1 7/5/16 Video Visits on MyBellinHealth with Android Video Visit Requirements and Set-Up for Android: 1. To conduct a video visit via your Android you will need the following: a. An active MyBellinHealth account. If you do not have an account or have forgotten your userid or password, please call (888) , available 24 hours/7 days a week, prior to your video visit appointment. b. Android with MyChart app downloaded. To download the MyChart app: Go to the Play Store on your phone and search for MyChart. Download the MyChart app from Epic. Once the app is downloaded, open the MyChart app. Under the Select a Healthcare Provider, select Wisconsin or Michigan, then select MyBellinHealth. Once you have set this up on your phone, the next time you open the app the MyBellinHealth account will always display. Login using your MyBellinHealth login credentials. To Schedule your Video Visit via Your Android: 1. Log into your MyChart app using your MyBellinHealth credentials h Patient Education - printed copies may be out of date. (q3yrs) Page 1 of 7

2 2. Select the Schedule Appointments icon. 3. From the Schedule Appointment screen: a. Select Video Visit on the Reason for Visit drop-down menu. b. Select Provider, Bellin Video Visit on the Provider and Location drop-down menu. c. Select Find Available Times. 4. A list of dates and times will display. 3 5. Review your appointment information and enter your reason for visit in the Comments field. 6. Select Schedule. 7. You will see a message that your appointment has been scheduled. Note: The first time you schedule a video visit on your phone, you will be prompted to allow the MyChart app access to your Microphone and Camera. Once your appointment has been scheduled via your smartphone, you will need to answer a questionnaire. To do this, please go to the home screen on your MyChart app and select Questionnaires. 4 Select Acknowledge for each agreement and select the reason for your video visit. Select the reason for your video visit. Indicate if you have an employer clinic code from your employer and enter it in the field. Select Continue h Patient Education - printed copies may be out of date. (q3yrs) Page 4 of 7

5 The next screen will allow you to review your answers and change them if necessary. If everything looks correct select Submit Questionnaire. 8. Once your appointment has been scheduled, please go to the main screen of the app and select Questionnaires. To Test Your Connectivity to the Video Visit or Start Your Video Visit: 1. It is important that you test your connectivity to your Video Visit before you start the visit. To test your Video Visit, login to the MyChart app on your phone and select the Appointments tab h Patient Education - printed copies may be out of date. (q3yrs) Page 5 of 7

6 2. The Appointments screen will display your upcoming appointments. Next to the video visit appointment a video camera icon will display. a. Green camera icon means you can join the video visit by selecting the visit. b. Gray camera icon means it is not time to join the appointment but you can test your connectivity by selecting the appointment. 3. You can test your connectivity up to 15 minutes before your appointment. To test this, select your video appointment from the Appointments list. 4. Select the Test Appointment button. 7 5. You can connect to your video visit up to 15 minutes before the appointment time. From the Appointments screen select your appointment. 6. Select Begin Visit. 7. The screen will indicate that it is connecting and waiting for your provider to connect as well. Once the provider connects you will see him or her in the window. 8. The red button will disconnect you from the video visit. If you get disconnected and need to reconnect, the Appointment screen will display and you can select Begin Visit to reconnect. 9. The camera icon on the left will allow you to switch between the front and back camera on our phone. 10.
