2 Instructions for Returning Sensors for Warranty Credit; If the normal RMA form is used rather than the Q-Series Sensor Recall form, the material returned will be handled under the normal warranty/excess policy. Materials returned under the Sensor Recall are not eligible for any other form of material and/or labor relief. 1) If product is being replaced on a job site, new product should be ordered and the old sensors replaced on the site before completing the Q-Series Sensor Recall form. 2) Complete the Q-Series Sensor Recall form following the instructions below. A separate form must be completed for each job site/location where sensor products are being replaced. All fields, except where noted in the instructions, MUST be completed in order to receive credit. Contact Customer Support (see bottom of page) for an RMA number (Return Material Authorization Number). 3) Package replaced materials and return to Buffalo Grove along with the Sensor Recall form. Instructions for Completing Sensor Return Material Form; I. a. Customer / Branch Name: Enter the name of the branch, VAP, or Installer company making the request for return credit. If this is a direct customer return the Customers name should be entered b. VAPs and Installers must fill in their Siemens Customer ID. This field is not required for the Siemens branch offices or Customer Direct Returns c. Enter a contact name, , and phone. This is the person to be contacted for questions regarding the submitted request form, and informed of the status of the submitted request. d. Branch Offices only enter the local plant and storage location the parts are being returned from II. A separate form must be completed for each job on which the sensors are being replaced. a. Job Name; Enter the name or description of the job. If the sensors are being returned from stock, simply enter the word STOCK in the job name field. b. Job Number; this field is for Siemens branches only. This field could be used for a branch Service Account number. c. Job Location; Enter the street address, city, state, and zip code for the job III. a. Select the option from the list indicating how you, or the person in your office instructing you to return this material, were informed of the product recall b. If you select other, enter how you learned of the recall IV. Return Information A line should be completed for each unique sensor part number being replaced. If more than 6 unique part numbers are being returned from this location please attach a second form following the same instructions. a. Indicate the sensor part number being replaced b. Indicate how many of that part number are being returned c. If the part number is being returned from stock and there is no replacement being installed, enter the original Purchase Order number for the parts if it is available. The remainder of the fields in the line can remain blank (skip d, e, f) d. If the returned part has been replaced with a new sensor, indicate whether or not the wiring in the installation was converted from AC to DC. Only replacement P/Ns with 20 or 30 (Ex: QFA20xx ) will be given credit for wiring. e. If the returned part has been replaced with a new sensor, enter the Part Number and Purchase Order number for the replacement parts. If the recalled sensors on a site are being replaced with multiple new part numbers, complete a line for each replacement part number even if the same PO number is used. V. General (Siemens internal use only) a. DO NOT enter any information in the gray area fields. This information will be entered by Customer Support.
