4 Introduction This user guide describes how EvaluationWeb users in the jurisdiction of Colorado enter CTR, Outreach, HCPI, Individual-level and Group-level, and CRCS intervention data into EvaluationWeb. Who enters data? Health department staff with administrator or non-administrator EvaluationWeb accounts may enter data. When is data entered? You can enter your data at any time. However, Luther Consulting strongly encourages you to enter data on an ongoing basis as you collect your data to avoid having to enter large amounts of data right before a deadline. When do completed data forms lock? CTR data which have been submitted to the EvaluationWeb database can be modified for 100 days after the Session Date that you enter in the Agency section of the CTR data-entry window. After that point, however, the data are locked and can no longer be modified. To request that CTR data for a particular form be unlocked, you may send a request to the Luther Consulting Support Services help desk. (See Getting Help for contact information.) However, be aware that the form will relock overnight after it has been unlocked, so modifications must be made quickly. Data for non-ctr interventions (Individual-level, Group-level, Recruitment Outreach, CRCS, HCPI, Material Distribution) do not lock. Entering Data into EvaluationWeb Page 4 of 80

10 CTR Overview Entering CTR Data CTR Overview CTR data is entered into the online CTR form and the data in the form are then saved to the EvaluationWeb database. The form is divided into several sections. The following table briefly describes each section. Note: The data you enter into a CTR form locks and can no longer be modified 100 days after the Session Date (seen in the Tip above image). 18 Notes: If you select Declined Testing or Test not offered for Test Election, the subsequent items in the HIV Test Information section no longer display. If you select No for Result Provided, another item displays: If results not provided, why? 11. Select Yes for Show HIV Test 2 to expand the HIV Test 2 area. 19 13. Select Yes for Show HIV Test 3 to expand the HIV Test 3 area. If you do not have a third HIV test in the test event for this client, skip to Step Complete the HIV Test 3 area the same as the HIV Test 1 and HIV Test 2 areas. 15. 22 Note: If you select No for either item in this section, the Outcome items will change to Reason not items. 17. Click the Submit Form button at the bottom of the window. Result: A Data Submitted dialog box appears. 18. Click OK in the dialog box to close the box. Result: The data you entered in the CTR form is saved to the database. 23 Entering Referral Data Entering Referral Information Perform the steps in the table below to enter CTR referral information into EvaluationWeb. Step Action 1. Log in to EvaluationWeb. (See Logging in to EvaluationWeb for instructions.) 2. Click the Data Entry button on the left side of the page. 3. Click Enter/Edit Client Information. 4. Type a Form ID into the box. 26 8. Select a response for Was client referred to PCRS/PS? Notes: If you select Yes for this item, another item will appear: If yes, was client interviewed for Partner Services? Select a response for Was client referred to HIV Prevention services? Note: If you select Yes, another item will appear: If yes, did client receive HIV Prevention Services? 27 10. Select a response for the last item in the window. 11. Click the Submit Form button at the bottom of the window. Result: A Data Submitted dialog box opens. 12. Click OK in the dialog box. 28 Viewing and Editing CTR Data Viewing Data Perform the steps in the table below to view previously entered CTR data Step Action 1. Log in to EvaluationWeb. (See Logging in to EvaluationWeb for instructions.) 2. Click the Data Entry button on the left side of the page. 3. Click Enter/Edit Client Information. 4. Type a Form ID into the box. 30 9. Change responses to any of the items under the Test Information tab. 10. Click the Submit Form button at the bottom of the window. Result: A Data Submitted dialog box opens. 11. 31 Entering Data for an Outreach Intervention Creating a New Outreach Session New Session Perform the steps in the table below to enter your data into a new Outreach session in EvaluationWeb. Step Action 1. Log in to EvaluationWeb. (See Logging in to EvaluationWeb for instructions.) Reminder: Choose an Outreach intervention during the login process, or use the Intervention button on the main page to switch to an Outreach intervention. 2. Click the Data Entry button on the left side of the main page. 3. Click Enter/Edit Aggregate Information. Result: The outreach intervention data-entry window opens, with the Manage Session tab active. Entering Data into EvaluationWeb Page 31 of 80

32 4. Do one of the following: To start a session from scratch i. Click the New Blank Outreach Session button in the Create New Outreach Session area. To start a session using default responses i. Click the Default Set arrow and select a default set from the dropdown list. Result: The Session Information tab becomes active. ii. (Optional) Click the View Default Set button to view default responses for the set selected in the previous step. ii. Continue to Step 5. Result: The default responses display in a new window. After viewing the responses, close the window. iii. Click the New Outreach Session Based on Default Set button. Entering Data into EvaluationWeb Page 32 of 80

33 Result: The Session Information tab becomes active, with default responses already selected. iv. Continue to Step Type or select responses for all items in the window to enter or change information. Entering Data into EvaluationWeb Page 33 of 80

