2 Section I HCA Powerscribe 360 Interfaces Operational Documentation Introduction Hospital Corporation of America (HCA) has contracted Austin Radiological Association in order to provide hosting and support of Nuance Powerscribe 360 system in order to support dictation and voice recognition of radiological reports. This document describes the system of HL7 interfaces built for the project. 2

3 Section II Title Operational Documentation Architectural Summary Physical Architecture Key Components HCA Meditech HCA used Meditech as the hospital information system. Meditech is the repository for all scheduling, patient, exam, and report data. Meditech is also the origin of all radiological orders. HCA Cloverleaf HL7 Engine HCA uses Cloverleaf to route and format all HL7 interface messages 3

4 ARA Corepoint HL7 Engine ARA uses Corepoint Integration Engine to format and route all HL7 interface messages. Currently Corepoint is hosted on NEOCORE. Powerscribe 360 (PS360) Nuance Powerscribe 360 is ARA s voice recognition and dictation system. ARA radiologist dictate and sign HCA radiological reports in PS360. PS360 s interfaces are hosted on XXXXXXX. Interface Support Administrator (ISA) ARA Users the internally developed ISA to support interface support operations and to hold configuration information. ISA is currently hosted on XXXXXXXX. Exam Repository ARA stores key exam and patient information for all exams that pass through ARA systems in the Exam Repository. The Exam Repository holds information that used by several ARA systems to support operations a decision making. The Exam Repository is currently hosted on XXXXXXX. 4

5 Message Summary The following are the primary steps in the message flow of the HCA Powerscribe 360 Interfaces 1. HCA sends an order message 2. HCA sends an order completion message 3. Powerscribe sends a preliminary or final result. 4. HCA sends a preliminary or final result 1. Independent of the other message flows HCA sends a demographic update message 5

14 HCA.IN.ExamRepository HCA.IN.ExamRepository is a Processing Action List adds the necessary metadata to control the processing of HCA messages for the Exam Repository. Then it sends a copy of the message to the Exam Repository connection. 14

15 HCA.Dictation.IN HCA.Dictation.IN is an entry action list that routes messages from PS360 to HCA through the HCA.Dictation.IN.HCA action list and to the Automated Billing System through the HCA.Dictation.IN.ABS action list. 15

16 HCA.Dictation.IN.HCA HCA.Dictation.IN.HCA is a processing action list that formats PS360 result messages and sends them to the HCA through the HCA.OUT connection. 16

17 HCA.Dictation.IN.ABS HCA.Dictation.IN.ABS is a processing action list that will add the necessary metadata for ABS processing and send a copy of the message to the Automated Billing System connection (ABS.Processing.SRC). Supplemental Flow This section is document calls to external systems (Database, Webservice, etc). The HCA Powerscribe 360 interfaces do not make any external calls. 17
