3 Home To begin the subscriber registration process, visit to securely register to view your specific benefit information via the NVA website. Simply click on the Subscriber Registration link located in the upper right hand corner of our Home Page. 3

4 Subscriber Registration Next, you will be directed to the Subscriber Registration screen (shown at right). Fill out all registration fields and click on the Submit button. After clicking Submit, you will be sent an e- mail to your registered address (shown on the next slide). Required Fields 4

5 Confirmation From: To: Date: 10/24/ :10:53 EDT Subject: Registration at Click on the Continue Registration Process link provided in your message to continue on to the verification portion of the registration process. 5

6 Verify Registration Information After clicking on the Continue Registration Process link, you will be directed back to the Subscriber Registration page to verify your registration information and accept the Internet User Agreement (shown at right). Once you have agreed to the User Agreement and completed the registration process, you will then be allowed specific access to your account. 6

7 Registered Subscriber Home You are now registered and logged-in, allowing you access to navigate through the NVA website via the navigation toolbar or via the NVA Smart Buyer subscriber home page (shown at right). Beginning with the View Eligibility option on the navigation toolbar, you can review each toolbar option s capabilities and the information available to you. 7

8 View Eligibility Clicking on the View Eligibility link in the navigation toolbar will bring you to the Select Dependent page (shown at right). Click on a dependent name to view that eligible family member s data (as shown in the following slide). 8

9 Eligibility Details You have now been directed to the dependent s Eligibility Details screen (shown at right). 9

10 Print Your Own ID Card Click on the Print ID Cards link in the navigation toolbar to print ID cards from your local computer, at your convenience (shown at right). After confirming your subscriber information shown on the screen, click the Print ID Card button. 10

11 Find an Eyecare Professional (ECP) To locate a participating eyecare professional (ECP) in your area - via the subscriber homepage, first make sure you are on the Vision Benefit Maximizer ECP Search Tool tab. Next, click the Find Eyecare Professional link in the navigation toolbar or click the Try it now link within the NVA Smart Buyer preview screen. You will then be directed to our ECP search page (shown in the next slide). 11

12 Find Eyecare Professional Search Screen You will have the option of searching for a participating ECP either by Zip Code + Mile Radius or by State + City (or State + Provider Name). Once you have entered the necessary search criteria, simply click on the Find Provider button to view the list of participating ECPs within your search area. 12

13 Vision Benefit Maximizer Provider Search Results Your provider search will list the ECPs in your search area plus their location, contact information, service features, and exclusive to NVA s Vision Benefit Maximizer, it lists the number of frames available to you at $0 out of pocket (under your benefit) at that particular provider. 15 Nominate an Eyecare Professional If you do not find your preferred provider in NVA s network, you may choose the Nominate Eyecare Professional link. Fill in all required fields and click on the Submit button found at the bottom of the screen. 16 Subscriber Access Materials Click on the Subscriber Materials link to find forms and additional important information (shown at right). Please Note: Subscriber materials may be added at any time, so check back often. Click on the material s link to view the document. In this case, you will view the Authorized Representative Form. 16

17 Assign Authorization for Access The Authorized Representative Form will open with Adobe Reader in a separate window from the website. (This particular form can be used if you wish to designate another person to access your vision care benefit data). 17

18 Frequently Asked Questions Under the Helpful Info heading on the navigation toolbar, click on the Frequently Asked Questions link to access NVA s responses to some of our members most commonly asked questions. 18

19 Wellness Next, you can click on the Wellness link in the navigation toolbar to stay informed of important information that may help you better protect your family s eye health, no matter what their age. 19

20 View Processed Vision Claims Click on the View Claims link to view a list of dependents (shown at right). From here you will be able to click on the name of the dependent in question to view their listed claims (as shown on next slide). 20

21 Claim Schedule You can now see the schedule of this member s claims. Click on a claim number to view further details into that claim. 21

22 Claim Detail Clicking on your claim number will direct you to that claim s detail screen (shown at right), identifying the claimant, DOB, Payment Status to Provider (or member in the case of out-of-network claims), Claim Number, Date of Service, Provider Name, and the type of service(s) that were provided. 22

23 Submitting a Claim When submitting a claim, simply click on the Submit Claim link to view a list of dependents (shown at right). From here you will be able to click on the name of the dependent you wish to submit a claim for. (Please Note: in order to submit your claim online, you will need to have your receipt(s) saved to your computer in.gif,.jpg, or.jpeg format) 23

27 Once you have submitted your claim, you will be taken to the Claim Confirmation screen where you can view and print your claim information (to print, click on the Print Claim Confirmation link at the bottom of the screen). Claim Confirmation From this page you can now navigate back to the Subscriber Home screen to see two ways to access the NVA Smart Buyer s Guide to Frames. Click on the Subscriber Home link at the top left of your screen. 27

28 Smart Buyer s Guide to Eyeglass Frames To access the NVA Smart Buyer s Guide to Eyeglass Frames, first make sure you are on the Frames tab within the NVA Smart Buyer preview screen. Next, you may either click the Eyeglass Frames link in the left hand navigation toolbar or click the Discover which frames suite you best. link under the Frames tab (shown at right). You will then be directed to our Smart Buyer s Guide to Frames page (demonstrated in the next slide). 28
