5 Changes to PS Data Reporting Requirements As of September 16, 2016, PS grantees are no longer required to report Client Summary Risk Reduction Activities (RRA) data through EvaluationWeb. Due to this change, you can no longer add or edit any client summary records. However, all previously submitted data for RRA programs are still available through Reflexx for running reports and exporting. 6 Accessing the PS HIV Testing Data Entry Form Accessing the Data Entry Form Follow the steps below to access the PS HIV Testing data entry form in EvaluationWeb. Step 1. Log in to EvaluationWeb.com with your user name and passphrase. Note: You must select a PS HIV Testing program in the Choose a program window during the login process. (The program may not necessarily have in its name.) Result: The Select Intervention window opens. (Depending on how your Program is set up in EvaluationWeb, you may not see this window. 9 HIV Testing Data Entry and Management Entering a New PS HIV Testing Record Entering a PS HIV Testing record Follow the steps below to enter a new PS HIV Testing data record into EvaluationWeb. Step 1. Type a Form ID into the ID section of the data form. Note: For some users, EvaluationWeb may auto-populate the Form ID. 2. Type or select responses for the items in the Agency section. 13 Step Notes: If you select Test not offered or Declined Testing for Test Election, several of the items in the section will no longer display. You do not need to make selections for these items. If you select No for Result Provided, another item will display: If results not provided, why?. Select a response for this item. If you select Yes for Show HIV Test 2, a new section for a second HIV test will display. Complete this section the same way as you completed the HIV Test 1 section. 14 6. Select a response for Choose one if in the Behavioral Risk Factor Section. Notes: If you select Client completed a behavioral risk profile, more items will display for risk behaviors in the last 12 months. Select responses for each of these items. 15 Note: If you select Yes for these risk behavior items, sub-items may display. Be sure to select responses for all sub-items. 7. Select responses for one or more Session Activity items in the Activity section. Tip: Click in the Session Activity boxes to displays the selections. 16 8. (Optional) Type responses for the Local Use Fields in the Local section. Note: Check with your agency s EvaluationWeb administrator to find out whether your agency uses any of the Local Use Fields. 9. Select responses for each of the items in the Referrals section. Note: Selecting certain responses in the Referrals section will make additional items display. When this happens, be sure to select responses for the additional items. 17 10. Select a response for the Surveillance question. 11. Select responses for the 5 items in the HIV Incidence section. Note: Selecting some responses in the HIV Incidence section will make additional items display. Select a response for the first question in the PS /PS Additional Questions section: Note: The particular question displayed depends on whether you previously indicated that the client s HIV test result was positive or negative. The questions are: (For negative clients) - Is the client at high-risk for HIV infection? 19 13. Select one or more Services for HIV Negative Clients Only or Services for HIV Positive Clients Only in the Navigation and Prevention and Essential Support Services section. Notes: For all services, you can select the service as being Referred. For some services, you can also select the service as being Provided. The particular services displayed depend on whether you previously indicated that the client s HIV test result was positive or negative. 20 14. Select any Additional Support Services For All Clients in the PS /PS Additional Questions section. 15. Click Submit Form at the bottom of the form. Result: A Data Submitted dialog box opens. 16. Click OK in the dialog box to close it. Result: The data you entered is saved to the database. Note: If you forget to type or select a response for a required item in the data form, a dialog box will open. The box will indicate which item has missing information, as seen in the image below. 21 Viewing a Previously Submitted HIV Testing Data Form Viewing a previously submitted form Follow the steps below to view an HIV Testing form already saved to the EvaluationWeb database. Step 1. Click the Data Entry button in the menu of the left side of the EvaluationWeb landing page. 2. 22 Step Result: A list of previously submitted HIV tests displays, listed by Form ID. The Form ID tab is active. 3. Click the View ID button for the form you would like to view. 23 Step Note: If you do not see your Form ID in the list: Locate the Please enter Form ID filter box. Type any portion of the ID into the box. 24 Printing and Exporting an HIV Testing Record Printing & Exporting a Record Follow the steps below to print and export a PS HIV Testing record. Step 1. View the record you would like to print or export. 2. Scroll to the bottom of the record, and then click the Print Form button. 25 Step Result: A new browser window or tab opens and displays your data. A Print dialog box displays on top of the table. Note: Your browser may open a dialog box that looks different from what is shown in the screen shot above. 3. Use the Print dialog box to make printing selections and initiate printing. 