1 Prcedure f applying fr TCS Class 3 Cmpany Type Digital Signature Certificate (DSC) Please read and fllw the fllwing very carefully. Incmplete / imprper filled frms delay the Certificate issuance prcess. Keep a cpy f all dcuments fr yur recrd. 1. Fill the TCS Class 3 DSC - Cmpany Request Applicatin Frm with Pen in BLOCK LETTERS 2. Write FULL NAME in the frm as mentined in the Prfs attached. 3. Write addresses clearly, matching Prf f address. 4. Applicant shuld sign n 3 rd and 7 th page f the frm, and acrss phtgraph n 1 st page. 5. D nt verwrite, r use Whitener. Else applicant shuld apply full signature at place f crrectins id Write id carefully & neatly. DSC can be dwnladed nly thrugh s received in that id. Hence a valid & active id shuld be prvided, which cannt be changed nce the DSC is generated. The DSC cannt be dwnladed in case id is wrng r nt accessible. 7. Attach latest cpies f fllwing 4 prfs (as per the checklist in the frm, page n. 4 and 5) a. Prf f the Cmpany s Incrpratin (depending n Pvt. Ltd, Partnership...) b. Pht id prf f applicant (Passprt/ Pancard / Driving License) c. Residence address prf f applicant (Passprt / Ratin card / Driver s license / Telephne bill / Electricity bill / Bank Statement) d. Cmpany s / Prprietr s PAN Card All abve shuld be either Ntarized by Public Ntary r attested by Gazetted fficer nly. e. Authrity letter given n last page duly signed at all 3 places, by applicant & authrizing persn, with stamp & designatin. 8. Spelling f details filled shuld match exactly with prfs attached. 9. Send all these dcuments t us at fllwing address alngwith payment. Nte: In the case where the address n the pht-id prf prvided e.g. cpy f Passprt r Driving License des nt match with the address in address prf prvided, an affidavit shuld be submitted alng with the supprting dcuments and the applicatin frm. Imprtant: In case the prcedure fr nline enrllment and dwnlading f DSC is t be dne by us, yu wuld have t frward us thse s, which yu wuld receive frm TCS n id mentined in Frm, immediately. 8 Declaratin I hereby agree that I have read and understd the fllwing instructins carefully and ensure prper usage f the Digital Certificate. 1. The certificate shuld be dwnladed nt the same machine/device frm where the request was initiated. 2. After placing an nline request fr a certificate, the fllwing activities shuld nt be carried ut until the certificate is successfully dwnladed: Frmatting f the machine the machine Reinstallatin r upgrade f the internet brwser n the machine frm which the certificate request was initiated 3. At the time f registratin, a valid ID that is accessed regularly shuld be prvided. 4. Certificate revcatin is permanent and irreversible. If my certificate is revked, I will have t reapply fr a fresh certificate. The same will be apprved nly after the payment f necessary applicable charges. 5. The security level in the Internet Brwser shuld be set t Medium and all scripting shuld be enabled. 6. The Certificate Trust Chain has t be dwnladed fr using my certificate. (Link: 7.
