5 HCQIS Access Roles and Profile (HARP) Account HARP is a secure identity management portal provided by the Centers for Medicare and Medicaid Services (CMS) Previously, users managed their Quality payment Program (QPP) data through the Enterprise Identify Data Management (EIDM) system If you already have an EIDM account and have verified it, you can sign in to qpp.cms.gov with your user ID and password and you don t need to register for a HARP account 5
6 Why Do I Need a HARP Account? HARP will now act as the EIDM account has in previous years Will allow QPP data to be submitted to the portal via qpp.cms.gov Will allow clinicians/groups to view their QPP score Track changes/improvements in score 6
7 Register for a HARP Account 1. Go to and click sign in on the upper right hand corner 2. Click the register tab at the top of the page 7
8 Register for a HARP Account 3. Click Register with HARP at the bottom of the page, at which point you will be redirected to the HARP site to complete your registration 8
9 1. Enter profile information (name, date of birth, address, SSN) Register for a HARP Account: Profile Information You must provide all required information and agree to the Terms and Conditions before you can click next Note: enter your personal home address, not your organization s address 9
10 2. Create your user ID, password, and challenge question Register for a HARP Account: Account Information The challenge answer is not case sensitive and has a minimum character length of 4 (maximum of 100) alphanumeric characters 10
11 3. Based on the information provided in Profile Information, Experian will generate personalized remote identity proofing questions unique to you. Answer the remote proofing questions for Experian to confirm your identity Register for a HARP Account: Remote Proofing 11
12 If your identity cannot be verified, you will receive an error message prompting you to contact Experian Verification Support Services If Experian cannot verify your identity, you will need to initiate a manual proofing process Register for a HARP Account: Remote Proofing Error 12
13 4. Your HARP account has been created! Please click Login to Complete Setup to set up a device for twofactor authorization In addition to the confirmation messaging on the registration page, you will also receive an which confirms your registration and contains your User ID Register for a HARP Account: Remote Proofing Confirmation 13

Enter your newly created User ID and password, agree to the Terms and Conditions, and then click Login Register for a HARP Account: Two-Factor Authentication CMS uses two-factor authentication every time you access your HARP account. This means that each time you login, you will be prompted to enter a new security code sent to the device you set up in the following steps 14
15 After logging in, you will be prompted to add a device type select either SMS or Voice from the drop down After selecting the device type, you will be prompted to enter the phone number where you wish to receive your security code. Then click Send Code Register for a HARP Account: Two-Factor Authentication 15
16 6. Once your account setup is complete, you will be able to sign in to QPP or go back and sign in to your HARP user profile Register for a HARP Account: Process Complete! 16

18 New users will have to select the organization (practice, APM entity, registry or virtual group) to connect to Manage Access Connect to an Organization 18
19 Connect to an Organization Locate applicable organization via Legal Business Name or TIN 19
20 Selecting a Role Each organization must have a Security Official before anyone can request a Staff User role or additional Security Official roles If you are the first person to request access to the organization, you will only be able to request the Security Official role If Security Official is not the appropriate role for you, contact an administrator in your organization to let them know your organization needs a Security Official 20
21 First Security Official If you are the first Security Official, you will be automatically approved after the required information you provide is validated You must have the practice TIN, NPI for a connected clinician and the PTAN You will approve pending role requests from other users requesting access to the practice. For additional information on how to approve other users in your organization 21
22 HARP User Roles Organization Staff User Role Lets You Type Practice View eligibility information for all clinicians in the practice View claims measure data during the performance period submitted by individual clinicians in the practice Access the Physician Compare Preview Submit data on behalf of the practice, whether reporting individually or as a group View individual and group MIPS performance feedback and payment adjustment information Request a Targeted Review APM Entity Submit quality data on behalf of the Shared Savings Program ACO entity, Next Generation ACO entity, or CPC+ Practice Site View MIPS performance feedback for the Shared Savings Program ACO entity or Next Generation ACO entity, with clinicians scored under the APM Scoring Standard Registry Submit data on behalf of all your clients View preliminary performance feedback for your clients based on the data you submit on their behalf Virtual Group View a list of all the practices and clinicians participating in your virtual group Submit data on behalf of the virtual group View virtual group MIPS performance feedback Security Official Role Lets You Do everything the Staff User can do PLUS: Register a group for the CMS Web Interface Register a group for the CAHPS for MIPS survey Approve all additional user requests (manage access to the organization) Do everything the Staff User can do PLUS: Approve all additional user requests (manage access to the organization) Do everything the Staff User can do PLUS: Approve all additional user requests (manage access to the organization) Do everything the Staff User can do PLUS: Approve all additional user requests (manage access to the organization) 22
