1 PrognoCIS The allows you to organize and associate documents to a patient chart. These may be digital images, consent forms, receipts, or any other document that has been faxed or scanned into a digital image. The applicable file extensions must be defined within the PrognoCIS Attach Type properties in order to be accepted into a patient chart. Note: Only Documents that have been manually attached to a chart using the may be detached when applicable. Other attachments displayed in the Document List (e.g.: Progress Notes, Letters Out) are automatically generated within the EMR chart and cannot be disassociated once they have been generated. See the Detaching Documents headings below. Select Folder / Select List: Pane The is comprised of folders that enable you to organize the incoming attachments such as scans, faxes, letters, or other documents. Scanned or faxed items will automatically transfer into the appropriate directory by means of an FTP utility. User-defined directories may be created in addition to the standard folders (i.e.: detach, move, fax, and scan) if applicable.

2 Attach To: Pane The Attach To: panel allows you to define the attachment so it is easily identifiable from the Document List on the patient chart. Patient: - allows you to search for a specific patient the attachment belongs to by clicking. Note: This will default to the current Patient Encounter if one has been selected prior to entering the. Attach: - allows you to categorize the Type in the Document List by clicking. Note: This can then be further classified by assigning an appropriate Category below. Order No.: - allows you to search for a specific Lab/Rad Order # if applicable by clicking. Note: This field is only applicable for Lab Result and Rad Result Attach Type. This label will be hidden for all other types selected above. Date: - allows you to select a calendar date for the attachment if applicable by clicking. Note: The current system date defaults if another one is not selected or specified. From: - allows you to identify the sender of the attachment by clicking. Note: The options available within this pick list are directly linked to the Attach Type. In addition, the specific Name: of the medic, insurance, vendor, etc. may be identified below. Name: - allows you to search for a specific person that corresponds to the From: by clicking. Note: This field will be hidden when the attachment is From: the patient.

3 Category: - allows you to classify the nature of the Attach Type within its category. Note: The options in this pick-list are user-defined and will vary within your database. Below are some common categories defined in conjunction with the six standard Attach Types (i.e.: Billing, Lab Results, Legal Documents, Letters In, Rad Results, and Other Documents. A 7 th standard Type (Progress Notes) does not have categories associated with it. Subject: - allows you to describe or define the specific contents of the attachment. Note: You can free-text in a description manually or build a library of common definitions that can be accessed by clicking. Message To: - allows you generate an alert to anyone within PrognoCIS by clicking.

4 Attaching an Image or Document to a Patient Chart 1. Select Patient Encounter. 2. Click and select the desired patient to open the Encounter screen. 3. Select Messages Attach. 4. Select the appropriate Select Folder: by clicking. 5. Select the desired attachment in the Select List: pane, which will both be highlighted in the list and also display ). Note: A preview of the selected document or image will display in the right pane of the window. 6. Define the Attach To: criteria based on local procedure. Note: See the above explanations for the elements within this pane for additional guidance. 7. Click the attach button. The document will be removed from the screen and be attached to the selected Patient chart under the Document List, which can be accessed from the Patient Encounter (Face Sheet) screen.

5 Detaching an Other Docs Document 1. Select the document from the Patient Encounter Document List. 2. Double-click the Other Docs attachment, which will open in its own Show Dialog window.

6 3. Click the Delete button inside the document preview window, which will prompt for confirmation of the deletion. Note: If the delete button is disabled, please verify the Maximum Days property is defined to allow the documents to be detached (for xxx number of days). 4. Click the yes button. Note: This actually moves the attachment from the selected patient chart into the PrognoCIS detach folder from which it can then be attached to the correct chart or deleted from the system.

7 Detaching A Lab Results or Radiology Results Document 1. Select the document from the Patient Encounter Document List. Training Quick Steps 2. Double-click the Results attachment, which will open in the CPOE Results window.

8 3. Click the Detach button inside the document preview window, which will prompt for confirmation of the deletion. 4. Click the yes button. Note: This actually moves the result will be detached from the selected patient chart; however, it still remains within the PrognoCIS system as a stored Result. It can then be associated correctly.

9 Detaching A Legal Docs Document Training Quick Steps 1. Locate the Legal Docs document from the Patient Encounter Document List. Note: Do not select it from the Document List. Just note that Category and Description for the document that must be detached. (Notice in this example there are 2 Legal Docs, but one is a Consent Form and the other is an Advanced Directive). 2. Go to the Patient Register. 3. Click the legal button, which will display all the current Legal Documents associated with the patient.

10 4. Select the in the Del column beside the document that must be detached. 5. Click the ok button. There is no confirmation prompt; however, the document will be removed from the chart. Note: This actually deletes the document entirely from the selected patient chart and from PrognoCIS. If the document is valid (though for another patient), it should be rescanned and then attached to the correct patient.
