1 ACGME CPT PROGRAM How to Start the ACGME program The ACGME program will automatically start when you login. Do not look for an icon. The Login screen is similar to the picture below. If you do not see a login screen click on START, click on SHUTDOWN, Select Close all programs and login as a different user. To login, for Username, use the first initial of your first name and your full last name. For example, Tom DeMeester, MD will use TDeMeester. For the password, the first time you login leave the password field blank and press Enter. The System will give you a message that "the password has expired do you want to change your password?", answer yes and put in a new password that is 5 to 8 characters long. You can use letters and numbers for your password. The password is not case sensitive. Please remember your new password for future reference. After you enter your password the program will start automatically. Main Form

2 Step 1 (Resident Information) A) Please do not modify any settings in the following 6 menu options. Program and Institution Information Resident Information Attending Physicians Information Old Procedures by RRC Procedure Name Rotation Name Maintenance Specialty Name Maintenance B) You do need to go into the Resident information option and confirm that your individual info is correct. If it is not then please work through Peggy Robertson (323) on getting the corrections made, do not make the corrections yourself. Step 2 (Resident Rotation) This should be updated periodically as the resident progresses through the various rotations of the program Go into Resident Rotation selection under Data Entry Select your name from the drop down list by clicking on the down arrow. Then click on ADD button. Continue to fill out the rest of the fields (Rotation, No. Months, PGY Level, Institutions) Repeat this process for each of the rotations. Click on the left and right arrow to review all the rotations.

3 Step 3 (Program Information for Resident) The program information indicates full years of education in a resident s total training. Choose a resident name from the drop down list by clicking on the down arrow. Enter Year, Parent Program, From and To month/year. The Specialty field can be entered by clicking the ellipsis button (the button with the three dots located at the end of the field). Enter the information and then click on Save button. Repeat this process for each year.

4 Step 4 (Procedures by CPT Code) Only one resident should take credit as a surgeon for each operation. On same patient/same day/ same operation/ a senior resident takes credit as a surgeon, another resident may take credit as a First Assistant or a senior resident takes credit as a Teaching Assistant and a more junior resident takes credit as a surgeon. To add procedures by CPT code, click on the Add button (When this is done, the Close button changes to Cancel). Enter Resident Name, Credit Role, Attending Physician, Rotation, Institution by clicking on the down arrow and choose from the drop down list. Enter Patient Name and Patient ID. To enter the CPT code, first click in the CPT Code field then click on the ellipsis button (the button with the three dots) to display the CPT Procedure Lookup screen. Enter text for the CPT description or a number for the CPT code on the first line (use the asterisk as a wildcard character before and/or after you have typed text or numbers), and click on the appropriate search option (CPT code, CPT description, RRC procedure, or RRC code). Then click on the Search button. The CPT codes or CPT descriptions (depending on the selection) that match the text or number you have entered on the first line will be displayed in the Procedures grid. If there are no records matching the search criteria, the following message will be appear " No Matching Records Found", click on the OK button and re-enter new search criteria. Make a selection by either double clicking on an item in the Procedure grid or clicking once on an item in the Procedure grid and then click on the Select button. The item selected will now appear on the Residents procedures screen. Tab through each field to automatically display text. If you are entering more than one CPT code, press the Tab key to tab through each field on the current row until a new blank row is displayed. To select the CPT code for credit, double click on an item in the CPT Code list. The CPT code selected will now appear in the field "CPT code for credit. Click on Save to save the record. Repeat this process for each operation.

5 Step 5 (Report) Procedures by Resident This report will display/print a list of procedures performed by residents (on/after July1, 1997), the number of times the resident acted as Chief Surgeon, Junior Surgeon, Teaching Assistant and First Assistant for each procedure, and the number of times the resident acted as surgeon for each procedure (sum of Chief Surgeon and Junior Surgeon fields). This report is grouped by Resident Name. On Destination, click on Screen if you want to view from the computer screen or Printer if you want hard copy of the report. Click on Procedure by Resident. Go to Residents and choose name from the down arrow. Enter Procedure Date Range from and to date (If leave blank, it will print out from the beginning to the most present time). Click on Print to get the print out. Make sure you save your data and logout of Windows 95 when you are done.