4. Close the browser window displaying your data to return to the HIV Testing form. 5. Click the Export to PDF button at the bottom of the HIV Testing form. 27 Editing a Previously Submitted HIV Testing Data Record Editing an HIV Testing Record Follow the steps below to edit an HIV Testing data record that was previously submitted to the EvaluationWeb database. Note: HIV Testing forms lock 90 days after the Session Date. They can no longer be edited at that time. To have forms unlocked, send an request to the Luther Consulting help desk, and include in your the Form IDs you would like to have unlocked. Step 1. View the form you wish to edit. 2. 28 Result: The form displays in edit mode. 3. Make any desired changes in the form. 4. Scroll to the bottom of the form, and then click the Submit Form button. 5. The Data Submitted dialog box opens. 6. Click OK to close the dialog box. Result: The changes you made are saved to the database. 29 Introduction Using Reflexx to Create PS Reports Introduction You can use the Reflexx report writer to: Create reports from your PS data, viewable in several formats including graphs Export data to a comma separated values (CSV) file, which you can then open in Microsoft Excel or import into a statistical package such as SPSS or SAS. Save data queries Accessing Reflexx Accessing Reflexx Follow the steps below to open the Reflexx report writer. Step 1. Click the Reports button on the EvaluationWeb landing page. 2. 31 Creating a Report Setting up a Report Query Creating a report involves two tasks: setting up a report query, and then running the query. When EvaluationWeb finishes running the query, it generates a report for you to view. This section gives the actions to follow to set up a report query. The next section gives the actions to follow to run the query. If instead of creating a report you would like to create a dataset for export into Microsoft Excel or for import into statistical software, see Quick Data Export to a CSV File or Exporting Data to MS Excel, MS Word, or PDF. NOTE This and the next section describe how to set up and run a basic report query. For more information on the various types of reports Reflexx offers and for detailed instructions on all Reflexx functions and tools, refer to the Using Reflexx computer-based training course and the accompanying Using Reflexx User Guide. Both of these resources are available on the EvaluationWeb Help page. Perform the following steps to set up a Reflexx report query. Step 1. 32 Step Result: The New Report window opens. There are three report types for PS data. They are shown in the image below. TIP HIV Testing Data (Slower, Live Data) includes data submitted to the database the same day you run the report HIV Testing Data (Fast, 24 Hour Delay) is optimized for speed. Therefore, it builds and displays a report for you more quickly, but it does not include data submitted to the database the same day you run the report. In general, it is better to use the faster report type, unless you need to view same-day data. Reminder: Even though you can no longer enter PS Client Summary RRA data into EvaluationWeb, you can still create reports from Client Summary RRA previously submitted to the database. 33 Step 2. Select one of the three PS report types and click the Create Report button. Result: The report setup window opens. This is the window in which you set up your report query. It displays three main sections: Step 1 Click and Drag Variables for Report Step 2 Set Visibility and Computation Options Step 3 Date and Break Out Selection Entering and Managing Data in EvaluationWeb for CDC Program Announcement PS Page 29 of 68

34 Step 3. Drag and drop variables from the Available Variables area to the Selected Variables area in Step 1. Note: Drag and drop only the variables you want include in your report. Tip Selecting a variable category in the list of Quick Picks will filter the variables that display in the Available Variables area. For example, in the image above, Demographics has been selected in the list of Quick Picks, so only demographics variables display in the Available Variables area. 4. Select check boxes in the Step 2 area to make specific variables visible in your report, to add counts to your report, and to add statistics such as means, minimum values, maximum values, and standard deviations to you report. 36 Running a Query Perform the following actions to run a report query and create a report. 1. Access the Welcome to Reflexx page, and then set up a data query. 2. Click the Report button in the bottom right corner of the Reflexx setup window. Result: Reflexx creates your report. Your data is displayed in a tabbed interface giving you different views of the data. The Counts tab is active. 3. Click each tab one at a time to view all report data. NOTE For detailed information on viewing and manipulating report data on the various tabs, refer to the Using Reflexx computerbased training course and the accompanying Using Reflexx User Guide. 37 Quick Data Export to a CSV File Exporting to a CSV file If you want to quickly export data from the EvaluationWeb database, use the Reflexx Quick Export to CSV feature described in this section. This feature exports your data to a.csv file, making the data available for import into Excel or into statistical programs such as SAS or SPSS. Note that this method is faster than exporting data to an MS Excel, MS Word or PDF file because the data is not formatted when exported to a CSV file, therefore requiring less processing time. Note: This section explains how to create a.csv file from a new report query. However, you can also create a.csv-formatted file from a saved query. To do so, click the Find Saved Report icon, and then locate and open the saved report query. This will take you to the Reflexx setup window, from where you can export the report s data. Follow the steps below to create a CSV file from a new query. Step 1. Access Reflexx, and then click the Export Data to CSV icon on the Welcome to Reflexx page. Note: Depending on your browser, the file may download locally to your system s download folder, or your browser may open a Save window that you can use to manually save the file locally. 