1 Students & Parents, ATHLETIC/CO-CURRICULAR ONLINE REGISTRATION I hope you are having a great summer. It is hard to believe that the start of the fall sports season is only a few weeks away. With that being said, I am excited to announce that the online athletic registration is up and running. As always, HUHS students planning to participate in athletics for the school year MUST complete all required paperwork and pay the $120 participation fee before the first day of practice or tryouts. You can access these forms by signing into your Family Skyward account. Once in Skyward, choose Online Forms on the left hand side. I have attached a tutorial to this message to help guide you through this new process. You will need to download the WIAA Clearance Form (Physical) in step 2, you may also pick up a hardcopy of this form in the Athletic Office. All freshman or students new to HUHS must turn in the WIAA Clearance Form (Physical) signed by the athlete s doctor and parent/guardian. All other students must have a physical on file that is dated after 4/1/2016. The WIAA Clearance Form as well as the $ Co-Curricular fee should be turned in to the Athletic Department once all signatures have been obtained. The Athletic Department will verify that all forms and payments have been received and your athlete may pick up his/her Oriole Card in the Co-Curricular Office. Remember, a hardcopy of the WIAA Clearance Form (Physical) and fees MUST be turned in and paid to receive the Oriole Card. It is then the athlete s responsibility to turn the Oriole Card into his/her coach. This notifies the coach that the athlete has met all requirements to participate in practice/tryouts. Until an Oriole Card is issued and given to the coach, athletes will not be able to participate in practice/tryouts. No exceptions! Click on Make Athletic/Co-Curricular Fee Payment on the right of screen. Log in to Web Store to make fee payment.

2 Click here to log in to Family Access Select the student for whom you are completing the forms. Click on the link to fill out Online Forms. Click on Make Athletic/Co-Curricular Fee Payment on the right of screen. Log in to Web Store to make fee payment. (Please note that Athletic Fees must be paid before you can register your child for any sport or co-curricular activity.) Once you have completed the payment click Complete Step 1 and Move to Step 2.

3 Click Yes to complete the Concussion Form Agreement. Read all forms carefully and be sure to complete all signatures and dates When you have completed the form click Complete Step 2 and move to Step 3.

4 Click on the link to download the required Physician Statement. Have your child s Physician Complete this form and return to the Athletic office. After you have downloaded this form, click Complete Step 3 and move to Step 4. Click Yes to complete the Co-Curricular Permission Slip.

) If you have completed each step you will see the words completed, the date and time that the step was completed.

5 Complete all fields, signatures and dates. After you have downloaded this form, click Complete Step 4 and move to Step 5. (Please note: As you complete each step a green checkmark with the date and time will appear.) If you have completed each step you will see the words completed, the date and time that the step was completed. If you have any step that indicates not completed, you will need to go back to that step and complete it. Once all steps are completed click on Submit Athletic Forms. Your information and payment will be reviewed and confirmed by the Co-Curricular/Athletic Department. If all information and payments have been received your child will be added to the roster for the requested sport/co-curricular activity.
