3 1. Introduction This application enables user to enter details of various audits to be carried out for the vessels, in terms of ISM, ISPS and MLC audits. Filling Contract Review form is the very first step of this Audit process. 2. Process Flow 2.1 How to start the application Go to IRS-website ( & go to the Marine Section. Click on (ISM, ISPS & MLC Audit / Inspection Request). 2.2 Login Screen The Login Screen for ISM/ISPS/MLC Audit Request is shown as below: 2.3 Mode of Audit Requests The Audit Request can be done for Vessels Classed with IRS and Non-Classed Vessels. 4 2.3.1 Audit Request for Classed Vessels: If user selects Audit Request for Classed vessels then user s credentials (i.e. User s ID & Password) have to be entered. The end user will be given access to enter into the system for classed vessels on submission of correct credentials. Login button has to be clicked to enter the system. On typing wrong id or password, an error message will be displayed - You Have Entered Wrong Credentials. Please Check User name & Password. User Id and Password will be the same as for IRS Website to view the vessels Survey Status. After login into the system, the User/Owner will be given access to view his own vessels. Rest of the vessels will not be displayed in the Ship Name List on the Contract Review Form Audit Request for Non-Classed Vessels: For Audit Request of Non-Classed vessels ID & Password are not required. User is required to enter the ID & select the Audit Type. 2.4 Procedure to Fill Contract Review for ISM/ISPS/MLC Audit Request: Common Details Section As the user logins into the system, he ll find the first form as Common Details. This form enables user to fill all the common details of ISM, ISPS & MLC Audit requests. User can simultaneously apply for all the three audits. Select the type of Audits & other corresponding details on the form & click on <Proceed> button to further fill the ISM, ISPS & MLC Audits/Inspection Forms. Page 4 of 13

6 Note: Common Details Section for SMC, ISPS & MLC Audits can be filled simultaneously but DOC Audits have to be filled separately. Any other audit type cannot be combined with DOC Audits ISM Audit Request Page 1 & Page 2 have to be filled for ISM Audit Request. Fill both the pages & click on <Save> button to save the ISM Audit Request Data. On clicking <Print PDF> button, a pdf copy of entered data can be generated for user s reference. Page 6 of 13

8 Confirmation message will appear on successful completion of form ISPS Audit Request Page 1 & Page 2 have to be filled for ISPS Audit Request. Fill both the pages & click on <Save> button to save the ISPS Audit Request Data. On clicking <Print PDF> button, a pdf copy of entered data can be generated for user s reference. Page 8 of 13

10 Confirmation message will appear on successful completion of form MLC Inspection Request The screen-shot shown below represents MLC Inspection Form. User has to fill the required data on the screen. Click on <Save> button to save MLC Inspection Data. On clicking <Print PDF> button, a pdf copy of entered data can be generated for user s reference. 11 2.5.1 Uploading Documents: Onsaving either of the Contract Review Form, link to Upload Documents gets enable. Please ensure to fill all the Contract Review Forms (ISM / ISPS / MLC) before clicking <Upload Documents> Button. The following screen will appear on <Upload Documents> click. Page 11 of 13

12 This screen gives the combined checklists for ISM / ISPS & MLC Audits. Select the options as applicable. Upload the combined set of documents in zip format. Click on <Upload & Submit> button to submit the contract review form at HO. Page 12 of 13

13 2.5.7 General Validations: 1.) Fields marked with asterisk are mandatory on the screen. Audit Type, IR Number, Ship Name, Company Name, Proposed date of Audit, Audit Category, Company Contact Person & Proposed Place of Audit are mandatory fields. 2.) Proposed Date should be future date. 3.) Internal Audit Dates should be less than Proposed Audit Date. 4.) On Page2, Valid Till Date should be greater than Issue Date. 5.) Last Date of Periodical DOC Audit should be past date. 6.) SMC valid till date should be greater than SMC Issue date. 7.) Last Endorsement date should be past date. 8.) All the date fields should be in dd/mm/yyyy format. 9.) Uploading all necessary documents in zipped form is required for submitting the Online Contract Review form at Head Office. Page 13 of 13