42 6. Click the arrow for the downloaded file to see file options, and then click Open in the dropdown menu. Result: A data file opens, displaying the data that matches selections you made in the Reflexx report query. You can save the file locally in the format you need. 43 Saving a Report Query Saving a query Follow the steps below to save a Reflexx report query. Important: Saving a report query refers to saving the selections you make in the Reflexx report setup window. You are not saving the actual report data you get when you run the query. Therefore, if you were to set up a query, run it, and save it, and then run the saved query at a later date, you may likely see different data. This is because new data records may have been added to the database in the days between your two query runs. If you run a query and want to save the exact data snapshot you get at that specific time, then do a quick export to a.csv file or export your data to an MS Excel, MS Word, or.pdf file. Perform the actions below to save a report query. Step 1. Set up a report query in the Reflexx setup window. 2. 44 Result: The Save Report window opens. 3. Type a Name and Description in the Create new saved Report section of the window. 4. Click Save at the bottom of the window. 45 Viewing a Saved Report Perform the actions below to view a saved report. Step 1. Click File in the menu bar, and select Find Saved Report in the dropdown menu. Result: Reflexx creates the report and displays the report data in a tabbed interface, with the Counts tab active. 50 Exporting Data to MS Excel, MS Word, or PDF Exporting to Excel, Word, or PDF Follow the steps below to export data to Microsoft Excel, Microsoft Word, or to a PDF file. Reminder: Exporting data to an Excel, Word, or.pdf file requires extra processing time because the data is formatted. For quicker exporting. export data to a.csv file. This is quicker because the data in a.csv file is not formatted. Step 1. Set up a data report query. 2. Run the report query. Result: Your report opens in a tabbed interface with the Counts tab active. 3. Do one of the following: Continue to the next step to export from the Counts, Computations, Graphs, or Print All tabs. 51 4. Click the Export to PDF, Export to Excel or Export to Word button at the bottom of the Counts, Computations, Graphs, or Print All tabs Result: The data is exported in the file format you chose. You can save this file on your computer. You do not need to complete the remaining steps. Note: Instead of the dropdown file button seen in the screenshot above, you may see a dialog box. 52 5. Click the Export to PDF, Export to Excel or Export to Word icon near the top right corner of the List tab. Result: An Export Options window opens. 6. 54 Viewing Agency Data Other EvaluationWeb Functions View agency data Follow the steps below to view data associated with your agency. Step 1. Click the Agency Data button in the menu on the EvaluationWeb landing page. 2. 56 Changing Your Password Changing password Follow the steps below to change your EvaluationWeb password. Note: You may change your password no more than once every six days. Step 1. Click Other in the menu on the landing page. 2. Click Change Password. 57 Step 3. Type your Old Password, your New Password, and then Confirm New Password in the Change Password window. 4. Click Submit. 58 Setting Default Responses Setting defaults You can set up your data-entry windows so that certain items have particular responses already selected whenever you go to the windows. You would do this for items which often have the same response for many of your HIV Testing forms. If you set up default responses for various form items, you still have the ability to change the responses whenever you are entering data into a form. Follow the steps below to set default responses in your data-entry windows. Step 1. Click Other in the menu on the left side of the EvaluationWeb landing page. 2. Click the Set Defaults button. 59 Step Result: The defaults selection window opens in a tabbed interface, with the Client Defaults tab active. 3. Type or make selections for each item that you would like to set a default for. 60 Step 4. Click Set Defaults at the bottom of the window. Result: A Data Submitted dialog box opens. 5. Click OK to close the dialog box. 6. Click the Testing Defaults tab. 7. 61 Step 8. Click the Set Defaults button. Result: A Data Submitted dialog box opens. 9. Click OK to close the dialog box. 10. Click the Client Risk Defaults tab. 11. Type or make selections for each item that you would like to set a default for in the Client Risk Defaults tab. 63 Viewing Administrator Messages Viewing messages EvaluationWeb administrators may periodically send messages about the system to EvaluationWeb users. Follow the steps below to view these messages. Step 1. Click Other in the menu on the left side of the EvaluationWeb landing page. 2. Click the View Messages button.