34 6. Click the Submit Form button at the bottom of the window. Result: A Data Submitted box opens. 7. Click OK in the dialog box. Result: The dialog box closes and the Client Information tab becomes active. 8. Type Counts for each item in the Client Primary Risk pleat. Entering Data into EvaluationWeb Page 34 of 80

35 Notes: A count total is displayed at the bottom of the each pleat under the Client Information tab. The count total must be the same as the Total Number of Contacts you entered under the Session Information tab. This is true for all pleats under the Client Information tab. 9. Click the Client Gender pleat to move it into view. Entering Data into EvaluationWeb Page 35 of 80

36 10. Type Counts for each item in the Client Gender pleat, making sure the count total is the same as the Total Number of Contacts you entered previously. 11. Click the remaining pleats one by one, and type counts into each one: Client Ethnicity Client Race Client Age 12. Click the Submit button at the bottom of the window. 37 13. Click OK in the dialog box. Result: The dialog box closes and the Materials tab becomes active. Note: In order for materials to be displayed under the Materials tab, a user with EvaluationWeb administrator privileges must first log in to EvaluationWeb and select allowed materials for the intervention in question. If this is not done, you will not be able to enter numbers under the materials tab. 14. Type responses for each item under the Materials tab. 15. Click the Submit Form button at the bottom of the window. Result: A Data Submitted box opens. 16. Click OK in the dialog box. Result: The dialog box closes and the data you entered are saved to the EvaluationWeb database. 17. 39 Viewing and Editing Data for an Outreach Intervention Viewing & Editing an Outreach Session Perform the steps in the table below to view and edit Recruitment Outreach data you previously saved to the EvaluationWeb database. Step Action 1. Log in to EvaluationWeb. (See Logging in to EvaluationWeb for instructions.) Reminder: Choose an outreach intervention during the login process, or use the Intervention button on the main EvaluationWeb page to switch to an outreach intervention. 2. Click the Data Entry button on the left side of the main page. 3. Click Enter/Edit Aggregate Information. Result: The outreach intervention data entry window opens, with the Manage Session tab active. Entering Data into EvaluationWeb Page 39 of 80

40 4. Click the Edit Existing Session arrow in the Edit/View Existing Outreach Session area, and then select an item in the dropdown list. Tip: You can limit the number of sessions appearing in the dropdown list by entering a Start Date and End Date and then clicking the Limit List button in the Edit/View Existing Outreach Session area. 5. Click the Edit Outreach Session button. Result: The Session information tab becomes active and displays the information for the session selected in the previous step. 41 6. Make any desired changes in the window, and then click the Submit Form button at the bottom of the window. Result: A Data Submitted dialog box opens. The Client Information tab becomes active. 7. Click OK in the dialog box. Result: The box closes and the Client Information tab becomes active. 8. Click each pleat under the Client Information tab one by one to view or modify data. Reminder: The count total in each pleat must be the same as the Total Number of Contacts entered under the Session Information tab. 9. Click the Submit button at the bottom of the window. Result: A Data Submitted box opens. 10. Click OK in the dialog box. Result: The dialog box closes and the Materials tab becomes active. Entering Data into EvaluationWeb Page 41 of 80

42 11. Modify data under the Materials tab as needed, and then click the Submit Form button at the bottom of the window. Result: A Data Submitted box opens. 12. Click OK in the dialog box. 43 Entering & Editing HE/RR and CRCS Intervention Data HE/RR and CRCS Data Individual- Level Data Group- Level Data HE/RR Interventions include data collected at the Individual-Level and the Group-Level. Data entry windows for CRCS, Individual-Level HE/RR, and Group-Level HE/RR are very similar so data entry for these three intervention types is addressed in the table below. See notes regarding any data-entry variances between Individual and Group Level. Step Action 1. Log in to EvaluationWeb. (See Logging in to EvaluationWeb for instructions.) Reminder: Choose an HE/RR individual-level or group-level intervention during the login process, or use the Intervention button on the main page to this switch to this type of intervention. 2. Click the Data Entry button on the left side of the main page. 3. Click Enter/Edit Client Information. Note: If you are working in the Group-Level Intervention, there will be additional options in this drop-down menu: Entering Data into EvaluationWeb Page 43 of 80

44 Result: The client enrollment data-entry window opens, with the Client ID tab active. Note: Information specific to how to fill out data entry fields for Enter/Edit Group Contact and Manage Cohorts, will be covered in the following section, entitled Enter/Edit Group Contact and Manage Cohorts. 4. Do one of the following: Enroll a new client in the system: Type the New Client ID in the box Click Enter New Client ID Edit an existing client: Select a Client ID from the drop down menu Click Submit under the Work with Existing Client tab Entering Data into EvaluationWeb Page 44 of 80

