2 Dear Parents and Students: Please read this packet thoroughly and follow the instructions regarding fees, schedule changes, and parking permits. NO ELECTIVE CHANGES will be permitted. STEP 1 STUDENT SCHEDULES AND CHANGES / OR CALL IN Enclosed is a copy of your child s schedule. Alternative classes may be listed due to the following reasons: Student did not meet course prerequisite Course availability restrictions (course filled up or course cut) Conflict(s) in the student s schedule If you are requesting a schedule change, answer these questions: Is my schedule missing a period during the day? Did I attend summer school or take an on line course and the course I took is listed on my schedule? Do I want to drop a class(es) and have release time? Please note: There will be no elective class changes at all. If you answered YES to any of these questions above and need to request a change, follow the steps below: 1. Please speak clearly. If we can t understand your phone number, we will not be able to call you back.
3 5. Please DO NOT call more than once. 6. A counselor will return your call within 24 hours. Please give us a contact number where you or your student will be easily reached. 7. We will not have walk in schedule change opportunities. If you want to request a change online, instructions will be ed through Infinite Campus prior to your call in day. STEP 2 PAYMENTS (FEES & PURCHASES) Fee payments can be made ONLINE. To make a payment for your student, please follow online payment directions explicitly to ensure your child s fees are paid correctly. All fees and purchases must be PAID IN FULL, no partial payments will be accepted. If you have just registered your student(s) for the first time in Gilbert Public Schools, you will need to pay by check or money order as your online parent portal access is not available until school begins. Credit/Debit Card Payments: Credit card purchases must be made through the Infinite Campus Parent Portal under the Payments tab. To see all fees, click the FEES tab. To make payments, click the PAYMENTS tab. For optional purchases, such as a parking permit, leave the boxes blank if you do not wish to purchase these items. Please note : The ability to make a credit card payment through Infinite Campus smart phone application is not available and the DRHS Bookstore can no longer accept credit card payments. Check and Money Orders: Check and money orders may be mailed in or dropped off at the DRHS front office reception desk. The enclosed Fee Payment form must be included. The DRHS Bookstore won t open to the public until the first day of school. Optional purchases are as follows: Parking Permit when paying online the Parking Permit form must be dropped off or mailed in before the permit will be issued. Student Activity Card allows free admission to regular season home athletic events and dance discounts. Tournament and State athletic events are not included. This year it can be prepaid online or by mailing in/dropping off the enclosed fee form. Yearbooks May only be jostensyearbooks.com. ID AND ACTIVITY CARDS All ID cards will be made during the second week of school. Seniors who have release time, EVIT students, and students with seminary should use their schedule to get off campus until their ID card is made.
4 UPCOMING DATES and NOTES Freshman Orientation : Monday, August 1, 2016, 5:30 8:00 p.m. Doors Open at 5:30 p.m. Program starts at 6:00 p.m. Parents go to the Auditorium Students go to the Main Gym First Day of School : Wednesday, August 3, 2016 Open House: Locks & PE Clothes : Official Schedules : Athletics : Monday, August 8, 2016, 6:00 7:30 p.m. These items will be sold the first day of school in the Spirit Store. Shirts $12, Shorts $12, Locks $6. Cash or check only. ( Athletes who are signed up for a PE class during the day must purchase 2 locks one for their PE locker, and one for their athletic locker. All students must pick up a blue schedule in the cafeteria on the First day of school. Schedules will be available prior to A and 1st hours. PLEASE READ IF YOUR CHILD WILL BE INVOLVED IN ANY SPORT AT DRHS. Athletic Clearance Packets for the school year must be completed online through the RegisterMyAthlete.com website. Clearance packets will then be processed through the athletic office during the summer. All students planning to participate in the fall MUST have their packets completed online before the end of July. We encourage all athletes for winter and springs sports to complete their packets online prior to the start of school. If you are UNABLE to upload the physical forms, concussion form and birth certificate into the online registration system, please have your child bring them to the Athletic office and we will upload them for you. But please be aware that if you can upload these documents yourself, the clearances will go much faster for your child. If you have turned in a concussion certificate and birth
