1 Stage 2 Meaningful Use Measure #27 & 28 Timely Access A & B OBJECTIVE: MEASURE A: Provide patients the ability to view online, download and transmit their health information within four business days of the information being available to the EP. More than 50 percent of all unique patients seen by the EP during the EHR reporting period are provided timely (available to the patient within 4 business days after the information is available to the EP) online access to their health information, with the ability to view, download, and transmit to a third party. MEASURE B: More than 5 percent of all unique patients seen by the EP during the EHR reporting period (or their authorized representatives) view, download, or transmit to a third party their health information. Changed to 1 patient in final rule for 2015 and Stage 2 Meaningful Use Measure #36 Secure Electronic Messaging OBJECTIVE: MEASURE: Use secure electronic messaging to communicate with patients on relevant health information. For an EHR reporting period in 2016, for at least 1 patient seen by the EP during the EHR reporting period, a secure message was sent using the electronic messaging function of CEHRT to the patient (or the patient-authorized representative), or in response to a secure message sent by the patient (or the patient-authorized representative) during the EHR reporting period. How to meet the measures for Patient Portal Timely Access A & B and Secure Electronic Messaging In order to meet the Timely Access A measure, you must collect the patient s address and web enable them on the Patient Info screen for 50% of the patients seen during the measurement period. (See Exhibit A below.) In order to meet the Timely Access B measure 1 patient seen during the measurement period must sign into their portal account and View My Health Summary (See Exhibit B - green arrow). In order to meet the Secure Electronic Message measure, you must send a message via the portal to a patient. (See Exhibit D and E ). If you are not currently using the patient portal, an excellent first step is to activate and use a test patient to become familiar with the setup and utilization of these features. Follow the steps below to activate Happy Person, create an encounter/superbill then send a message from their portal and reply back to them from your EHR (See Exhibit D and E ). Page 1 of 6
2 Activate the Patient s access to the portal: Exhibit A 1. Enter patient s (blue arrow) and hit <Update> (red arrow) 2. Click on No Web to activate and activation information (green arrow). 3. If you need to resend an activation code to the patient, first click on the Deactivate Patient Portal button and then click on No Web. Page 2 of 6

5 When a patient sends a message it will display in the Routed Documents for the employee set up to receive these messages for the provider and in the patient chart. Open the message, add your comment. (The patient will view your comment when they view the document in their portal. ) Make sure that the MR Print box on the left is checked (Blue Arrow) and then click on the < > box to send back to the patient (Red arrow). Click on the <Add> Button to send (next page, Green arrow). Make sure that you get the Message Box that states Reminder sent successfully sent (Yellow arrow). Exhibit D Page 5 of 6