45 Note: If you know the Client ID you want to work with, you can search for it by typing it in the Limit Client List field. (Client IDs are case sensitive.) Result: The Demographics tab becomes active. Result: The Summary tab is displayed and you may edit any existing client data. Notes: If there has been no data entered for the Client Risks tab, you will be directed to that tab. If you are working in the Group-Level Intervention, there is an option to choose a corresponding Cohort; which, when selected, will limit the Client list to clients associated with that Cohort: 5. 47 6. Click the Submit button at the bottom of the window. Tip Click the calendar icon to open a calendar from which you can select data. Result: A Data Submitted box opens. 7. Click OK in the dialog box. Result: The dialog box closes and, in addition to the Client ID and Demographics tabs, the Client Risks, Hybrid Population Pre-Test Survey, Client Contacts and View All tabs become active. Note: If you are entering data for the CRCS or Group-Level Intervention, the Hybrid Population Pre-Test Survey and Hybrid Population Post-Test Survey tabs will be replaced with GLI/IDU Pre-Test Survey and GLI/IDU Post-Test Survey data entry tabs. 8. Under the Client Risks tab, select the appropriate response from the drop down menu for Self Reported HIV Testing history Note: If you choose Yes, tested before, an additional item displays: Self-reported Entering Data into EvaluationWeb Page 47 of 80

48 most recent HIV test result. Select the appropriate response from the drop-down menu. 9. Select a response for Has client ever been incarcerated? Note: If you select Yes, an additional item will appear: Incarcerated in jail or prison in the past 12 months? Select the appropriate response from the drop-down menu. 10. 51 14. Click OK in the dialog box. Result: The dialog box closes and the Summary and Client Contacts tabs become active. 15. Do one of the following: To edit an existing contact: Click the edit icon to the right of the contact s information under the Summary tab. To enter a new client contact: Click the Add New Contact under the Summary tab. Result: The Client Contact tab becomes active with the Session Information field. 52 16. Edit or enter new Session Information for that client. Notes: If you select Yes for Incentive Provided, an additional item appears Specify Incentive: If you select Yes for Was Client Pre-Screened with the Substance Use and Mental Health Prescreen tool in the last 12 months or at Intake, then additional items appear in the Risk Information Section: Entering Data into EvaluationWeb Page 52 of 80

53 If you select Yes, client was prescreened for either Was the Client prescreened for Substance Use using the pre-screen tool? Or Was the Client pre-screened for Mental Health using the pre-screen tool?, then the list of Reasons changes to Outcomes: 17. Click the Submit button at the bottom of the window. 54 18.. Click OK in the dialog box. Result: The dialog box closes and the Referrals tab becomes active 19. To add referrals to the selected Client ID, click Add Referrals 20. Result: Referral options populate under the Referrals tab. 21. Enter the Referral Date and select the appropriate Referrals. 22. Click Add Referrals. Entering Data into EvaluationWeb Page 54 of 80

55 Result: A Data Submitted box opens. 23. Click OK in the dialogue box. Result: The dialog box closes and the Referrals tab populates with the previously selected referrals, with a Pending status under Referral Outcome. 24. Do one of the following: I. To update an existing referral, click Update next to the Type of Service you want to edit: Result: A screen pops-up with data entry fields to provide further information on the selected Service Type: Entering Data into EvaluationWeb Page 55 of 80

57 Click the Submit button at the bottom of the window. Result: A Data Submitted box opens. Click OK in the dialogue box. Result: The Referrals tab is active and the Referral Outcome is updated to reflect the changes made. Note: If you select the Delete Referral checkbox, the service referral will be deleted from the list of referrals. II. If you are working in Individual-Level Intervention, click the Hybrid Population Pre- Test Survey, If you are working in CRCS or Group-Level Interventions, click the GLI/IDU Pre-Test Survey tab to enter survey responses for this section: Notes for Individual-Level Intervention: If you choose No Risk or Small Risk for Thinking about what you know about HIV and your own behaviors, do you think you have no risk, a small risk, a moderate risk, or a great risk of getting HIV?, the following additional questions appear: Entering Data into EvaluationWeb Page 58 of 80

63 Entering & Editing HE/RR and CRCS Intervention Data Data Entry for Group Contacts and Managing Cohorts Group-Level Data The following section addresses data entry specific to Group-Level Data. Follow the steps below to enter data pertaining to Group Contacts and Cohort Management. 1. Log in to EvaluationWeb. (See Logging in to EvaluationWeb for instructions.) Reminder: Choose the HE/RR group-level intervention during the login process, or use the Intervention button on the main page to this switch to this type of intervention. 2. Click Enter/Edit Group Contact: Result: A screen pops-up with the Cohort tab active to enter data regarding the related co 3.