5 certificate in a prior year, we will upload those documents for you, you will only have to upload the four (4) pages of the new physical and the domicile statement. (Please make sure all empty spaces are filled in with correct information or your child will not be cleared to go out for sports). 9 Device Protection Plan (DPP) As part of the district s Digital Learning Project with the 1:1 technology initiative, parents have the option to enroll in the district s Device Protection Plan (DPP). Enrollment in the plan will minimize the potential repair and/or replacement parts associated with the device. By selecting this plan on the Technology Device User Agreement, (See Appendix C) you agree to pay an annual fee of $50 to be enrolled in the DPP. If a device becomes damaged, your student will take it to the school s library or front office and fill out the Student Device Repair Form (See Appendix D). The device will then be sent to Technology Services repair department to be fixed. If the device is lost, a fee of $200 will be assessed. If the device is stolen while at school, the incident must be reported by the student to their teacher or front office within 24 hours. If the device is stolen outside of school, the parent/guardian must file a police report within 48 hours. A copy of the report must be turned into the school s front office. Enrollment in the DPP does not begin until the annual $50 payment has been received. Enrollment in the DPP needs to be completed by September 2nd, 2016 for the current school year. Students enrolling in Gilbert Public Schools later in the year will be given 2 weeks after their official GPS enrollment date to enroll in the Device Protection Plan. What is covered under the plan? Accidental damage, such as cracked screens or cases, broken keyboards, etc. Battery replacement (if it is determined that the battery is malfunctioning) Damage of original power cord. Any lost power cords will be assessed a fee of $25. If the device is stolen, a police report must be filed within 48 hours. A copy of the report must be sent to the school s front office. Hardware issues (video cable, broken ports, speakers) Intentional damage to the device is NOT covered under the plan. Repair/Replacement Costs Parents and students who choose NOT to purchase the Device Protection Plan are responsible for 100% of all repair and replacement costs for the device. The following prices are estimates, as exact costs for each repair will be made upon further inspection of the damaged device. Device replacement $ Level 1 damage Level 2 damage includes broken keyboard, keys that pop off, get lost or cannot be reattached, cracked display or other major repairable damage to the device. includes breakage to the base or display enclosure and/or damage to the battery. $75.00 $50.00 Level 3 damage includes replacement of original power cord if damaged. $25.00

Most of the audio issues were not the students fault, but the USB ports sometimes were (not always). I ve pulled bits of paper and a small bead out of these. For devices not covered by the Device Protection Plan, each damage incident will incur a separate charge. Some repairs such as dead battery, video cables, chargers that don t work may be covered by the warranty. Should the student s device become damaged, the student will be provided a loaner device, if one is available, while their assigned device is being repaired. This loaner device may not be of equivalent performance or features. NOTE: All repairs must be made by GPS certified technicians.

ABOUT SENIOR ADS The La Cordillera yearbook is an all-color, 300 page minimum yearbook encompassing the memories of the school year. Parents, family, and friends all have the opportunity to submit advertisements for their graduate which reflects on the student s childhood and lifetime accomplishments. Advertisement photos and text must be school appropriate and present the students of Desert Ridge High School in a positive way; this includes dress code, language, etc. For information on appropriateness, please consult the school and district handbooks. SUBMISSION INFORMATION & AD DETAILS To ensure your student s advertisement is correct, please complete all of the information efficiently as possible; ads must be submitted in full- no exceptions. Read the following information and choose the best layout to fit your needs- we only offer what is displayed. In addition, each ad comes with a set amount of text to write a dedication to your graduate. Within two to three weeks of submitting the ad you will receive a call confirming your submission from a yearbook staff member. If you have any questions regarding submission, please the senior staff at drhs.yb.seniors@gmail.com. IMAGE FORMAT Digital images are preferred and should be submitted in JPEG format. Digital photos can be submitted via USB flash drive or . If you wish to send in hard-copy photos, please attach the photos to the order form in the same envelope. Please note any USB or hard-copy photos may not be returned. Please do not modify hard-copy picture size; do not cut your photos to fit the boxes on the ad layout- we will not be able to use the pictures. If there are photos you want in a specific picture box, please write the corresponding numbers from the chosen layout s picture boxes on the back of hard-copy photos in pencil. You may also name the photos (if on USB or ) with the corresponding numbers from the chosen layout s picture boxes. Please take into consideration the recommendations for picture orientation (horizontal v. vertical) on the layouts. We want to ensure photo content is not accidentally cropped out.
