all calibrations are traceable to the National Institute of Standards and Technology (NIST) by report number and date. When no national standards exist, the standards are traceable to international standards or the basis for calibration is otherwise documented.
