1 Web Data Entry Tool Training guide 2018 MIPS reporting Logging into the dashboard: Please click on the link ( to be taken to the dashboard. Login to the screen below using your AAD member ID credentials. ***If you need to reset your password or encounter problems logging into the dashboard, please click on the Reset Password? icon at the bottom. You can also FIGmd for technical support 1

2 After logging in you will be taken to the main DataDerm MIPS dashboard page. **This is where you will enter to see how your MIPS scoring and points will be assessed. You can navigate between the MIPS Quality dashboard and the 2018-Web Tool. Entering your 2018 MIPS Quality data: On the left hand side of the screen you should see a menu. Please select 2018-Web Tool and then Web Data Entry This is where you will be able to enter patients one-by-one into the system. 2

3 Once you have selected Web Data Entry 2018, you will see the main screen below with the Manage Provider Profile and the Report Measures sections listed at the top. Select the provider you are wishing to add patients for by checking the box on the left hand side next to their name. If you believe a provider is missing from this section, you may add a new provider by clicking the blue button on the lower right hand side of the screen labeled Add New Provider. 3

4 You will need to complete all of the designated fields listed in the box below in order to add the new provider to the dashboard. Once added, please click Save. Once the appropriate provider has been selected, you can either click Next on the bottom right hand side of the screen OR you can click on Select Measures from the left side menu. The first time you enter the Web Data Entry 2018, you will need to select which measure(s) you would like to report on by placing a check mark in the box to the left of the corresponding measure. 4

5 Be sure to click on the Save button in order to ensure all of your measure choices have been selected and saved appropriately. Once the measures have been selected, you can navigate to the next section by selecting Next or clicking on the Report Measures icon at the top of the screen. Once you are in the Report Measures section, please select the Patient Information to enter new patients into the dashboard. You will need to complete all of the fields in the Patient Information section with a red asterisk next to them. This indicates required fields. The required fields are: Date of Visit, MRN, Insurance Type and Age. Once you have completed all of the required fields, you may select Proceed at the bottom of the box. 5

6 After selecting Proceed, you will be taken to the following page where you will answer designated questions for each of your chosen measures. ***Please note that the questions on the right are designed to test patient eligibility for the selected measures on the left. You will need to place a check mark next to each statement that applies to the patient in order to be able to choose the corresponding reporting code at the bottom of each measure set. Depending on your selection of answers to each question, you may be prohibited from answering the remaining questions. 6

7 If you are reporting on multiple measures, you can navigate to the next measure by clicking on the measure title listed on the left hand side. When you are done with that particular patient s entry, please hit the green button, Finish at the top of the screen. 7

8 You will then be taken to the following page: **Under Advanced Status: You will be able to view all of the patients entered and the various measures you have chosen. You will see which patients are listed as Eligible, Met, Not met, Not applicable, Exclusion, or Exception for each measure. You will also be able to delete patients from your dashboard as well as view your performance for each measure. Please follow this process for each patient you are trying to enter for each provider. You will be able to add more patients directly from this page by clicking on Patient Information. 8

9 Editing Previously Entered Patient Visits: If you need to edit an already existing patient visit, this can be done under the Reported Visits section. You can select the patient entry then edit any answers you have already listed for that patient by clicking Reset. ***As always, be sure to click Finish when you are done in order to save the newly edited entry. 9

10 Editing Patient Information If you need to edit an already existing patient information (ex: MRN, visit date, insurance type, age, etc.), this can be done under the Reported Visits section. Select the patient entry You will be taken to the following page per the patient visit selected. You will click on Edit Patient Information located at the top right of the screen. 10

11 The following page will display the previous patient entry. You may edit any of the following patient information and then click proceed. After clicking Proceed you will be taken to the page for that specific patient s measures. If you d like to edit the measure answers you may; if not, you can click Finish on the top right. 11

12 Entering Multiple Visits for a Patient: In order to add a second or third visit for an already entered patient, you will need to click on the Patient Information button. In order to find an already existing patient, you will need to type the assigned MRN (unique identifier) for that patient previously into the search bar at the top of the patient information box. You should see the corresponding MRN or unique identifier appear along with that patient s information prepopulated into the box. You will then just need to add the new Date of Visit. Click Proceed to be taken to the next measure page. 12

13 Please complete the questions for each measure(s) that applies to the patient and their new visit date. 13

14 Deleting Patient Visits: In order to delete a patient visit under Reported Patient Visits, please click on the trash can icon under the Remove column per each patient visit date you would like to delete. Please be sure you find the correct patient entry (date of visit), if you need to search for a particular patient or visit date, you can use the search bars at the top of the box to find the correct patient entry. ***If you would like to see how these Quality measures will be scored on your MIPS dashboard, please review the MIPS Quality training guide attachment on our website. 14
